Facial asymmetry is quite common and, when sufficiently severe, can warrant surgical orthodontic intervention. The causes of facial asymmetry are numerous and can be generally classified as congenital, developmental, or subsequent to pathology or injury. A systematic and comprehensive examination, diagnosis, and treatment plan are requirements for successful correction of facial asymmetry. Several of the more common facial asymmetries presenting for treatment to the orthodontist and surgeon are discussed in this article. Some fundamental surgical and orthodontic principles for treating these d

A concensus conference on the timing of facial osteotomies was held in Jönköping, Sweden. Seven teams consisting of a maxillofacial surgeon and an orthodontist representing five countries discussed treatment planning for young patients with maxillomandibular deformities with special emphasis on timing. Patients with severe syndromic facial deformities were not considered. Consensus was reached that for most deformities it is best advised to wait until growth has ceased, as determined by longitudinal cephalometric data. This rule is less tight for Angle Class II than for Class III cases. For as

Bimaxillary osteotomies and a sliding genioplasty were performed to correct mandibular micrognathia and maxillary vertical hyperplasia in a 19-year-old white male with a Class II, division 1 malocclusion and anterior open bite. At a follow-up appointment with the orthodontist 4 weeks after the surgical procedure, the mandibular anterior teeth showed severe gingival recession, exposing labial root surfaces on the mandibular central incisors. Grafting procedures were able to restore labial soft tissue on the affected teeth. It is suggested that wound healing contraction of gingival tissue in the

The combined efforts of the orthodontist and surgeon are necessary for successful treatment of most patients with maxillary retrusion. Both the aesthetic and functional manifestations of this dento-facial deformity can usually be effectively treated by maxillary advancement, augmentation of the midfacial region and orthodontics. Treatment planning, pre- and post-surgical orthodontic treatment principles, and surgical considerations in treating 25 adults with maxillary retrusion are described and illustrated. Surgical advancement of the maxilla by Le Fort I osteotomy, proper alignment of the te

In recent years, invisible orthodontics has been one of the concerns of both orthodontists and patients. As two types of invisible orthodontics, clear aligners fit for cases that have high demand for living quality, need distalization of molars or intrusion of incisors, suffer from severe periodontitis, or that have non-abundant time for visits. While customized lingual appliances fit for cases that have high demand for aesthetics, have deep overbite or complex malocclusion, need maxillary expansion or retraction of incisors, or that have low compliance and good economic conditions. Based on a…

An impacted maxillary incisor in the early mixed dentition can pose a clinical challenge for orthodontists and pediatric dentists. Clinical observations supported with a 3-D imaging technique can help not only in assessing the precise location of the impacted tooth and its relation with neighboring teeth but also in correctly executing a treatment plan. The purpose of this paper is to describe the management of a severely rotated and horizontally impacted central incisor using intrabony rotation and simultaneous orthodontically-guided eruption. Timely intervention avoided disturbance in arch i…

Cone-beam computed tomography provides orthodontists with 3-dimensional images of the craniofacial region and valuable information for diagnosis and treatment planning of craniofacial or dental anomalies. However, a narrow focus on the skeletal and dental contributions to malocclusion can cause failure to identify skeletal or soft-tissue pathologies of the craniofacial structures unrelated to the orthodontic concerns. Two cases are presented that demonstrate skeletal and soft-tissue anomalies identified as incidental findings on cone-beam computed tomography scans of asymptomatic orthodontics …

Orthodontic tooth movement is basically a biological response toward a mechanical force. The movement is induced by prolonged application of controlled mechanical forces, which create pressure and tension zones in the periodontal ligament and alveolar bone, causing remodeling of tooth sockets. Orthodontists often prescribe drugs to manage pain from force application to biologic tissues. Nonsteroidal anti-inflammatory drugs (NSAIDs) are the drugs usually prescribed. NSAIDs block prostaglandin synthesis and result in slower tooth movement. Prostaglandins have been found to play a direct role in …

