Pediatric Infectious Skin Conditions.

Pediatric infectious skin conditions are frequently managed in primary care offices. Familiarity with these conditions and an understanding of a patient's individual characteristics, including vaccine status, ethnicity, and risk of drug-resistant pathogens, is crucial for proper diagnosis and management. Proper diagnosis considers these factors but is also aided by dermoscopy, which is a noninvasive tool for improving diagnostic accuracy both by confirming the diagnosis but also ruling out other diagnoses. Through a proper diagnosis, appropriate treatment can be administered for these skin con

Allergic skin disease: major highlights and recent advances.

AD, urticaria, and ACD account for a large percentage of the skin disorders that present at medical offices and emergency departments. With a high degree of morbidity, these diseases can be extremely difficult to manage. Consequently, these diseases continue to be the focus of much of the new research in the field. Recent studies have furthered the understanding of the complex relationship between immune and nonimmune factors in the pathogenesis of these disorders. This research has quickly led to the development of new treatment modalities and protocols for patient care, especially for AD. Ho

Autonomic Dysfunction and Postural Orthostatic Tachycardia Syndrome: What Every Frontline Clinician Needs to Know.

Autonomic dysfunction, particularly postural orthostatic tachycardia syndrome (POTS), is increasingly recognized in adolescents and young adults. Early recognition in primary care is critical, as these conditions can substantially affect functioning, school participation, quality of life, and health care utilization. This review provides practical, clinically relevant guidance for pediatric and family medicine clinicians, and emphasizes recognition, office-based assessment, initial management, and referral strategies. Key considerations include differentiating POTS from mimicking conditions, e

[Healthcare management of an epilepsy clinic: factors involved in the demand for health care and clinical situation of patients].

INTRODUCTION: Epilepsy is a chronic illness that requires a long-term periodic follow-up of the patient and this means that as time goes by the number of patients attended increases, with the ensuing added cost for the healthcare system. AIM: To determine the factors involved in the time until an epileptic patient's next visit. PATIENTS AND METHODS: Our sample consisted of a selection of patients who visited the epilepsy clinic at our hospital consecutively during one year. Their clinical situation and relationship with the medical advice they were given, together with the factors involved i

Diagnostic testing and return visits for acute problems in prepaid, case-managed Medicaid plans compared with fee-for-service.

Enrollment of Medicaid recipients into capitated, case-managed systems of health care has been advocated as a means to control costs. We studied the effect of such systems on care for urinary tract infection (UTI), pelvic inflammatory disease, and vaginitis among women enrolled in Aid to Families with Dependent Children in capitated demonstration programs in Santa Barbara County, California, and Jackson County, Missouri (prepaid), compared with similar but fee-for-service (FFS) counties in Ventura County, California, and St Louis, Mo. Structured abstracts were performed on 2382 outpatient char

A Pilot Study of Use of a Software Platform for the Collection, Integration, and Visualization of Diabetes Device Data by Health Care Providers in a Multidisciplinary Pediatric Setting.

BACKGROUND: Diabetes devices provide data for health care providers (HCPs) and people with type 1 diabetes to make management decisions. Extracting and viewing the data require separate, proprietary software applications for each device. In this pilot study, we examined the feasibility of using a single software platform (Tidepool) that integrates data from multiple devices. MATERIALS AND METHODS: Participating HCPs (n = 15) used the software with compatible devices in all patient visits for 6 months. Samples of registration desk activity and office visits were observed before and after intro

Highlights of the 2022 Vietnamese Society of Hypertension guidelines for the diagnosis and treatment of arterial hypertension: The collaboration of the Vietnamese Society of Hypertension (VSH) task force with the contribution of the Vietnam National Heart Association (VNHA): The collaboration of the Vietnamese Society of Hypertension (VSH) task force with the contribution of the Vietnam National Heart Association (VNHA).

