[Malformations of the lower extremities].

Malformations with deficiencies of the lower extremities are rare. They are usually caused by toxic influences during pregnancy between the 4th and the 12th week of gestation. Some malformations have a genetic origin. The total incidence of congenital deficiencies of the lower extremities is approximately 18 in 100,000 newborns. The most common deficiencies are fibular hemimelias, followed by congenital femoral deficiencies and tibial hemimelias. Hemimelias are often associated with deficient toes or ray defects. Congenital pseudarthrosis of the tibia is less common, but this diagnosis is unde

[The treatment of rheumatoid foot deformities].

Rheumatoid hindfoot deformity presents with hindfoot eversion, flattening of the longitudinal arch and abduction of the forefoot. Splayfoot, as the typical rheumatoid forefoot deformity, is mostly associated with various toe malformations, i.e. hallux valgus, hammer toe and claw toe, which may either be attributed to hindfoot malalignment or develop as a separate entity. The algorithm of treatment, comprising clinical assessment of both lower limbs, includes both orthotic shoe devices and surgical treatment. In rheumatoid flatfoot, arthrodesis of the hindfoot with lengthening of the lateral colu

A novel spinal orthosis with embedded shape memory alloy for adolescent idiopathic scoliosis.

Bracing is the most effective measure for the treatment of mild-to-moderate adolescent idiopathic scoliosis (AIS). However, the balance between three-dimensional correction of scoliosis and wear comfort is still not ideal. To design a novel bracing with embedded shape memory alloy for the treatment of AIS that can improve patients' comfort and provide sustained and effective three-dimensional correction force, as well as to preliminarily verify its clinical efficacy. A Ni‒Ti shape memory alloy material was constructed with 20 ℃ martensite and 0 ℃ austenite. On the basis of the traditional rigi

Integrating Rehabilitation Knee Orthosis and FES-Feedback System for Isometric Training Protocols.

The present work investigates a preliminary case of integration of functional electrical stimulation (FES) and a portable knee orthosis with a built-in dynamometer for rehabilitation protocols. Building upon previous work (Freire, 2024), this updated system aims to enhance user integration, validate structural improvements, and integrate an FES feedback loop with the system's operation. The proposed training orthosis is equipped with biomechanical torque measurement capabilities, allowing execution of isometric knee extension exercises at predefined angles.A closed-loop control approach allows

Use of myoelectric orthosis after stroke or traumatic brain injury: a systematic review.

BACKGROUND: Upper extremity (UE) paralysis and weakness due to stroke or traumatic brain injury (TBI) can limit independent functioning. Myoelectrically controlled orthoses can be used for compensatory support for activities of daily living (ADL), and for restorative rehabilitation to reduce disability. OBJECTIVE: We investigate the use of UE myoelectric orthoses (UE-MEO) for compensatory and/or restorative use after stroke or TBI. METHODS: We conducted a systematic review (PROSPERO CRD42024577225) from 15 databases including MEDLINE®, Embase®, and APA PsycInfo®. Peer-reviewed reports from 2

Use of the Neurological Hand Deformity Classification in clinical practice for children with cerebral palsy: a case study approach.

INTRODUCTION: The management of hand deformities of people with neurological impairment is complex and challenging. The use of orthoses to manage hand deformity in clinical practice continues despite low level evidence. Understanding the purpose of an orthosis supported by a biomechanical framework, the Neurological Hand Deformity Classification (NHDC), to identify deforming forces and how to resolve them with appropriate orthotic intervention may reduce controversy surrounding orthosis use. METHODS: Detailed case descriptions were used to demonstrate how the NHDC can be clinically applied to

The effect of spinal orthoses on pain, kyphosis angle, balance, fall risk, and quality of life in older adults with hyperkyphosis: A systematic review.

Hyperkyphosis, defined as excessive forward thoracic curvature, is associated with pain, impaired balance, increased fall risk, and reduced quality of life in older adults. Spinal orthoses have been proposed as a therapeutic option. This systematic review evaluated their effects on pain, kyphosis angle, balance, fall risk, and quality of life. The protocol was registered in PROSPERO (CRD42024607584) and followed PRISMA 2020 guidelines. Comprehensive searches of PubMed, Web of Science, Scopus, CINAHL, and Cochrane Library up to November 2024 identified 16 eligible studies involving 757 particip

Effect of a restrictive orthosis on the paralyzed hand in combination with constraint-induced movement therapy in a patient with chronic stroke: a case report.

