Twenty-year clinical experience with fixed functional appliances.

INTRODUCTION: Considering the large number of fixed functional appliances, choosing the best device for your patient is not an easy task. OBJECTIVE: To describe the development of fixed functional appliances as well as our 20-year experience working with them. METHODS: Fixed functional appliances are grouped into flexible, rigid and hybrid. They are different appliances, whose action is described here. Four clinical cases will be reported with a view to illustrating the different appliances. CONCLUSIONS: Rigid fixed functional appliances provide better skeletal results than flexible and hyb

Proficiency of Clear Aligner Therapy: A Systematic Review and Meta-Analysis.

The aim of this study was to evaluate the efficacy and efficiency of orthodontic treatment using clear aligner therapy (CAT). This efficiency was measured using the Peer Assessment Rating (PAR) index, the American Board of Orthodontics (ABO) index, or the similarity between the final ClinCheck and the final scanned models. A search was done electronically between 1998 and 2021 using the Cochrane Library, PubMed, and Google Scholar databases. Three reviewers individually rated the articles. The ROBINS tool and the Cochrane risk of bias tool were used to evaluate the quality of observational res

Effects of initial stresses and time on orthodontic external root resorption.

Optimum stresses for a favorable response to orthodontics are still unknown. Here, we compared the effects of initial periodontal ligament (PDL) stresses over time in orthodontic external root resorption (OERR), necrosis, and the TRAP+ cell population. Forty-two rats (Fischer CDF) were treated with 10 cN of force for 5 different time periods. Finite element (FE) models of the first maxillary molars were constructed from μCT scans to calculate initial PDL stresses. The scans were also used for OERR measurements before histology. Time, stress, and their interaction were significant to result in

Adult nonsurgical retreatment of a skeletal Class I hypodivergent, Class II subdivision malocclusion with an iatrogenic postextraction bowing pattern using buccal fixed appliances and miniscrew-supported maxillary distalization: A case report.

Orthodontic retreatment after extraction-based therapy can be complicated by iatrogenic bowing ("roller-coaster") patterns with residual spaces, posterior segment tipping, and loss of incisor torque, often accompanied by bite deepening. This case report describes nonsurgical retreatment of a 29-year-old woman with a skeletal Class I, hypodivergent pattern and a Class II left subdivision who presented with severe deep overbite, increased overjet, marked bimaxillary incisor retroclination with clinical labial root prominence of the maxillary and mandibular incisors, residual maxillary extraction

Clear Aligners or Traditional Braces? An Effective Plan in the Ortho-Perio Treatment of Gingival Recessions.

BACKGROUND: Gingival recession, a common clinical condition characterized by apical migration of the gingival margin, often presents in patients undergoing orthodontic treatment. Orthodontic forces can influence periodontal tissues positively or negatively, depending on the type of appliance used. The debate continues between clear aligners and traditional fixed braces concerning their effectiveness in managing such cases within interdisciplinary treatment plans. METHODS: A prospective, randomized clinical trial was conducted with 60 patients (30 per group) aged 20-35 years presenting with an

The Evaluation of Degree of Monomer Conversion, Biaxial Flexural Strength, and Surface Mineral Precipitation of Orthodontic Adhesive Containing Sr-Bioactive Glass Nanoparticles, Calcium Phosphate, and Andrographolide.

This study examined the degree of monomer conversion (DC) and mechanical properties of experimental orthodontic adhesives containing monocalcium phosphate monohydrate (MCPM), Sr-bioactive glass (Sr-BAG) nanoparticles, and andrographolide. Experimental adhesives were prepared with a 4:1 powder-to-liquid ratio, containing methacrylate monomers with varying formulations of glass fillers and additives. DC was measured using ATR-FTIR (n = 5) with and without bracket placement under two curing protocols: conventional LED (1200 mW/cm2, 20 s) and high-intensity LED (3200 mW/cm2, 3 s). The biaxial flex

Tooth-borne distraction of the lower anterior subapical segment for correction of class II malocclusion, subsequent to genioplasty.

INTRODUCTION: Alveolar distraction is mainly used to increase height and width of the alveolar crest. This technique, however, is not typically used for lengthening the perimeter of the dental arch or improving teeth axes. We applied alveolar distraction in a tooth-borne manner in the second stage of our original method and obtained favorable results. We therefore present an outline of this method. CASE REPORT: Genioplasty was first performed to create an infrastructure for sequential advancement of the subapical alveolar segment. After bone union, anterior subapical alveolar osteotomy was pe

Strategies for canine cortical bone anchorage in fixed orthodontics with extraction of the first premolars: A case report.

Control of canine torque is critical in extraction-associated orthodontic treatment. This case report describes a 20-year-old female diagnosed with dental Class I malocclusion, accompanied by a high-angle vertical skeletal pattern, lingually inclined maxillary canines, a biprotrusive facial profile, shallow overjet, and deep overbite. Treatment was performed using a multi-bracket fixed appliance. Four premolars were extracted. Crimpable gate springs, TMA archwires, and lingual powerarms were used for resolving cortical anchorage of the maxillary canines, while mini-implants were utilized for v

Camouflage treatment of skeletal Class III malocclusion in an adult cleft-palate patient using passive self-ligating system.

This case report describes the successful camouflage treatment to correct a moderate skeletal Class III malocclusion in a 19-year-old male cleft-palate patient. Early closure of the palate produced palatal scar tissue that inhibited midfacial growth, causing maxillary arch deficiency, severe maxillary crowding, and anterior and posterior crossbites. Combined surgical-orthodontic therapy would have been the preferred treatment of choice; however, the patient declined this option because of surgical risks and costs. Therefore, nonextraction camouflage treatment using a passive self-ligating brac

The Adjunctive Soft-Tissue Diode Laser in Orthodontics.

Lasers are a relatively new addition to the orthodontist's armamentarium. This article reviews the fundamental basic science of available soft-tissue lasers, with an emphasis on diode lasers, and discusses various adjunct applications of the diode laser for soft-tissue orthodontic procedures. Diode lasers function by cutting with an initiated hot tip and produce minimal to no interaction with healthy dental hard tissue, making them suitable for soft-tissue procedures. The contact cutting mode provides enhanced bloodless site visibility and facility to perform delicate soft tissue procedures, w

Occupational health problems and adverse patient reactions in orthodontics.

The purpose of the present investigation was to study the nature and frequency of adverse reactions to materials and procedures among orthodontists and their patients. A questionnaire about this topic was mailed to practicing members of the Norwegian Orthodontic Society, of which 137 (about 75%) responded together with 127 chairside assistants. About half of the orthodontic personnel had experienced adverse reactions. The majority of the problems were dermatoses, comprising dryness, redness, itching, thickening, reduced tactile sensitivity, fissuring, soreness/desquamation and pain of hands an
