Longitudinal analysis of pupillometry according to type of pharmacotherapy in Parkinson's disease patients.

The aim of this study was to evaluate the effect of different types of pharmacotherapy on pupillometric parameters in PD patients. Dopamine was shown to induce mydriasis by excitation of alpha-adrenergic receptors at the dilator pupillae muscle. Pupil diameter may thus serve as an indirect measure of peripheral dopaminergic activity. A total of 63 PD patients were treated with different types of dopaminergic pharmacotherapy. Pupil examination was performed before the first application of dopaminergic pharmacotherapy and then after one year of treatment in the OFF state and in the ON state. The…

A case series of accidental xylazine intoxication in humans; Is there a role of naloxone as an antidote?

Xylazine is a type of sedative commonly used in veterinary medicine. It acts on the central alpha-2 receptor and suppresses norepinephrine r … uman injection with xylazine. Naloxone was administered in one of the cases. Acute hypertension and mydriasis were observed; however, no apparent reversal of toxidrome was seen. This finding reveals the question regarding the efficacy and benefit of naloxone usage in xylazine intoxication. General management remains supportive of care focusing on ventilatio …

Two patients with the heterozygous R189H mutation in ACTA2 and Complex congenital heart defects expands the cardiac phenotype of multisystemic smooth muscle dysfunction syndrome.

De novo heterozygous mutations changing R179 to histidine, leucine, or cysteine in the ACTA2 gene are associated with Multisystemic Smooth Muscle Dysfunction Syndrome (MSMDS). Characteristic hallmarks of this condition, caused only by these specific ACTA2 mutations, are congenital mydriasis (mid-dilated, non-reactive pupils), a large persistent ductus arteriosus (PDA), aortic aneurysms evolving during childhood, and cerebrovascular anomalies. We describe two patients, a 3-day-old newborn and a 26-year-old woman, with this unique mutation in association with a huge PDA and an aorto-pulmonary wi…

[Ophthalmologic complications and local anesthesia. Pathophysiology and types of eye complications after intraoral dental anesthesia, and clinical recommendations].

The present article reviews the different types of ophthalmologic complications following administration of intraoral local anesthesia. Since the first report by Brain in 1936, case reports about that topic have been published regularly in the literature. However, clinical studies evaluating the incidence of ophthalmologic complications after intraoral local anesthesia are rarely available. Previous data point to a frequency ranging from 0.03% to 0.13%. The most frequently described ophthalmologic complications include diplopia (double vision), ptosis (drooping of upper eyelid), and mydriasis …

Tournay's description of anisocoria on lateral gaze: reaction, myth, or phenomenon?

The year 1917 witnessed the opening of a great debate in neuro-ophthalmology, with Tournay's description of a phenomenon thought to be universal: anisocoria in lateral gaze, due to mydriasis of the abducting pupil and a minor miosis of the fellow one. Through many publications, this phenomenon was soon assessed and confirmed to be physiologic, while the cause of the pupil reaction in lateral gaze was extensively discussed. Cases of abolition and reversal of the phenomenon were also reported in specific clinical situations, such as neurosyphilis, and its study became part of the neuro-ophthalmo…

Coil embolization of an intracranial aneurysm in an infant with tuberous sclerosis complex: A case report and literature review.

BACKGROUND: Tuberous sclerosis (TS) is a multiorgan neurocutaneous syndrome. Vascular manifestations are often extracranial. There is a paucity of cases involving TS combined with intracranial aneurysms reported in the literature. As a result, treatment has not been well described. CASE DESCRIPTION: We report the case of a 13-month-old female infant with a prior diagnosis of TS that was found to have new onset of left eye ptosis, anisocoria, and papillary mydriasis indicative of left third cranial nerve palsy. A magnetic resonance angiogram (MRA) of the brain revealed a left internal carotid …

Suspected Vestia foetida poisoning in young goats.

CASE HISTORY: Two crossbred, castrated male goats, a 5-month-old and an 8-month-old, were observed ingesting Vestia foetida (Solanaceae). Later, the goats were seen standing splay-legged and apparently disoriented. CLINICAL FINDINGS: When examined, both goats were in sternal recumbency and had mydriasis; the younger goat had a diminished menace response. When the goats were made to stand, they were ataxic and had muscle fasciculations of the hindquarters and face. Both had halitosis consistent with the odour of crushed Vestia leaves. The animals were treated with a mixture of vitamins and int…

From Field to Fundus: A Peripheral Retinal Tear Following Football-Related Ocular Trauma.

Sports-related ocular trauma is an important cause of preventable visual morbidity, particularly in contact sports such as football (soccer). Although many injuries initially present with mild anterior segment findings, significant posterior segment pathology may coexist and can be easily overlooked. We report the case of a 17-year-old male football player who presented with left eye pain, blurred vision, and periorbital bleeding following blunt ocular trauma during a football match. Examination revealed traumatic mydriasis with anterior uveitis, subconjunctival hemorrhage with conjunctival la…
