Peroral Endoscopic Myotomy for the Treatment of Esophageal Diverticula: A Systematic Review and Meta-analysis.

BACKGROUND: There is limited evidence on the efficacy of peroral endoscopic myotomy (POEM) in patients with esophageal diverticula. AIMS: This meta-analysis aimed to assess the efficacy and safety profile of POEM in patients with Zenker (ZD) and epiphrenic diverticula. METHODS: With a literature search through August 2020, we identified 12 studies (300 patients) assessing POEM in patients with esophageal diverticula. The primary outcome was treatment success. Results were expressed as pooled rates and 95% confidence intervals. RESULTS: Pooled rate of technical success was 95.9% (93.4%-98.3%…

Peroral endoscopic myotomy is a safe and effective treatment modality for geriatric patients with achalasia.

BACKGROUND: Peroral endoscopic myotomy (POEM) is an appealing treatment for older patients, as it is minimally invasive but highly efficacious similar to surgical myotomy. However, there is a lack of systematic studies analyzing POEM outcomes in young (< 65 years) versus geriatric patients (≥ 65 years). Hence, we aimed to compare the safety and efficacy of POEM in young versus geriatric patients. METHODS: Records of all achalasia patients who underwent POEM at our institution between April 2014 and May 2019 were reviewed. Patients were categorized into two groups: young (< 65 years) and old (…

An evaluation of the complication risks following peroral endoscopic myotomy in patients on antithrombotic therapy.

BACKGROUND: Per oral endoscopic myotomy (POEM) is used to treat a variety of esophageal motility disorders and is associated with relatively few complications. However, complications in patients on antithrombotic therapy (AT) is not well-studied. We hypothesize AT patients have a higher risk of post-operative bleeding and 30-day complication rate compared to all other patients, even when these medications are held peri-operatively. METHODS: A single-institution retrospective review of a prospectively collected database of patients who underwent POEM procedures January 2011-July 2019 was perfo…

The Modern Age of POEM: the Past, Present and Future of Per-Oral Endoscopic Myotomy.

BACKGROUND: Per-oral endoscopic myotomy (POEM) has gained widespread enthusiasm amongst foregut specialists since its introduction in the mid 2000s as an effective and less invasive treatment option for achalasia. As more than 6000 POEM procedures have been performed to date throughout the world, we aim to summarize the history and current state of POEM in the treatment of esophageal motility disorders. METHODS: We performed a comprehensive review of the published literature focusing on the history and development of the POEM procedure, and its most current applications and outcomes. RESULTS…

Early outcomes of peroral endoscopic myotomy with fundoplication for achalasia cardia - Is it here to stay?

BACKGROUND: Peroral endoscopic myotomy with fundoplication (POEM-F) has garnered widespread attention since it tries to achieve an appropriate antireflux procedure during the same setting. The outcomes of POEM-F are currently unexplored. METHODS: Patients who underwent POEM-F at our center were retrospectively studied to assess its safety and early outcomes. RESULTS: Twenty patients [70% male; median age 32 years (range 25-41)], with a median symptom duration of 2.1 years (range 1.5-3.7) with achalasia were included. POEM-F procedure was completed in a median of 118 min (range 90-160), with …

Establishment of a per oral endoscopic myotomy program at a rural tertiary care center.

BACKGROUND: This study's purpose is to determine the application and effectiveness of a POEM program in the rural healthcare setting. Achalasia has a substantial impact on the lives of afflicted patients. Traditionally, a Heller myotomy with fundoplication has been the standard of care for treatment. In 2008, the first per oral endoscopic myotomy (POEM) was performed in Japan. Since 2017, our rural healthcare institution has performed approximately 60 POEMs. METHODS: An IRB approved, single-institution retrospective review of patient outcomes after POEM was performed along with prospective an…

Heller myotomy is the optimal index procedure for esophageal achalasia in adolescents and young adults.

