INTRODUCTION: Maxillo-mandibular osteonecrosis is exceptional outside a context of cervico-facial radiotherapy. Bisphosphonates are non-metabolized pyrophosphate analogues which inhibit osteoclastic activity. Bisphosphonates are prescribed for the treatment of malignant hypercalcemia, osteolysis associated with metastatic bone disease, Paget's disease and osteoporosis. Maxillomandibular osteonecrosis with bisphosphonates can be observed in 1/10000 patients, but is probably underestimated due to lack of dental examination.

The authors present the review of the literature on the role of tumor necrosis factor alpha (TNF alpha) in scleroderma systematica (SS). It is shown that TNF alpha participates in activation of vascular endothelium, regulation of immune response and metabolism of the connective tissue by modulation of fibroblastic function. SS patients exhibit a systemic and local rise of TNF alpha content. This rise contributes to SS progression, development of fibrosing alveolitis and skin fibrous alterations in Raynaud's syndrome.

We present a patient with bilateral scalp necrosis caused by giant cell arteritis (temporal arteritis). A 67-year-old woman, who had been treated with 5 mg of oral prednisolone every other day for polymyalgia rheumatica, developed painful egg-sized regions of necrosis on both of her temples. PURPOSE: To report a side effect of epidural corticosteroid injections for back pain.
DESIGN: Case series.
METHODS: Review of clinical charts and photographs.
SETTING: Private retina practice.
RESULTS: Two patients developed acute retinal necrosis syndrome following epidural corticosteroid injections for back pain. Referral was delayed in one patient. One patient developed bilateral secondary rhegmatogenous retinal detachment, and both developed secondary macular pucker.

OBJECTIVE: To report a case of necrosis of the glans penis caused by necrotizing granulomatous arteritis, an uncommon form of presentation of this vasculitis that basically affects the upper and lower respiratory tracts and glomerulus. Although bile duct injuries are common among the complications of laparoscopic cholecystectomy, hepatic vascular injuries are not well described. Between January 1990 to December 1999, 83 patients with bile duct injuries have been referred to our clinic. Two of them had liver necrosis due to hepatic arterial occlusion. In this study we wanted to demonstrate the relationship between the fracture of the femoral neck and femoral head necrosis. For this purpose we reviewed the new literature, the dates of the ASIF documentation and some cases from the Davos Hospital. Head necrosis appears in all types of fractures and is independent of the implant. We can find femoral head necrosis in all age groups but the risk is higher the younger the patient. The necrosis can develop many years after the accident. There are not enough long-term studies to explain this phenomenon.
