The short video lecture for robotic bilateral axillo-breast approach to lateral neck lymph node dissection.

Lateral neck lymph node dissection (LND) along with total thyroidectomy is the standard treatment for thyroid cancer patients with metastases to the lateral neck lymph nodes. In general, lateral neck LND removes lymph nodes located at levels II to V ipsilateral to the thyroid cancer and preserves the spinal accessory nerve, internal jugular vein, and sternomastoid muscle during surgery. This video article was written to introduce the robotic bilateral axillo-breast approach for lateral neck LND and to describe the surgical method.

The effect of thymectomy during central neck dissection in papillary thyroid carcinoma: a case-controlled study.

Central lymph-node dissection (CND) as part of total thyroidectomy is recommended in the treatment of papillary thyroid cancer. CND with thymus resection for achieving more oncological clearance is suggested in guidelines, but the benefits of this technique are still unclear due to the risk of parathyroid glands injury and postoperative hypocalcemia. The aim of this study is to evaluate the risk and benefits of thymectomy in CND with total thyroidectomy. We retrospectively reviewed the records of 188 patients with total thyroidectomy and CND. Participants were divided into 110 patients with CN

Neck dissection prolongs survival in patient with stage IVC hypopharyngeal carcinoma with mixed responses to nivolumab.

Surgical intervention aids in maintaining quality of life in patients with recurrent metastatic head and neck carcinoma with mixed responses … administration was resumed, and this patient is presently alive and disease-free. Immediately after neck dissection, the LNs that responded and those that did not respond to nivolumab were separated and evaluated. The LNs that responded well to nivolumab presented with prominent interstitial fibrosis. Conversely, in LNs that enlarged after nivolumab, significant …

Sinus Bradycardia after Extensive Neck Dissection and Total Thyroidectomy in a Pediatric Patient: A Case Report.

Postoperative complications after total thyroidectomy with extensive neck dissection in thyroid malignancies are well documented in the current literature. However, sinus bradycardia as a postthyroidectomy complication is a rare phenomenon and, to the best of our knowledge, few studies have identified it as a perioperative condition. In our study, we report a case of 9-year-old boy with papillary thyroid carcinoma, who underwent total thyroidectomy and bilateral neck dissection. Postoperatively, the surgery was complicated by initial vocal cord paresis and chyle leak. The patient also suffered…

Technique of flap elevation for robot assisted selective neck dissection via retroauricular approach: a surgeon's guide.

The retroauricular approach is being increasingly used in surgeries of head and neck in an attempt to avoid a disfiguring scar over the face or neck. The elevation of flap correctly is of paramount importance. The lateral to medial (vis-a vis posterior to anterior) approach can be challenging as the anatomical relations guiding the surgeon have a different orientation. The surgeons need to revisit the anatomy of the face and neck and identify reliable anatomical structures that will act as "new" landmarks to ensure proper dissection. 14 cases of robotic selective neck dissections and from Janu…

Comparison of spinal accessory dysfunction following neck dissection with harmonic scalpel and electrocautery - A randomized study.

INTRODUCTION: Harmonic scalpel is being increasingly used in neck dissection as alternative to conventional electro-cautery for achieving haemostasis. Use of harmonic scalpel has been shown to significantly reduce intra operative blood loss and intra operative time in neck dissection. But how safe is it with regards to nerve injury (spinal accessory nerve and other nerves) during neck dissection. We intended to study the spinal accessory nerve injury during neck dissection by both harmonic scalpel and electro cautery technique and compared postoperative recovery of shoulder function after neck…

Predictive factors and pattern of locoregional recurrence after prophylactic central neck dissection in papillary thyroid carcinoma.

BACKGROUND: Prophylactic central neck dissection (pCND) at the time of the total thyroidectomy (TT) remains controversial in clinically nodal-negative (cN0) papillary thyroid carcinoma. Our study was designed to examine the predictive factors and pattern of locoregional recurrence (LRR) after pCND in the context of the postoperative stimulated Tg (sTg) level. METHODS: A total of 341 patients who underwent TT and unilateral pCND were analyzed. Patients with an identifiable lesion on ultrasonography or whole-body scan within 6 months of surgery were excluded. LRR was defined as an identifiable …

A cost-utility analysis for prophylactic central neck dissection in clinically nodal-negative papillary thyroid carcinoma.

BACKGROUND: Although prophylactic central neck dissection (pCND) may reduce future locoregional recurrence after total thyroidectomy (TT) for low-risk papillary thyroid carcinoma (PTC), it is associated with a higher initial morbidity. We aimed to compare the long-term cost-effectiveness between TT with pCND (TT+pCND) and TT alone in the institution's perspective. METHODS: Our case definition was a hypothetical cohort of 100,000 nonpregnant female patients aged 50 years with a 1.5-cm cN0 PTC within one lobe. A Markov decision tree model was constructed to compare the estimated cost-effectiven…

[Clinical features of unruptured vertebral artery dissection presenting as isolated occipital headache and/or neck pain].

Vertebral artery dissection(VAD)presenting as isolated occipital headache and/or neck pain is being increasingly diagnosed because of the development of magnetic resonance imaging(MRI). While a majority of the patients diagnosed with this condition shows a favorable prognosis, the pain may be a predictor of fatal stroke in some patients. We aimed to find out the features of headache with VAD, identify the clinical manifestations indicative of VAD, and determine the ideal diagnostic approach to this condition to avoid fatal stroke. We reviewed medical records of 41 consecutive patients who show

Elective neck irradiation does not improve regional control or survival in pT3-T4N0 oral cavity SCC after high-yield neck dissection: A retrospective multicenter analysis.

OBJECTIVES: In pT3-T4 oral cavity squamous cell carcinoma (OCSCC), standard management includes surgical resection with elective neck dissection (END), frequently followed by postoperative radiotherapy (PORT). While PORT is commonly administered for advanced primary tumors, the benefit of elective neck irradiation (ENI) in patients with a pathologically node-negative (pN0) neck after adequate high-yield END remains uncertain. This study evaluated whether ENI improves regional control or overall survival (OS) in this setting. METHODS: We conducted a retrospective multicenter cohort study of ad…