Cuspid gigantism or radicomegaly is a rare occurrence often associated with a hereditary, X-linked condition, called oculo-cardio-facio-dental (OFCD)syndrome. This syndrome is also characterized by atrial septal defects and/or ventricular septal defects, congenital cataracts and unusual facial characteristics.Confirmation of this syndrome is often provided by dentists or orthodontists when they diagnose an extremely large canine root length from the panoramic radiograph. The following case report shows a multidisciplinary approach to the dental treatment of a patient with OFCD syndrome and a c…

Tooth transposition is a subject that intrigues orthodontists because of the associated treatment planning. Approximately 0.3-0.4% of the population has this type of tooth disharmony, and in the literature, most authors are in disagreement about the treatment approach. In this article, a case is presented of bilaterally maxillary canine-first premolar transposition associated with bilaterally upper lateral incisor agenesis treated in a very unusual way. The transposed teeth were orthodontically reversed to their normal sequence and the missing lateral incisor spaces closed. We choose this appr…

BACKGROUND: The profound influence of orthodontic treatments on facial aesthetics has been a topic of increasing interest. This study delves into the intricate interplay between orthodontic treatments, facial feature alterations, and aesthetic perceptions.
METHODS: A total of 73 patients who had undergone orthodontic treatment were included in this study. Facial photographs were taken before and after treatment. Ten orthodontists provided facial aesthetic ratings (FAR) for each patient's frontal, profile, and overall views. 48 facial landmarks were manually placed by the orthodontists and nor…

The objectives of this study were to evaluate the assessments of maxillofacial surgeons, orthodontists, and laypersons on the predicted aesthetic outcome of various surgical options in Class III correction and the associations between certain initial cephalometric values and the judges' preferred option. Pre-surgical lateral headfilms and coloured profile photographs of 18 skeletal Class III Caucasian adult patients (10 males and 8 females) with a mean age of 24.5 years were used. The headfilms were hand traced and digitized. Conventional cephalometric analysis was performed. Computerized pred…

Ectodermal dysplasia can present in various ways. A reduced number of constructed teeth, deformed teeth and an underdeveloped alveolar process are common oral features in ectodermal dysplasia. Although the dental treatment goal is always to restore function, there is no standard treatment protocol for this condition. The dental treatment plan must be individually tailored based on the oral situation, (technical) possibilities, age and wishes of the patient. This requires a multidisciplinary approach among different healthcare providers: a maxillofacial prosthodontist, an oral and maxillofacial

The health of dental tissue is an important guarantee to obtain good orthodontic effect, and also the basis to achieve beauty, stability and function after orthodontic treatment. The classification of dental diseases is complex and diverse, which has different effects on orthodontic treatment. It is necessary to make clear diagnosis before orthodontic treatment, and take preventive and therapeutic measures according to the etiology and clinical symptoms, so as to achieve the goal of health treatment. Orthodontic treatment might cause a variety of changes in the teeth and pulp tissue. Such chan

Esthetic dilemmas often require an interdisciplinary approach to achieve an optimal result. This article describes such a case in which the patient presented with unesthetic porcelain crowns on structurally compromised maxillary anterior teeth. The maxillary incisors had failing root canals, advanced mobility, and deep periodontal pockets. In addition, there was a congenitally missing maxillary lateral incisor. The canine substitution created a severely canted appearance exacerbated by the patient's high smile line. A coordinated interdisciplinary treatment plan was put into action to satisfy

Dens evaginatus is a rare dental anomaly that occurs during tooth development and results in an abnormal protrusion from the occlusal surface of the affected tooth, often in the area of the central groove between the buccal and lingual cusps. Of clinical importance to the orthodontist is that these occlusal tubercles fracture easily or can be worn away, resulting in direct pulp exposure in a noncarious tooth. This can cause severe complications, including loss of tooth vitality, facial infection in the form of an abscess or cellulitis, or osteomyelitis of the jaw. If extraction of premolars is
