Recognition of the presence and severity of newborn jaundice by parents, nurses, physicians, and icterometer.

OBJECTIVE: To determine how well parents, nurses, physicians, and an Ingram icterometer can detect the presence and the severity of jaundice in newborns. SETTING: Normal newborn nursery in a 340-bed teaching hospital. PATIENTS OR OTHER PARTICIPANTS: Nurses and physicians caring for nursery infants and parents of the infants. INTERVENTIONS: Physicians and nurses examining newborns documented whether they detected jaundice in the infants and, if so, the estimated bilirubin level and the extent of cephalocaudal progression of the jaundice. An assistant taught the parents how to examine the inf

Hyperbilirubinemia in otherwise healthy infants.

Treatment of hyperbilirubinemia in otherwise healthy infants is controversial, since hyperbilirubinemia in the absence of hemolytic disease, hepatobiliary disease or prematurity is not clearly associated with kernicterus or other long-term morbidity. Phototherapy, exchange transfusion and the withholding of breast milk are the only treatments that have proved effective. Home phototherapy may be useful in treating infants for whom hospitalization is otherwise unnecessary.

Congenital Central Hypoventilation Syndrome: A Case-Based Learning Opportunity for Neonatal Clinicians.

Congenital central hypoventilation syndrome (CCHS) is a rare and sporadic neurocristopathy characterized by alveolar hypoventilation and autonomic nervous system dysfunction. CCHS manifests quickly after birth, initially as respiratory distress. Mortality risk is estimated at 38 percent, with a median age of death of three months of age. A timely and accurate diagnosis is critical. Genetic testing for PHOX2B gene mutations is necessary to confirm the diagnosis; however, laboratory turnaround time often imposes an additional 7-14-day waiting period on an often anxious family. Neonatal clinician

Digital Simulation Improves, Maintains, and Helps Transfer Health-Care Providers' Neonatal Resuscitation Knowledge.

Purpose: To safely care for their newborn patients, health-care professionals (HCP) must undergo frequent training to improve and maintain neonatal resuscitation knowledge and skills. However, the current approach to neonatal resuscitation simulation training is time and resource-intensive, and often inaccessible. Digital neonatal resuscitation simulation may present a convenient alternative for more frequent training. Method: Fifty neonatal HCPs participated in the study (44 female; 27 nurses, 3 nurse practitioners, 14 respiratory therapists, 6 doctors). This study was conducted at a tertiary

Part 2: The cardiovascular system and congenital heart disease.

Midwives are increasingly performing the examination of the newborn. In the second of a four-part series, this article considers the importance of the cardiovascular examination in the screening process. The significance of history taking, knowledge of risk factors and auscultation of the heart will be explored. The necessity for early detection and treatment of congenital cardiac abnormalities, along with the prerequisite referral pathways that the Newborn infant physical examination (NIPE) requires will also be highlighted.

Nonimmune hydrops fetalis.

Nonimmune hydrops fetalis (NIHF) is a term that defines an edematous fetus that does not have erythroblastosis fetalis from isoimmunization. All of the other reasons that a fetus develops NIHF are included in this category. The causes of NIHF range from diseases to structural anomalies to genetic abnormalities. The mere presence of NIHF is a poor prognostic indicator, even with the etiology unknown. Some causes of NIHF are treatable with intrauterine therapies. These currently include cordocentesis to administer intrauterine transfusions or medications and fetal surgery. Prenatal testing to es

The Effect of Sexual Abuse and Prenatal Substance Use on Successful Breastfeeding.

Barriers to breastfeeding in women with substance use disorders (SUDs) often exist. Neonatal abstinence syndrome-related feeding difficulties, maternal SUD-related maladaptive behaviors, and psychological comorbidities can adversely affect breastfeeding. A neglected barrier that frequently occurs in women with SUDs is a history of sexual abuse. It is important that nurses and providers understand each maternal and/or infant factor that can affect the breastfeeding course to assist effectively with lactation support for these frequently misunderstood dyads.

