Pearls: diplopia.

Double vision may arise from ocular, neurologic, or extraocular muscle disorders. The approach to patients with diplopia requires a systematic approach to the history and the physical examination. There are many challenges in the evaluation of diplopia, ranging from the patient's mental status to the fine points of the examination. This article provides a process for interviewing and examining these patients, explaining the rationale and differential diagnosis of various clinical presentations. Common causes of monocular and binocular diplopia are addressed by the pattern of diplopia described

Unilateral blepharospasm and blepharoptosis with bilateral vertical gaze palsy in thalamic hemorrhage.

We present here case of a 49-year-old female with unilateral left sided blepharospasm and blepharoptosis, bilateral vertical gaze palsy, right hemiparesis and right seventh nerve supranuclear palsy as a manifestation of left thalamic hemorrhage. To our knowledge this is the first reported case of a combination of unilateral blepharospasm and blepharoptosis with bilateral vertical gaze palsy.

[Convergence excess associated with neurological diseases: surgical treatment].

BACKGROUND: Lesions of the supranuclear pathways for convergence control can lead to convergence deficit or convergence excess. Whereas pathophysiology of acquired convergence excess is now fully covered in the literature, no specific paper on its surgical treatment could be found. PATIENTS: Cases 1 and 2: Parinaud's syndrome with convergence excess in attempted upgaze (+ convergence retraction nystagmus and VIth nerve palsy in case 2). Case 3: opsoclonus and convergence excess due to hysterical conversion after head trauma. Case 4: acquired nystagmus, accommodative spasm due to hyperopia. Ca

Hypomyelination with atrophy of the basal ganglia and cerebellum (H-ABC). Report of a new case.

Hypomyelination with atrophy of the basal ganglia and cerebellum (H-ABC) syndrome is a new neurodegenerative entity, which was first described by van der Knaap in 2002 in 7 patients aged from 2 months to 2 years. We describe a new, 42-month-old female patient who developed progressive dystonia, spasticity and oculogyric eye movements since the age of 3 months. The diagnosis was made by characteristic MRI findings including supratentorial hypomyelination and progressive atrophy of basal ganglia and cerebellum. Oculogyric eye movements have not been described in patients with H-ABC syndrome befo

[Advantages of Krats's laser pointer associated with the Weiss coordimeter in the study of cyclofusion].

INTRODUCTION: The cyclofusional abilities of some patients can lead to an underevaluation of their deviation. This study proposes an original and specific analysis of cyclofusion and discusses its practical applications. PATIENTS AND METHOD: We followed 19 consecutive patients, 14-69 years of age (mean, 43.7 years), presenting slight excyclotorsion (measured by the striated Maddox glasses), never exceeding 4 degrees and often at 0 degrees (63%) even in down gaze. A positive Bielschowsky head tilt test was assessed in all cases but was moderate in 75%. Objective excyclotorsion was observed in

Atypical presentation of a unilateral septic thrombosis of the cavernous sinus.

A 7 year-old boy with unilateral septic cavernous sinus thrombosis following a pyoderma in the eyebrow is described. Problems in diagnosis and the role of magnetic resonance imaging (MRI) in the management are highlighted. Patient recovered with mild residual sequelae. The unique feature of this case was absence of chemosis and non-toxic presentation of the patient.

[Opsoclonic-myoclonic-ataxic syndrome].

INTRODUCTION: The opsoclonic-myoclonic-ataxic syndrome (OMAS) is that in which there are at least two of the cardinal signs (opsoclonus, myoclonus and ataxia). OBJECTIVES: To report three cases, their evolution and aetiology, and discuss the main physiopathological basis and guidelines for treatment. CLINICAL CASES: We present three cases admitted to the Internal Medicine Department with the diagnosis of OMAS. The investigations included standard biochemical and haematological tests, a full study of the cerebrospinal fluid, computerized axial tomography of the skull and thorax, and magnetic

[Acute necrotizing encephalopathy with horizontal gaze palsy].

Acute necrotizing encephalopathy (ANE) of childhood is a newly proposed disease entity characterized by symmetrically distributed necrotic brain lesions in the thalamus, cerebral white matter, brainstem, and cerebellum. We report a 4-year-old girl with severe psychomotor delay and horizontal gaze palsy as sequelae of ANE at 17 months of age. The computed tomography showed bilaterally symmetrical low density areas in the thalamus and low density areas in the left middle cerebral artery (MCA) region and the posterior cerebral artery (PCA) region. MRI (T 1-weighted) revealed unevenly distributed

[Superior oblique myokymia--a case report and pathogenetic consideration].

A 30-year-old male visited us with complaints of dizziness and diplopia of abrupt onset in September, 1988. Neurological studies demonstrated paroxysmal rotatory to-and-fro oscillations of the left eye. There has been the same clinical episode at 25 years of age, lasting approximately 6 months. The characteristics of his ocular involuntary movement with the mild IVth cranial nerve paresis were as follows: quick, nonrhythmic, initially counterclockwise-rolling, more prominent in moving the left eye to the lower medial side, persisting for 1-10 seconds and rapidly repeated oscillations. When osc

Optic ataxia in Bálint-Holmes syndrome.

The objective of this review is to reinstate the diversity of visual perception and visuomotor symptoms following lesions to the posterior parietal cortex (dorsal visual stream). This diversity was overshadowed for a long time and shows the contribution of the dorsal visual stream not only to action but also to perception. More precisely, we propose that the visuomotor deficit in optic ataxia stems from two distinct components: visual proprioceptive deficit (hand effect) and visual attentional deficit (field effect) also affecting the perception in peripheral vision.

Oculomotor Abnormalities during Reading in the Offspring of Late-Onset Alzheimer's Disease.

INTRODUCTION: Eye movement patterns during reading are well defined and documented. Each eye movement ends up in a fixation point, which allows the brain to process the incoming information and program the following saccade. In this work, we investigated whether eye movement alterations during a reading task might be already present in middle-aged, cognitively normal offspring of late-onset Alzheimer's disease (O-LOAD). METHODS: 18 O-LOAD and 18 age-matched healthy individuals with no family history of LOAD participated in the study. Participants were seated in front of a 20-inch LCD monitor,

Efficacy of Smartphone-based Exercises in Conjunction with Modified Glasses Prescription in the Treatment of Convergence Insufficiency and Fusion Weakness.

PURPOSE: To determine the efficacy of Smartphone-based exercises in conjunction with modified glasses prescription in the treatment of convergence insufficiency and fusion weakness. METHODS: A total of 150 patients using glasses and having asthenopia between 15 and 30 years of age were subjected to thorough eye examination including binocular vision testing and refraction. Of them 26 individuals were found to have convergence insufficiency with fusion weakness and exophoria >6 pd. Subjects with amblyopia, manifest strabismus and other ocular diseases were excluded from the study. Modified gla