Hypertension is uncontrolled in over 50% hypertensive population in Vietnam which indicated a compelling need for new hypertension guidelines. The highlights were composed of three parts: the diagnosis of arterial hypertension, the recommendation of home blood pressure monitoring, and the treatment of hypertension. Our guideline applied flexibility based upon the "essential" and "optimal" concepts in the diagnosis and management of hypertensive patients according to the socio-economic status of Vietnam. Hypertension is defined as an office systolic blood pressure ≥140 mmHg and/or diastolic blo

Clinical implications of the 2013 ESH/ESC hypertension guidelines: targets, choice of therapy, and blood pressure monitoring.

The European Society of Hypertension (ESH)/European Society of Cardiology (ESC) 2013 guidelines for the management of arterial hypertension included simplified blood pressure (BP) targets across patient groups, more balanced discussion on monotherapy vs. combination therapy, as well as reconfirmation of the importance of out-of-office BP measurements. In light of these updates, we wished to review some issues raised and take a fresh look at the role of calcium channel blocker (CCB) therapy; an established antihypertensive class that appears to be a favorable choice in many patients. Relaxed BP

Safety and efficacy of lorundrostat, an aldosterone synthase inhibitor, in patients with uncontrolled hypertension: A systematic review and meta-analysis.

BACKGROUND: Limited data from randomized controlled trials (RCTs) indicate that lorundrostat, an aldosterone synthase inhibitor, may be effective in managing uncontrolled hypertension (HTN). Currently, no systematic review or meta-analysis (SR/MA) has assessed the safety and efficacy of the drug in such clinical conditions; this SR/MA aims to fill that knowledge gap. METHODS: Electronic databases and registers were searched from inception to June 30, 2025, for RCTs involving lorundrostat in the intervention arm and placebo in the control arm in individuals with uncontrolled HTN. Development and use of clinical vignettes to assess injury care quality in Northern Malawi.

BACKGROUND: It is known that outcomes after injury care in low-and-middle income countries (LMICs) are poorer than those in high income countries. However, little is known about healthcare provider competency to deliver quality injury care in these settings. We developed and used clinical vignettes to evaluate injury care quality in an LMIC setting. METHOD: Four serious injury scenarios, developed from agreed best practice, testing diagnostic and management skills, were piloted with high and low-income setting clinicians. Scenarios were used with primary and referral facility clinicians in Ma

Medical management of Ménière's disease.

OBJECTIVES: Ménière's disease (MD) is poorly understood with respect to its etiology, pathophysiology, clinical course, and treatment. Furthermore, in the absence of controlled clinical studies, empiric treatments have been used with varying degrees of success. In this paper, the authors review the current medical management of MD. STUDY DESIGN: Literature review. RESULTS: Because of a dearth of well-controlled studies, the medical management of MD remains empirical and is largely restricted to lifestyle changes, pharmacotherapy, and office-based procedures. The development of transtympanic

Epistaxis: evaluation and treatment.

Epistaxis, or nosebleed, is a common disorder that many patients will experience. Most patients go to the emergency room when they have an uncontrolled nosebleed, or they may present to an outpatient office. Most nosebleeds are not life-threatening and can be managed conservatively. Occasionally, hospital admission, referral to an otolaryngologist physician, and/or blood transfusion may be necessary. This article is an update on the latest information related to the prevention, causes, and treatment of epistaxis.

Faecal incontinence: a joint approach to guideline development.

Faecal incontinence is an embarrassing symptom that should be assessed. Faecal management systems (FMS) collect and contain liquid or semi-liquid stool, helping to preserve skin integrity and prevent environmental contamination. Nurses from three health boards across Wales joined together to produce guidance for using FMSs in practice. Backed by national organisations and the chief nursing officer for Wales, this was published in March 2011.

Special considerations for treating dental patients exhibiting the "post-polio syndrome".

A condition called "Post-polio Syndrome" (PPS) is a special type of neuromuscular disturbance that affects some elderly patients who had polio myelitis either as children or as young adults. It has been reported that approximately 1,600,000 polio survivors are alive today. Most will seek dental care, and up to half of the survivors will present with some form of PPS. This paper describes polio, its characteristics, and the long-term consideration of PPS, and discusses the special clinical implications related to this condition. Special emphasis is placed on physical impairments, breathing prob
