Radiological assessment of Paget's disease of bone after treatment with the bisphosphonates EHDP and APD.

The effects of therapy on the osteolytic bone lesions of Paget's disease have been assessed from serial bone radiographs. Changes in the rate of progression of lytic "wedge" lesions were measured and alterations in the texture of lytic "blade" lesions were graded on an empirical scale. Useful matching was possible using standard radiographs, although special care was needed to avoid artefacts from suboptimal positioning, magnification and variation in exposure. Serial radiographs were obtained of 57 lytic blade lesions in 54 patients receiving treatment with the bisphosphonate 1-hydroxyethylid

The 24-h urinary cyclic adenosine 3', 5' monophosphate/creatinine ratio: an useful approach to the diagnosis of parathyroid disorders and function.

24-h urinary cyclic adenosine 3', 5'-monophosphate/creatinine (cAMP/Cr) ratio was assessed in 10 patients with hypoparathyroidism, 6 with primary hyperparathyroidism, 7 with normocalcemic hypercalciuria and recurrent nephrolithiasis, 14 with osteomalacia, 25 with Paget's disease and 53 with symptomatic postmenopausal osteoporosis. In hypoparathyroid subjects the mean values of 24 h cAMP/Cr ratio were significantly lower than the control values, whereas in patients with parathyroid adenoma the mean values were higher and fell after parathyroid surgery. Patients with nephrolithiasis due to absor

Comparison of the ultrastructure of a malignant tumor of the mandible containing giant cells with Paget's disease of bone.

Distinction between lesions of the jaws containing giant cells can be difficult. This study reports the results of the study of the ultrastructure of 30 patients with Paget's disease in comparison with a patient diagnosed as having malignant giant cell tumor of the mandible. The ultrastructure of the malignant giant cell tumor of the mandible was found to contain nuclear inclusions morphologically identical with the 12-15 nm tubules characteristic of the nuclei of osteoclasts in Paget's disease of bone. No clinical or laboratory signs of Paget's disease was found in this patient. A search of t

Trauma in the Pagetoid: beware the non-specific scintigram.

Radionuclide brain scans and cerebral blood flow studies have become highly sensitive but they remain quite nonspecific diagnostic modalities. Previous reports have discussed similarities between Paget's disease of bone and subdural hematoma on radionuclide studies. We encountered a case in which Paget's disease produced all the described manifestations of subdural hematoma. The differential features of the two conditions and their pathophysiological bases are discussed.

[A test for acute hypocalcemia using swine and salmon calcitonin].

The production of acute hypocalcemia was investigated using either 80 U. MRC of salmon calcitonin or 160 1. mrc of pork calcitonin in 117 subjects (58 patients with Paget's disease, 22 with osteoporosis, 22 with Sudeck-Leriche algodystrophy and 15 control subjects). The results obtained with the two types of calcitonin did not differ. Induced acute hypocalcemia was more pronounced in patients with Paget's disease than in the other subjects studied. In patients with Paget's disease there were statistically significant correlations between the hypocalcemia produced and the initial levels of alka

"Skull on Fire": Monostotic Paget Disease of the Skull Bone.

An 81-year-old woman was evaluated for a stroke. CT showed no intracranial abnormalities but diffuse patchy aspect of the neurocranium. An MRI and F-NA PET/CT were performed to differentiate between metastases, Paget disease, hyperostosis frontalis interna, and primary malignancy. MRI yielded no additional findings. F-NA PET/CT showed diffusely increased uptake in the skull and 4 spots with intense uptake. No other suspicious skeletal foci were seen elsewhere. Low-dose CT showed no sign of malignancy elsewhere. Image findings together with elevated serum alkaline phosphatase levels, slightly i

A review of the embryological development and associated developmental abnormalities of the sternum in the light of a rare palaeopathological case of sternal clefting.

A sternal cleft or bifid sternum is a rare anterior chest wall abnormality. Although several cases have been reported in clinical literature, very little reference has been made to this anomaly in palaeopathological texts. This paper presents a case of superior sternal clefting observed in a middle-aged female with concurrent Paget's disease and congenital hyperkyphosis excavated from a 19th century Dutch psychiatric asylum cemetery in Bloemendaal, The Netherlands. The embryological development of the sternum and associated developmental abnormalities are reviewed and a differential diagnosis

Malignant degeneration of multilevel monostotic Paget's disease involving the thoracic spine: an unusual presentation.

