Isolated single coronary artery: diagnosis, angiographic classification, and clinical significance.

Isolated single coronary artery is a rare congenital anomaly occuring in approximately 0.024% of the population. This entity can be diagnosed during life only by coronary angiography. Ten patients with isolated single coronary artery are reported. Based on angiographic analysis, a new classification is proposed, according to the site of origin and anatomical distribution of the branches. Typical angina did not occur with single coronary artery in the absence of coexisting coronary artery disease or aortic stenosis. No correlation was apparent between the type of anomalous patterns and the symp

[Management of post-traumatic tibiotalar osteoarthritis following a failed osteosynthesis : retrograde plantar nailing].

Tibiotalar osteoarthritis, uncommon and often of post-traumatic origin, leads to functional limitations, chronic pain and significantly impairs the quality of life. Both tibiotalar arthrodesis and arthroplasty have been proven effective. Tibiotalocalcaneal (TTC) arthrodesis is generally reserved for ankle osteoarthritis associated with severe degeneration of the subtalar joint and hindfoot. Nevertheless, new data showed good outcomes regardless of the subtalar joint state. We present the clinical case of an 86-year-old female patient with severe tibiotalar osteoarthritis after a failed osteosy

The role of echocardiography in anomalous origin of coronary artery from pulmonary artery (ALCAPA): Simple tool for a complex diagnosis.

Anomalous origin of coronary artery from pulmonary artery (ALCAPA) is a rare coronary anomaly usually presenting in the first year of life. Adult presentation suggests a wide range of differential diagnosis such as myocarditis, dilated cardiomyopathy, and coronary artery disease. We have presented here the major distinctive echocardiographic features of ALCAPA through 4 cases. Since echocardiography is often used as the first imaging modality in these clinical scenarios, recognizing the characteristics of ALCAPA in a routine echocardiographic examination can transform a readily available, low-

[Recurrent epistaxis of unknown origin. A rare differential diagnosis and its treatment].

We present the case report on a 66-year-old male patient who appeared in our clinic with recurrent epistaxis. Anterior rhinoscopy during acute bleeding yielded no information on the origin of hemorrhage. On renewed epistaxis, angiography was performed on which an arteriovenous malformation in the sphenoid sinus could be identified as the cause of epistaxis. Embolization of afferent vessels successfully reduced the malformation and stopped the bleeding. In cases of recurrent epistaxis, especially in combination with an unremarkable anterior rhinoscopy, angiography is recommended for the detecti

A rare case of adult type ALCAPA syndrome: presentation, diagnosis and management.

We report a case of a 43 year-old previously fit and healthy woman presenting with cardiac arrest secondary to an anomalous origin of the Left Coronary Artery from the Pulmonary Artery (ALCAPA) Syndrome, a very rare congenital anomaly that it is extremely unusual to survive to adulthood if left uncorrected. To our knowledge, this is the first case of its type to be diagnosed and reported in an Australian hospital. Suspicion of this anomaly on conventional coronary angiography is essential given the life threatening nature of this condition.

[What is paruresis or shy bladder syndrome? A transdisciplinary research].

This paper checks typical aspects of Paruresis or Shy Bladder Syndrome, disease defined as the impossibility or difficulty to urinate in public toilets. We analyze the definition, differences, degrees, nature, etiology, traditional treatments, myths about the origin, and so on; but two are the main contributions of this transdisciplinary research: first, the description of the paruretic's daily life, which main core is the concealment of paruresis; and second: we offer an alternative to paruretic community, that is beneficial to other stigmatized groups, Coming out of the closet, process that

Three-dimensional reconstruction of computed tomographic images of anomalous origin of the left main coronary artery from the pulmonary trunk in an adult.

We present a 49-year-old female case of anomalous origin of the left main coronary artery from the pulmonary trunk. Multidetector computed tomography was performed, and 3-dimensional reconstruction of computed tomographic images found that the left main coronary artery originated from left sinus of the pulmonary trunk and the right coronary artery from the right coronary cusp of the aorta. We speculate that this patient's long life may be due to the dominant right coronary artery and rich collateral from the right coronary artery to the left coronary artery.

Hb Chile [beta28(B10)Leu-->Met]: an unstable hemoglobin associated with chronic methemoglobinemia and sulfonamide or methylene blue-induced hemolytic anemia.

Among the causes of life-long cyanosis are congenital methemoglobinemia due to M hemoglobins, congenital methemoglobinemia due to methemoglobin reductase deficiency, a small number of low oxygen affinity hemoglobins, and a small number of unstable hemoglobins that spontaneously form methemoglobin in vivo at an accelerated rate. We report an unstable hemoglobin with these characteristics that was observed in a family of indigenous (native American) origin living near Santiago, Chile. This variant has the substitution beta28(B10)Leu-->Met, unambiguously corresponding to the DNA mutation of CTG--

Primitive neuroectodermal tumor: a newly recognized cause of early fetal death.

Reviews in the literature of fetal and neonatal neoplasms deal only with congenital tumors, that is, tumors presenting at or shortly after birth. Virtually nothing is known about neoplasms occurring in the embryonal period or early fetal life. We report five small macerated fetuses with a very primitive embryonal tumor of neuroectodermal origin. The tumor characteristically showed massive vascular permeation. Expulsion of the fetus occurred during the fourth or fifth month of pregnancy. It is suggested that this primitive neuroectodermal tumor is a "new" cause of spontaneous late abortion.

Ultra-High-Density Catheter Ablation of Purkinje Premature Ventricular Contractions Causing Electrical Storm.

