The Dark Side of the Sunshine Vitamin: A Case of Acute Renal Failure and Hypercalcaemia From Vitamin D Overconsumption.

Vitamin D toxicity is a rare but potentially serious condition that can lead to severe hypercalcaemia and subsequent complications, such as acute renal failure, nephrocalcinosis, congestive heart failure and coronary calcium deposition, resulting in myocardial injury. We present a case of a 76-year-old male patient who developed severe hypercalcaemia and acute kidney injury due to excessive long-term over-the-counter vitamin D supplementation. The patient was initially treated with aggressive intravenous fluids and furosemide. However, a failed effective response prompted the addition of pamid…

Atypical clinical presentation of distal renal tubular acidosis: a case report registered in Amazonas, Brazil.

We report an unusual case of a 24-year-old girl with a history of recurrent hypokalemic paralysis episodes and skin lesions on the lower limbs and buttocks, both of which had an acute evolution. In subsequent investigations, the patient also had nephrocalcinosis, nephrolithiasis, hyperchloremic metabolic acidosis and persistent alkaline urinary pH. The findings were consistent with distal renal tubular acidosis as the cause of hypokalemic paralysis. Clinical findings, immunological tests and the result of skin biopsy suggested primary Sjögren's syndrome as an underlying cause. The patient deve…

The diagnostic role of urinary N-acetyl-beta-D-glucosaminidase (NAG) activity in the detection of renal tubular impairment.

The kidney function can be assessed by a number of methods. The urinary excretion of enzymes, in particular N-acetyl-beta-D-glucosaminidase (NAG), is considered a relatively simple, cheap, fast and non-invasive method in the detection and follow-up of renal tubular function under various conditions. The determination of urinary NAG provides a very sensitive and reliable indicator of renal damage, such as injury or dysfunction due to diabetes mellitus, nephrotic syndrome, inflammation, vesicoureteral reflux, urinary tract infection, hypercalciuria, urolithiasis, nephrocalcinosis, perinatal asph…

Primary hyperparathyroidism presenting as a palatal brown tumor.

Only 2% of all primary hyperparathyroidism cases occur in patients below the age of 30 years. Owing to the improved methods of blood analysis in the last 2 decades, most of the recent cases of primary hyperparathyroidism are diagnosed early and asymptomatically. This makes advanced disease with bone lesions extremely rare these days. A case of a 25-year old female patient with previously undiagnosed primary hyperparathyroidism who presented first with palatal swelling is reported. Further investigations revealed parathyroid adenoma, nephrocalcinosis, and extensive bone lesions involving the ma…

Life-threatening situations in sarcoidosis.

Life-threatening situations in sarcoidosis are extremely rare. They may be due to failure of vital organs--lungs, heart, kidney, liver and brain--and usually due to irreversible fibrosis. Respiratory failure follows irreversible pulmonary fibrosis and the development of cor pulmonale. Cardiac sarcoidosis is more sinister for it may be silent, ill-recognised with sudden death or high morbidity. It needs sophisticated techniques to uncover this latent iceberg. Renal failure may be due to granulomatous interstitial nephritis and/or nephrocalcinosis. Hepatic failure is due to intrahepatic cholesta…

Phenotypic characterization of X-linked hypophosphatemia in pediatric Spanish population.

BACKGROUND: X-linked hypophosphatemia (XLH) is a hereditary rare disease caused by loss-of-function mutations in PHEX gene leading tohypopho … lize serum concentrations of phosphate. Eleven patients had mild hyperparathyroidism and 8 patients nephrocalcinosis. CONCLUSIONS: This study shows that growth retardation and rickets were the most prevalent clinical manifestations at diagnosis in a large series of Spanish pediatric patients with XLH confirmed by mutations in the PHEX gene. Traditional treatment …

Dent's disease.

Dent's disease is a renal tubular disorder characterized by manifestations of proximal tubule dysfunction, including low-molecular-weight proteinuria, hypercalciuria, nephrolithiasis, nephrocalcinosis, and progressive renal failure. These features are generally found in males only, and may be present in early childhood, whereas female carriers may show a milder phenotype. Prevalence is unknown; the disorder has been reported in around 250 families to date. Complications such as rickets or osteomalacia may occur. The disease is caused by mutations in either the CLCN5 (Dent disease 1) or OCRL1 (…

Renal and pancreatic calcification in rheumatoid arthritis with Sjögren's syndrome.