Paget's disease is the second most common metabolic bone disease after osteoporosis and is characterized by abnormal bone turnover and remodeling that can lead to pain, pathological fracture, bony deformity and nerve compression syndromes. The lumbar region is the most commonly affected site within the spine followed by the thoracic and cervical spine. Even though the spine is affected very commonly in Paget's disease, malignant degeneration is exceptionally rare. Multilevel monostotic spine involvement due to Paget's disease is very uncommon. An unusual clinico-radiological manifestation of m

Coexistence of the monostatic Paget's disease, sensorimotor neuropathy and elderly onset rheumatoid arthritis.

Paget's disease is a chronic focal disease of the skeleton that affects up to 2-3% of the population over the age of 60. Polymyalgia rheumatica (PMR) is an inflammatory condition of unknown etiology characterized by aching and stiffness in the shoulder, pelvic girdle and the neck. There are two incompletely overlapping subsets of RA that have been recognized: one exhibits the classical RA clinical picture, while the other has a PMR-like onset in later ages of life. We reported a rare case of monostatic Paget's disease, sensorimotor neuropathy and elderly onset rheumatoid arthritis in an elderl

Paget's disease with vanishing bone pattern and spinal fusion.

We present a rare case of Paget's disease involving the spine and pelvis, with unusual imaging features, including severe bony expansion and fatty infiltration, resulting in a "vanishing bone" appearance. Other rare imaging features are the presence of spinal and sacroiliac ankylosis.

Dynamic bone studies of the labyrinthine capsule in relation to otosclerosis.

The pathological perilabyrinthine bone remodelling of otosclerosis is associated with a genetic predisposition and triggered by mechanisms so far unknown. A proposed viral aetiology of otosclerosis originates from a similar concept of Paget's disease. However, at present, it is not clear why a virus should cause otosclerosis, confined to the bony otic capsule with no effects on the general skeleton in some patients, and systemic Paget's disease with only occasional involvement of the bony otic capsule in others. Moreover, the extent and distribution of pathological bone remodelling is differen

Hyperphosphatasia with massive osteoectasia: a 45-year follow-up.

Hyperphosphatasia is a heterogeneous group of disorders characterized by a generalized skeletal disease and increased alkaline phosphatase. Increased bone remodeling secondary to increased osteoclastic activity appears to be the underlying feature of these disorders. These disorders include juvenile Paget's disease, expansile skeletal hyperphosphatasia, hyperostosis generalisata with striations, and Camurati-Engelmann's disease, type II. The genetic mutations for a number of these disorders have been identified. We present a patient with congenital hyperphosphatasia whose clinical and radiogra

Risedronate: a new oral bisphosphonate.

BACKGROUND: Bisphosphonates have been effective in the treatment of osteoporosis and Paget's disease of bone. Risedronate, the newest oral bisphosphonate, is approved by the US Food and Drug Administration for the prevention and treatment of postmenopausal osteoporosis and glucocorticoid-induced osteoporosis and the treatment of Paget's disease of bone. OBJECTIVE: This article reviews current studies of risedronate in osteoporosis and Paget's disease of bone and, to the extent possible, compares risedronate with other bisphosphonates and other therapies. Information on the pharmacokinetics an

[A case of Paget's disease with progressive sensorineural hearing loss involved in the bilateral bony labyrinths].

Paget's disease of the bone is a chronic, progressive disease of unknown etiology characterized by abnormal bony resorption and deposition. It is a common skeletal disease in Europe and North America, while in Japan it is very rare. Paget's disease of the temporal bone has been reported to cause hearing loss frequently. We report a 50-year-old woman with Paget's disease who had progressive bilateral sensorineural hearing loss and right vestibular dysfunction. CT and 3 Dimensional CT (3D-CT) images demonstrated resorption of the entire bony labyrinth and its surroundings on both sides and that