BACKGROUND: There is limited and conflicting data on the optimal intervention for the treatment of achalasia in adolescents and young adults (AYA), Heller myotomy (HM), esophageal dilation (ED) or botulinum toxin injection (botox). The goal of this study is to determine the most appropriate index intervention for achalasia in the AYA population. METHODS: We completed a longitudinal, population-based analysis of the California (2005-2010) and New York (1999-2014) statewide databases. We included patients 9-25 years old with achalasia who underwent HM, ED or botox. Comparisons were made based o…

In vivo experience with peroral endoscopic myotomy: An essential activity for developing the technique in humans.

INTRODUCTION AND OBJECTIVES: Achalasia is the most widely studied esophageal motility disorder. No treatment has achieved completely satisfactory results. The laparoscopic Heller esophagomyotomy is currently the most accepted technique. With the advent of minimally invasive surgery, the appearance of peroral endoscopic myotomy (POEM) has promising results. The primary aim of our study was to perform peroral endoscopic esophagomyotomy in animal experimentation models to perfect the technique and later apply it to humans. The secondary aims were to evaluate the intraoperative and postoperative c…

Peroral Endoscopic Myotomy (POEM) in a Patient with Complete Heart Block-a Case Report.

UNLABELLED: The ATP-binding cassette, subfamily G, member 2 (ABCG2) transporter, or breast cancer resistance protein (BCRP), plays a crucial role in the absorption and distribution of various compounds, including anti-seizure medications (ASMs). ABCG2 genetic polymorphisms have been shown to affect the pharmacokinetics of several drugs, including anti-cancer drugs and statins. Clinical studies suggested an association between the ABCG2 variations and ASMs pharmacokinetics. Thus, the aim of the present study was to evaluate the effect of the coding ABCG2 variants, 742T > G and 886G > C on the A

Thoracoscopy-assisted Heller myotomy for the treatment of achalasia: results of a minimally invasive technique.

BACKGROUND: Several surgical methods have been described to treat achalasia with a recent trend toward utilizing minimally invasive techniques to perform a myotomy. Since 1998 our institution has utilized a minimally invasive thoracoscopy-assisted technique (ThAM) that allows a myotomy to be performed under direct visualization. METHODS: From 1992 to 2002, 57 patients underwent transthoracic Heller myotomy at our institution. Thirty-eight patients (67%) who underwent ThAM were reviewed and compared with 19 (33%) who previously underwent myotomy through a standard open left thoracotomy (OM). …

The value of intestinal myotomy and myectomy in improving the reservoir capacity of the endorectal pull-through.

In laboratory models of massive small bowel resection and colectomy, intestinal myotomy has been shown to decrease stool frequency and malabsorption. Using physiologic and anatomic parameters of gastrointestinal function, we assessed the ability of three types of ileal myotomies to improve outcome after total abdominal colectomy, mucosal proctectomy, and endorectal pull-through (ERPT) without an enteric reservoir. Twenty puppies underwent ERPT. These dogs were randomly assigned to three experimental groups or a control group consisting of animals without a myotomy. The myotomies were performed…

[The importance of x-ray cinematography of deglutition for indicating the need for myotomy of the pharyngoesophageal sphincter].

Fourteen patients were examined one to four years after cricopharyngeal myotomy that had been carried out because of dysfunction of the pharyngo-esophageal sphincter. Twelve patients were examined radiologically. Eleven of the 14 patients were clinically improved or cured. In two patients who were not improved, the underlying condition was a polymyositis. The other patients suffered from an idiopathic dysfunction. Because of the small numbers involved, no detailed statistical analysis was carried out. Nevertheless, our results indicate that: Cricopharyngeus myotomy produces marked improvement …

Symptom duration is associated with esophageal remodeling in achalasia treated by peroral endoscopic myotomy.

Symptom duration may reflect cumulative disease burden in achalasia, but its relation to objective recovery after peroral endoscopic myotomy (POEM) remains unclear. In this single-center prospective observational cohort, 68 eligible patients treated with POEM were stratified by preoperative symptom duration (< 3 years, 3-10 years, and > 10 years). Clinical success was defined as an Eckardt score ≤ 3 at 3 and 6 months. Secondary outcomes included perioperative safety, persistent symptoms, high-resolution manometry, barium esophagram morphology, and subtype distribution. All patients underwent t…
