Orbital reconstruction by local flaps, distant flaps, free skin transplants or mucosal transplants, means numerous operations. Good results are often spoiled by postoperative shrinking, especially, when the patient previously had radiation. Therefore we use nowadays free flaps (forearm flap, dorsalis pedis flap) as fascio-cutaneous or fascio-mucosal flaps. By this procedure, reconstruction can be done in one or two stages and postoperative shrinking can be avoided. Our experience in 4 cases is demonstrated.

While the number of orbital surgeons is limited, it is hoped these can be recognized and patients referred to them by ophthalmologists not interested or trained in that specialty. Let the orbital surgeon determine whether he can handle the problem in 1 to 2 days, or whether a neurosurgeon should do the procedure or make it a joint effort. It may well involve other specialty team effort approaches. It is essential to have an understanding of x-rays, CT, angiography, and MRI techniques and films. Sit with these specialists to learn more and help to avoid negative, misdiagnosis reports in the int…

Twenty patients with severe orbital and orbitomaxillary deformities secondary to oncologic surgery and radiation were studied. Sixteen patients underwent complete reconstruction of the orbit and the periorbital area. The orbital reconstruction was accomplished with autogenous tissue (skin and dermis grafts) that was anchored to the bony orbit. The periorbital area was augmented with customized implants. Most patients now wear a satisfactory prosthesis with or without lightly tinted glasses, giving them an acceptable orbital symmetry.

Seventeen patients had spontaneous orbital hemorrhages. The usual symptoms were acute onset of pain, proptosis, and vomiting with decreased vision, limitation of motility, and ecchymosis of the eyelids occurring in some patients. The children often developed a progressive space occupying lesion that simulated a neoplasm. Most patients had underlying venous anomalies, although several elderly patients with atherosclerosis developed arterial hemorrhages with more abrupt and dramatic symptoms. Other associated conditions included hypertension, anemia, labor, and von Willebrand's disease. The visu…

We present a three-dimensional continuum model of electron transmission through a chiral electrostatic potential and show that it gives rise to chirality-induced orbital selectivity. In this model, electron transmittance depends strongly on the incident orbital angular momentum (OAM) associated with its transverse motion, and the selectivity reverses when the potential's handedness is inverted. The effect originates from a coupling between axial linear momentum and OAM mediated by the helical spatial dependence of the potential. For DNA-scale geometric parameters, this linear-orbital coupling …

AIMS: The purpose of this study was to determine the frequency and associated risk factors of orbital/periocular complications in patients with sinonasal tumour with orbital invasion managed with eye-sparing treatments.
METHODS: A retrospective case series of patients with primary sinonasal tumour with orbital invasion from January 2008 to December 2018. Patient factors were compared between the following groups: (1)patients with orbital/periocular complications versus those who did not and (2) patients who needed secondary oculoplastic surgical procedures versus those who did not.
RESULTS: …

OBJECTIVE: To describe release of the inferior orbital fissure (IOF) as a novel surgical technique that will improve access to the inferior and inferolateral orbital apex.
DESIGN: Laboratory investigation.
PARTICIPANTS: Human cadaver heads.
METHODS: Cadaveric dissection study whereby the technique of an IOF release was performed. A swinging eyelid preseptal approach was used to enable a subperiosteal dissection to the orbital floor without obstruction from orbital fat prolapse. A plane was dissected between periorbita and Muller's muscle, the smooth muscle that overlies the entire length of…

A 66-year-old woman status post bony lateral orbital decompression for thyroid eye disease presented with recurrent episodes of left lateral canthal edema and erythema, despite repeated courses of oral antibiotics, titanium hardware removal, and repeated exploratory orbitotomies with debridement and curettage. MRI later revealed an intraosseous fluid collection in the left greater sphenoid wing. Another exploratory orbitotomy was performed and an intraosseous abscess was identified. The purulent fluid was sent for culture, and the area was debrided and irrigated thoroughly with bacitracin solu…

Mucoceles can often present as a complication after prior sinus surgery or maxillofacial trauma when mucosal drainage is obstructed. Their presence in the orbit following orbital wall fracture and subsequent repair represent an exceedingly rare complication whose occurrence is limited to a few reported cases in the literature. In these patients, continuous cyst expansion and subsequent mass effect may lead to ophthalmic symptoms, including orbital pain, proptosis, diplopia, and globe dystopia. We report the discovery of an orbital mucocele after orbital floor fracture repair and its possible a…

BACKGROUND AND PURPOSE: Orbital compartment syndrome is a sight-threatening emergency caused by rising pressure inside the orbit. It is usually diagnosed clinically, but imaging might help when clinical findings are inconclusive. This study aimed to systematically evaluate imaging features of orbital compartment syndrome.
MATERIALS AND METHODS: This retrospective study included patients from 2 trauma centers. Proptosis, optic nerve length, posterior globe angle, morphology of the extraocular muscles, fracture patterns, active bleeding, and superior ophthalmic vein caliber were assessed on pre…

PURPOSE: Tumors of the medial orbital apex can be challenging to resect. Primary endonasal resection can be limited by requirements for fat retraction and poor angulation for lateral orbital dissection. Orbital resection alone can have space limitations and a limited view for dissection of the posterior aspect of the tumor. The combined transorbital transnasal approach integrates the retraction and instrumentation advantages of the transorbital approach with the optical and space advantages of the transnasal approach.
METHODS: In this cross-sectional cohort study, patients who underwent surgi…