Lesions requiring wound management in a central tertiary neonatal intensive care unit.

BACKGROUND: Most of the skin disorders that occur in neonatal intensive care units are due in part to the immaturity and vulnerability of the neonatal skin. Various iatrogenic diagnostic and therapeutic procedures are also conducive to iatrogenic damage. This study was to review the neonates admitted to our neonatal intensive care unit who needed wound management, and to assess the most common skin injuries and wounds, and their aetiology. METHODS: Data were extracted from medical records of neonates who needed wound management in our Neonatal Intensive Care Unit between January 31, 2012 and

Effects of maternal hypocaloric diet feeding on neonatal rat brown adipose tissue.

Hypocaloric diet (control diet diluted 1:1 with cellulose) feeding during pregnancy caused a reduction in the body weight of rat pups at birth whereas the main parameters of brown adipose tissue composition and thermogenic activity were unaffected. When prenatally underfed rat pups were nursed by untreated dams eating control diet during lactation, early (day 4 of life) and late (day 13 of life) neonatal body weight was rehabilitated and brown adipose tissue remained essentially unchanged. When prenatally underfed rat pups were nursed by dams that were fed with the hypocaloric diet during lact

Effect of preeclampsia and premature rupture of membrane on neonatal birth weight and length by gestational age: A retrospective study in China.

BACKGROUND: Preeclampsia (PE) and premature rupture of membrane (PROM) are considered significant risk factors for lower neonatal birth weight and birth length. However, very limited studies have reported the impact of PE and PROM on neonatal birth weight and birth length by gestational week. Therefore, we aimed to determine the effect of PE and PROM on neonatal birth weight and length by gestational age. MATERIALS AND METHODS: A total of 9707 singleton neonates were selected for this study. All the data were collected and documented in the obstetric register by the trained nurses in the Gyne

Anaphylactoid Syndrome of Pregnancy.

Anaphylactoid syndrome of pregnancy (ASP) is a widespread, proinflammatory, anaphylactic-like reaction that can occur when amniotic fluid enters the maternal blood circulation. ASP is characterized by four cardinal findings: respiratory distress, altered mental status, hypotension, and disseminated intravascular coagulation. ASP is commonly associated with maternal and neonatal mortality. Early recognition followed by prompt and aggressive treatment can improve survival rates and are among the most critical activities for nurses and other clinicians caring for women with ASP.

Safe oxygen saturation targeting and monitoring in preterm infants: can we avoid hypoxia and hyperoxia?

UNLABELLED: Oxygen is a neonatal health hazard that should be avoided in clinical practice. In this review, an international team of neonatologists and nurses assessed oxygen saturation (SpO2 ) targeting in preterm infants and evaluated the potential weaknesses of randomised clinical trials. CONCLUSION: SpO2 of 85-89% can increase mortality and 91-95% can cause hyperoxia and ill effects. Neither of these ranges can be recommended, and wider intermediate targets, such as 87-94% or 88-94%, may be safer.

Screening and outcome of retinopathy of prematurity.

Retinopathy of prematurity continues to be a formidable problem for the premature infant, his or her family, and physicians and nurses caring for the child. In the past year, advances have been made in screening for serious retinopathy and in our understanding of the long-term visual function of eyes that had retinopathy during the infants' neonatal period. Further work has also been undertaken in defining the role of laser photocoagulation and cryotherapy in the treatment of threshold disease.

Hope and parents of the critically ill newborn: a concept analysis.

Hope is a human phenomenon that is observed repeatedly in the neonatal intensive care unit setting. There have been few studies specifically examining the concept of hope in this population. The purpose of this article is to examine the concept of hope in parents of critically ill newborns. Uses of the concept will be investigated. Critical defining attributes will be provided. Antecedents and consequences of hope will be investigated. An overview of the empirical referents of hope will be presented.
