Neuromuscular Responses of the Cranio-Cervico-Mandibular System During Professional "110th Targa Florio" Rally Competitions: An Electromyographic Study in Real-World Racing Conditions.

Background/Objectives: Professional motorsport imposes substantial biomechanical, postural, and cognitive demands that require continuous neuromuscular adaptations to ensure vehicle control and head stabilization during competition. Although the cranio-cervico-mandibular system may contribute to these adaptive responses, its role in rally performance remains poorly understood. This study aimed to investigate neuromuscular adaptations of the cranio-cervico-mandibular system in professional rally drivers under real-world racing conditions and to explore the relationship between masticatory muscl

Characterising gait in paediatric neuromuscular disorders: an observational study of spatio-temporal gait in a clinical cohort.

AIM: Few studies have characterised the relationship between disease and gait function in children and young people with rare neuromuscular disorders (NMDs). This study aimed to describe the relationship between disease and gait in a large paediatric cohort from a neuromuscular outpatient clinic. METHODS: A prospective, cross-sectional study of gait in independently ambulant children and young people aged 4-21 years with a clinical or genetically confirmed NMD. Participants traversed an electronic walkway barefoot and in footwear at self-selected and fast pace. Analysis of disease included a

SEP Montage Variability Comparison during Intraoperative Neurophysiologic Monitoring.

Intraoperative monitoring is performed to provide real-time assessment of the neural structures that can be at risk during spinal surgery. Somatosensory evoked potentials (SEPs) are the most commonly used modality for intraoperative monitoring. SEP stability can be affected by many factors during the surgery. This study is a prospective review of SEP recordings obtained during intraoperative monitoring of instrumented spinal surgeries that were performed for chronic underlying neurologic and neuromuscular conditions, such as scoliosis, myelopathy, and spinal stenosis. We analyzed multiple mont

Anesthetic challenges in a patient with Hirayama disease with quadriparesis and autonomic dysfunction undergoing cervical spine surgery.

Hirayama disease is a rare neurological disorder, characterized by muscular atrophy of the distal upper extremities. The occurrence of spast … rugs, leading to histamine release. The use of multimodal monitoring including bispectral index and neuromuscular monitoring is crucial to prevent delayed recovery. Anticipation and management of exaggerated hypotension in response to anesthetic induction agents and prone position is the key to a successful outcome in patients with autonomic dysfunction.

Auditory evoked responses in the management of acutely brain-injured children and adults.

In recent years, the role of auditory evoked responses (AERs) in the intensive care unit (ICU) setting has expanded dramatically for both pediatric and adult brain-injured patient populations. AERs have unique value in early identification and evaluation of peripheral auditory dysfunction that can result directly from head trauma or as a consequence of intensive medical therapy (such as prolonged intubation and ototoxic drugs). AERs can also be applied in evaluating and monitoring neurologic status during the acute period following a severe brain injury, similar to their common intraoperative

Response to rocuronium and its determinants in patients with myasthenia gravis: A case-control study.

BACKGROUND: Several previous studies using univariate analysis have suggested that the pre-anaesthetic train-of-four (TOF) ratio, concentration of anti-acetylcholine receptor (AChR) antibodies and the presence of preoperative generalised muscular involvement are determinants of an increased response to neuromuscular blocking agents (NMBAs) in patients with myasthenia gravis. However, the determinants of the response of patients with myasthenia gravis to rocuronium, which is expected to be used more frequently since the advent of sugammadex, have not been studied. OBJECTIVES: To clarify whethe

Investigating the clinical use of structured light plethysmography to assess lung function in children with neuromuscular disorders.

BACKGROUND: Children and young people with neuromuscular disorders (NMD), such as Duchenne Muscular Dystrophy (DMD), develop progressive respiratory muscles weakness and pulmonary restriction. Pulmonary function monitoring of the decline in lung function allows for timely intervention with cough assist techniques and nocturnal non-invasive ventilation (NIV). NMD may find the measurement of lung function difficult using current techniques. Structured Light Plethysmography (SLP) has been proposed as a novel, non-contact, self-calibrating, non-invasive method of assessing lung function. The overa

Reframing liver test abnormalities after AAV gene therapy: a mechanistic diagnostic framework.

Hepatotoxicity remains one of the most frequent and clinically important safety concerns in adeno-associated virus (AAV) gene therapy. Yet the laboratory patterns observed after vector administration are biologically complex and often difficult to interpret. Alanine aminotransferase (ALT) and aspartate aminotransferase (AST) elevations are non-specific, as they may arise from AAV-induced innate and adaptive immune recognition of vector capsid, endoplasmic reticulum stress, mitochondrial strain, cholestasis, or background muscle turnover, especially in neuromuscular disorders where ALT and AST

Risk factors and outcomes of bloodstream infection from a urinary source in kidney transplant recipients.