PURPOSE: Many studies have reported the effects of constraint-induced movement therapy (CIMT) with an orthosis on the residual function of the impaired side of post-stroke patients. We encountered a case with left hemiparesis whose functioning of the impaired hand improved by CIMT with an orthosis not to assist as in the past but to restrict the residual function of the paralyzed fingers. METHODS: The patient was a 46-year-old woman with left hemiparesis due to cerebral infarction 18 months ago. The patient had resumed work but would easily tire while typing the keyboard. We found that the ex

Short-term effect of a carbon fiber custom dynamic orthosis and integrated rehabilitation on self-reported physical function, pain, speed, and agility in civilians.

BACKGROUND: Ankle-foot orthoses (AFOs) are widely used to restore mobility and reduce pain in individuals with lower extremity pain and disability. The use of a carbon fiber custom dynamic orthosis (CDO) with integrated physical training and psychosocial intervention has been shown to improve outcomes in a military setting, but civilian data are limited. OBJECTIVES: To use existing clinical data to evaluate the initial effectiveness of an integrated CDO and rehabilitative program and identify baseline characteristics that impact patient response to the intervention. STUDY DESIGN: Retrospecti

A review on different methods of scoliosis brace fabrication.

Adolescent idiopathic scoliosis is a 3D spinal deformity and mostly affects children in the age group of 10-16 years. Bracing is the most widely recommended nonoperative treatment modality for scoliosis in children. Scoliosis brace fabrication techniques have continuously evolved and currently use traditional plaster casting, computer-aided design (CAD) and computer-aided manufacturing (CAM), or 3D printing. This is a mini narrative literature review. The objective of our study is to conduct a narrative review of traditional, CAD-CAM and 3D printed brace manufacture. A narrative literature rev

Efficacy of extension wrist hand orthosis on pain, grip strength and electromyographic activities in lateral epicondylitis: A randomized single-blind clinical trial.

BACKGROUND: Lateral epicondylitis (LE) is one of the most common work-related complications characterized by pain, decreased grip strength and dysfunction of upper limb. Although conservative management such as physiotherapy and orthosis is reported as first line of treatment, sufficient evidence to decide their effectiveness is lacking and remains controversial. PURPOSE: The current study evaluated and compared the efficacy of a custom-made extension wrist hand orthosis adjunct to usual physiotherapy and therapeutics alone for subjects with LE. STUDY DESIGN: Prospective randomized single-bl

The Kinematic and Kinetic Responses of the Trunk and Lower Extremity Joints during Walking with and without the Spinal Orthosis.

Spinal orthoses are an effective option for restoring the spine to its original position and controlling poor posture. However, the effects of poor posture and spinal orthoses on the kinematics and kinetics of trunk and lower extremity joints remain unclear. A six-camera Vicon motion capture system and two AMTI force plates were employed to collect gait parameters, including joint angle (spine, thorax, hip, knee, and ankle), range of motion (ROM), and ground reaction forces (GRFs). Furthermore, joint moments and joint reaction forces (JRFs) were calculated using a full-body musculoskeletal mod

The effects of knee support on the sagittal lower-body joint kinematics and kinetics of deep squats.

UNLABELLED: Little work has been done to examine the deep squat position (>130° sagittal knee flexion). In baseball and softball, catchers perform this squat an average of 146 times per nine-inning game. To alleviate some of the stress on their knees caused by this repetitive loading, some catchers wear foam knee supports. OBJECTIVES: This work quantifies the effects of knee support on lower-body joint kinematics and kinetics in the deep squat position. METHODS: Subjects in this study performed the deep squat with no support, foam support, and instrumented support. In order to measure the fo

Oral appliances and functional orthopaedic appliances for obstructive sleep apnoea in children.

BACKGROUND: Apnoea is a breathing disorder marked by the absence of airflow at the nose or mouth. In children, risk factors include adenotonsillar hypertrophy, obesity, neuromuscular disorders and craniofacial anomalies. The most common treatment for obstructive sleep apnoea syndrome (OSAS) in childhood is adeno-tonsillectomy. This approach is limited by its surgical risks, mostly in children with comorbidities and, in some patients, by recurrence that can be associated with craniofacial problems. Oral appliances and functional orthopaedic appliances have been used for patients who have OSAS a