We describe a female patient who developed seropositive rheumatoid arthritis (RA) at the age of 12 years. After 10 years her disease was complicated by Sjögren's syndrome (SS) and distal (type 1) renal tubular acidosis (RTA). Seven years later she was noted to have nephrocalcinosis. At the age of 32, investigation of a short history of weight loss and abdominal pain revealed a benign gastric ulcer and chronic calcific pancreatitis. We believe she is the first patient with RA and SS in whom complicating renal and pancreatic calcification have been reported. Her case emphasizes the good prognosi…

Primary hyperoxaluria: report of a patient with livedo reticularis and digital infarcts.

Primary hyperoxaluria encompasses 3 rare genetic disorders of glyoxylate metabolism characterized by excessive urinary excretion of oxalic acid, resulting in oxalosis. Patients typically have recurrent calcium oxalate nephrolithiasis and nephrocalcinosis, leading to chronic renal failure and death from uremia. Oxalate can deposit in extrarenal sites such as the heart, walls of arteries and veins, bone, and skin. We report a patient who presented with acute renal failure and later experienced livedo reticularis and peripheral gangrene before the diagnosis of primary hyperoxaluria was establishe…

Hypophosphataemic Rickets: Similar Phenotype of Different Diseases.

Hypophosphataemic rickets (HR) is a group of rare disorders caused by excessive renal phosphate wasting in which the participation of fibroblast growth factor 23 (FGF23) can be prominent. These diseases pose therapeutic challenges with important consequences for growth and bone development in childhood, with higher risk of fractures and poorer bone healing, dental problems, and nephrolithiasis or nephrocalcinosis. In some cases, the diagnostic delay can be very long; laboratory findings and an exhaustive anamnesis could help distinguish between various pathologies, and FGF23 values-although cu…

[Stage I-III pulmonary sarcoidosis complicated by nephrogenic (vasopressin-resistant) form of diabetes insipidus].

Literature data and original experience of the authors with 6890 cases of respiratory sarcoidosis (stage I-III) suggest that diabetes ipsipi … ment the nephrogenic form of the pathogenesis is caused by defects in calcium metabolism leading to nephrocalcinosis with low sensitivity of renal tubular receptors to ADH. Adiurecrine treatment is unefficient. It is recommended to use chlorpropamide which raises sensitivity of the tubules to ADH.

[Acute renal failure caused by hypercalcemia].

BACKGROUND: Hypercalcaemia may cause acute renal failure. MATERIAL AND METHODS: We present and discuss two patients with acute renal failure caused by hypercalcaemia. RESULTS: Patient no. 1 was treated with too high doses of vitamin D for hypoparathyroidism. Patient no. 2 had been taking extremely high doses of calcium carbonate for dyspeptic pain. Volume depletion and renal vasoconstriction are the mechanisms that lead to acute renal failure. Long-lasting hypercalcaemia will lead to calcium deposits in the kidneys (nephrocalcinosis), which is known to cause chronic renal failure. INTERPRET…

Pediatric Dent disease presenting with rickets and end-stage renal disease: case report and literature review.

Dent disease is a rare disease with proximal renal tubular dysfunction, and is characterized by low-molecular-weight proteinuria, hypercalciuria, nephrocalcinosis, nephrolithiasis, and chronic kidney disease. Renal failure slowly progresses and end-stage renal disease may develop in the late decades of life. We report a case of a 15-year-old boy who was diagnosed with Dent disease 1 with a CLCN5 truncating mutation. The patient presented with arthralgia and rickets at the onset of Dent disease and he was diagnosed with end-stage renal disease at the age of 15 years. His only symptoms were arth…

[Updates on rickets and osteomalacia: the therapy for FGF23 related rickets].

FGF23 related rickets/osteomalacia was mainly composed of XLH, ADHR, ARHR1, ARHR2 and tumor induced rickets/osteomalacia. Although the mechanism for increasing serum FGF23 levels was different from disease to disease, therapies for these patients were basically the same ; pharmacological doses of active vitamin D and phosphate administration. It is noted that phosphate therapy is associated with the occurrence of secondary hyperparathyroidism and nephrocalcinosis. Moreover, recent evidences indicated that these combination therapies increased serum FGF23 levels. On the basis of these data, it …

Multiple neonatal endocrinopathies in McCune-Albright syndrome.

Two cases of McCune-Albright syndrome (MAS) are reported who presented in the neonatal period with profound failure to thrive, cardio-respiratory distress, precocious puberty and Cushing's syndrome for which both underwent bilateral adrenalectomy. Both girls had also bilateral nephrocalcinosis; in one case that may have been attributed to Cushing's syndrome, but in the second case the cause remained obscure with no obvious abnormality of calcium metabolism. The first girl had hydrocephalus which is uncommon in this condition and the second girl still failed to thrive at the age of 6 years, des…