The postacute myocardial infarction electrical storm is a life-threatening entity. Resistance to ischemia in Purkinje fibers may be the origin of short-coupled premature ventricular contractions that trigger severe arrhythmic events. We present a case where the use of emergency catheter ablation, guided by a 3D navigation system and an ultra-high-density mapping catheter, successfully terminated the arrhythmic storm. This case highlights the crucial role of advanced mapping and ablation techniques in managing complex postinfarction arrhythmias and provides valuable insights that may enhance fu

Psychosis as a Manifestation of Focal Impaired Awareness Seizure in a Geriatric Patient.

Focal impaired awareness seizures (FIAS), previously known as complex partial seizures, refer to focal seizures that start in one hemisphere of the brain and are associated with an impairment in consciousness. FIAS of temporal lobe origin most often present with psychopathology, such as behavioral and affective symptoms. It has a bimodal age distribution peaking at the extremes of life. Geriatric presentations can often be subtle and go unnoticed asides from a few symptoms, such as confusion and memory lapses. Here, we present a unique case of a geriatric patient with FIAS presenting as transi

Management of unusual causes of ulcers of lower extremities.

Treatment of any wound begins with the correct diagnosis. The vast majority of chronic wounds can be classified as pressure ulcers, venous ulcers, insensate ulcers, or arterial insufficiency ulcers. This article focuses on wounds of unusual origin that must be considered in evaluation. All of these wounds are difficult to treat, are more likely to occur in specialized clinical settings, may provide clues to underlying systemic illnesses, and can be life-threatening. Unusual wounds discussed include atrophe blanche, calciphylaxis, leukocytoclastic vasculitis, necrobosis lipoidica, necrotizing f

Familial congenital bronchiectasis: Williams-Campbell syndrome.

Williams-Campbell syndrome was first described in 1960 as a rare form of bronchiectasis. Its pathogenesis is characterized by the absence or markedly diminished cartilage around the bronchi. Although the familial nature was postulated early on, only one possible familial occurrence has been reported in the literature. We present two family members in whom respiratory symptoms developed within the first year of life and were found to have histopathologic changes consistent with Williams-Campbell syndrome. This world lend further support of a developmental origin for this type of bronchiectasis.

Aspects of heredity, syndromic associations, and course of conditions in which cutaneous lesions occur solitarily or in multiplicity.

Many tumors of the skin, whether originating from ectoderm or mesoderm, exist in either single or multiple form. In the multiple state there are frequently associations with other cutaneous and systemic abnormalities, sometimes serious and even life-threatening. The resultant syndrome is almost always inherited in an autosomal dominant pattern. By contrast, solitary lesions are not heritable, nor are they associated with other stigmata. The solitary forms appear in middle or late life, the multiple, syndromic forms in childhood or early adulthood.

Mechanisms of Fetal Adaptation to Chronic Hypoxia following Placental Insufficiency: A Review.

Placental insufficiency is associated with reduced oxygen and nutrient supply to the fetus, which may result in fetal growth restriction (FGR). In an attempt to cope with the hostile intrauterine environment, FGR fetuses respond through metabolic, endocrine, vascular, cardiac, behavioral, hematological, and immunological adaptive mechanisms. However, permanent sequelae may result from such adaptive mechanisms. In this review, we describe the mechanisms of fetal adaptation to the hostile intrauterine environment in FGR of uteroplacental origin and detail their pathophysiology and potential impl

Oncological emergencies associated with gastrointestinal tumors.

Oncological emergencies are defined as acute life-threatening conditions in cancer patients either as a result of the malignancy or as a result of its treatment. In this review, we focus on oncological emergencies associated with gastrointestinal tumors. They can be categorized by their system of origin as hematologic, neurologic or metabolic. Furthermore, we discuss mechanical emergencies such as intestinal obstruction and vena cava superior syndrome as well as acute gastrointestinal bleeding and pulmonary embolism. The patients' performance status as well as prognosis are essential during de

Oral health of indigenous children and the influence of early childhood caries on childhood health and well-being.

Dental caries in Indigenous children is a child health issue that is multifactorial in origin and strongly influenced by the determinants of health. The evidence suggests that extensive dental caries has an effect on health and well-being of the young child. This article focuses on early childhood caries as an overall proxy for Indigenous childhood oral health because decay during early life sets the foundation for oral health throughout childhood and adolescence. Strategies should begin with community engagement and always include primary care providers and other community health workers.

The role of dendritic cells in the pharynx.

The tonsils are organized as lymphoepithelial structures that play an important role in protecting both the upper respiratory and alimentary tract regions against incoming antigens. This function requires dendritic cells, professional antigen-presenting cells that act as peripheral sentinels, specializing in the uptake, processing and presentation of antigenic material. This article gives a brief review on dendritic cells with regard to their origin, life cycle and functions in the pharyngeal mucosa. The regulation of immune responses in tonsils by dendritic cells is discussed. Their importanc

[Bronchopulmonary ACTH-producing tumors].

Neuroendocrine tumors have the ability to produce the hormones and vasoactive peptides. Excess of these hormones leads to different symptoms and syndromes because of organs' injuries. Detection of ACTH origin by using of modern diagnostic methods is not always possible. Lungs and bronchi are one of the most frequent localization of ACTH-producing tumors. It is considered that carcinoids with bronchopulmonary localization like a benign tumors in the clinical course. But at the same time carcinoid tends to metastasize, so timely diagnostics and treatment improve quality of life significant and i

Complement inactivation by recombinant human C3 derivatives.

From the implications of the complement system in a large number of diseases, an urgent need for therapeutics effecting reduced complement activity in vivo has emerged. In this study we report the design of a novel class of enzymes of human origin that obliterate functional complement by a noninhibitory, catalytic mechanism. Combining the framework of human C3 and the enzymatic mechanism of cobra venom factor, a nontoxic snake venom protein, we established molecules capable of forming stable C3 convertase complexes. Although the half-life of naturally occurring C3 convertase complexes ranges b