BACKGROUND: Bacteriuria is common among kidney transplant recipients (KTR). Risk factors and outcomes associated with bloodstream infection due to a urinary source (BSIU) in KTR are poorly understood. METHODS: This single center case-control study from 2010 to 2022 compared KTR with BSIU to those with bacteria without bloodstream infection (BU). Multivariable logistic regression identified BSIU risk factors, and Cox models assessed its impact on graft failure. RESULTS: Among 3435 patients, who underwent kidney transplantation at Emory Hospital, 757 (22%) developed bacteriuria, among whom 142

Not just small adults-practical pearls in pediatric anesthesia: a narrative review.

Pediatric anesthesia presents unique challenges due to children's distinct physiological and anatomical characteristics, including variations in drug metabolism, airway structure, and respiratory and circulatory regulation. Despite significant advances in patient safety that have reduced anesthesia-related mortality over recent decades, the declining pediatric population has made specialized training and clinical practice increasingly difficult. This narrative review addresses practical aspects of pediatric anesthesia, emphasizing patient monitoring, airway management, and recent clinical adva

Evolving compartment syndrome detected by loss of somatosensory- and motor-evoked potential signals during cervical spine surgery.

Neurologic injury is a rare but devastating complication of spinal surgery that can result in mild sensory to severe motor deficits. Surgeons increasingly use electrophysiological spinal cord function monitoring, including somatosensory- and motor-evoked potentials, intraoperatively to provide information about spinal cord function, aid in surgical decision making, improve outcomes, and reduce complication rates. By providing real-time information about the dorsal and anterior motor column function, somatosensory- and motor-evoked potentials signals allow surgeons to reverse noticeable changes

Examining the Relationship Between Changes in Lower-Limb Isometric Strength and Subsequent Physical Training Outputs in Elite Australian Rules Football Players.

BACKGROUND: Neuromuscular assessments are commonly used to monitor athletes and inform decisions regarding readiness to train and compete. However, the extent to which changes in neuromuscular performance may influence subsequent training or match-play outputs remains unclear, limiting their practical utility. Accordingly, further investigation is required to determine whether these assessments capture neuromuscular changes that meaningfully influence subsequent physical output. OBJECTIVES: This study aimed to examine the acute relationship between changes in pretraining isometric strength an

Injectable Biodegradable Non-Hermitian Biotelemetric System Enabling Early Diagnosis of Acute Compartment Syndrome.

Acute compartment syndrome (ACS) requires timely diagnosis, delayed diagnosis can cause irreversible neuromuscular injury, limb loss, and even death, whereas current diagnosis still relies on subjective examination and intermittent invasive pressure measurements. Here, we report an injectable, biodegradable biotelemetric implant for accurate, continuous wireless monitoring of intracompartmental pressure (IcP), and further establish a predictive model that converts pressure-duration signals into quantitative assessment of ACS progression, providing a clinically relevant framework for diagnosis

All-in-one wearable drug efficacy assessment systems for bulbar muscle function using amyotrophic lateral sclerosis animal models.

Preclinical studies are crucial for developing amyotrophic lateral sclerosis drugs. Current FDA-approved drugs have been created by monitoring limb muscle function and histological analysis of amyotrophic lateral sclerosis model animals. Drug candidates for this disease have yet to be tested for bulbar-onset type due to the limitations of traditional preclinical tools: excessive animal use and discrete detection of disease progress. Here, our study introduces an all-in-one, wireless, integrated wearable system for facilitating continuous drug efficacy assessment of dysphagia-related muscles in

Pathophysiology and Treatment of Severe Traumatic Brain Injuries in Children.

Traumatic brain injuries (TBIs) in children are a major cause of morbidity and mortality worldwide. Severe TBIs account for 15,000 admissions annually and a mortality rate of 24% in children in the United States. The purpose of this article is to explore pathophysiologic events, examine monitoring techniques, and explain current treatment modalities and nursing care related to caring for children with severe TBI. The primary injury of a TBI is because of direct trauma from an external force, a penetrating object, blast waves, or a jolt to the head. Secondary injury occurs because of alteration

General anesthesia with cisatracurium and sevoflurane for a patient with primary carnitine deficiency receiving L-carnitine therapy: A case report.

RATIONALE: Lipid storage myopathies (LSMs) are a series of genetic disorders of lipid metabolism predominantly affecting muscle. The low incidence and lethal properties of this disease make anesthesia experience limited in such patients. Among all etiologies of LSMs, primary carnitine deficiency (PCD) is now considered highly treatable by early administration of L-carnitine, though it remains unclear whether L-carnitine is effective enough to protect diseased muscle against conventionally used neuromuscular blocking agents (NMBAs) during general anesthesia. Currently, no data are available con
