Neonatal and pediatric extracorporeal membrane oxygenation.

Extracorporeal membrane oxygenation provides an external circulation for the critically ill patient with components capable of supporting the cardiorespiratory system while the patient's innate organs recover from the primary insult. This therapy is often used in children with relatively well-elucidated indications. Although initially used in the setting of neonatal respiratory failure, the use of extracorporeal membrane oxygenation for pediatric respiratory and cardiac indications is not uncommon. The complications and the outcomes of extracorporeal membrane oxygenation compare favorably with

NADPH oxidase: a universal oxygen sensor?

NADPH oxidase is classically regarded as a key enzyme of neutrophils, where it is involved in the pathogenic production of reactive oxygen species. However, NADPH oxidase-like enzymes have recently been identified in non-neutrophil cells, supporting a separate role for NADPH-oxidase derived oxygen species in oxygen sensitive processes. This article reviews the current literature surrounding the potential role of NADPH oxidase in the oxygen sensing processes which underlie hypoxic pulmonary vasoconstriction, systemic vascular smooth muscle proliferation, carotid and airways chemoreceptor activa

Nocturnal oxygen enrichment in sleep apnoea.

We hypothesized that a modest oxygen enrichment, rather than 100% oxygen supplementation as used in previous trials, could result in improvement in ventilatory and cardiac symptoms, in patients with obstructive sleep apnoea (OSA), without jeopardizing the chemostimulant ventilatory drive. This hypothesis was tested in five male patients with OSA in a single-blinded trial consisting of one night spent sleeping in control room air (control night), followed by one night spent sleeping while exposed to air with a 9% enriched oxygen content (oxygen-enriched night). Oxygen enrichment resulted in a s

Issues in neonatal and pediatric oxygen therapy.

This article reviews the complex physiology of oxygen transport in the fetus and neonate, and how it differs from the older pediatric patient and adult. The common causes of respiratory distress unique to the neonatal and pediatric populations are reviewed in detail, including a brief discussion regarding the different modes of mechanical ventilation used in neonates. The increased susceptibility of infants to the toxic effects of oxygen is reviewed.

The site of oxygen limitation in soybean nodules.

In legume nodules the [O2] in the infected cells limits respiration and nitrogenase activity, becoming more severe if nodules are exposed to subambient O2 levels. To identify the site of O2 limitation, adenylate pools were measured in soybean (Glycine max) nodules that were frozen in liquid N2 before being ground, lyophilized, sonicated, and separated on density gradients of nonaqueous solvents (heptane/tetrachloroethylene) to yield fractions enriched in bacteroid or plant components. In nodules maintained in air, the adenylate energy charge (AEC = [ATP + 0.5 ADP]/[ATP + ADP + AMP]) was lower

Hyperbaric oxygen as a therapy of Bell's palsy.

The purpose of this study was to compare the therapeutic effects of hyperbaric oxygen (HBO2) to the effects of prednisone treatment in 79 subjects with Bell's palsy. Patients were randomly assigned either to the HBO2-treated group (n = 42) or to the prednisone-treated group (n = 37). The HBO2 group was exposed to 2.8 atm abs of 100% oxygen for 60 min, twice a day, 5 days a week and was given a placebo orally. The prednisone group was exposed to 2.8 atm abs of 7% O2 (equivalent to 21% O2 in air at normal pressure) following the same schedule as the HBO2 group; prednisone was given orally (total

Reactive oxygen species and the neurodegenerative disorders.

The major neurodegenerative disorders include Alzheimer's disease, amyotrophic lateral sclerosis, Parkinson's disease, and multiple sclerosis. Although their etiology and pathogenesis are unknown, numerous recent studies suggest that oxygen-derived free radicals play an important role. Furthermore, these reactive oxygen species are probably important in brain ischemia and reperfusion, Down's syndrome, and the mitochondrial encephalopathies. In this review, evidence for oxygen-derived free radicals in the pathogenesis of these disorders is discussed.

[Pre-eclampsia and oxygenated free radicals].

Preeclampsia, clinically defined by arterial hypertension, oedema and proteinuria is a frequently occurred complication of pregnancy. This disease seems to be linked to oxidative stress within placenta. The local accumulation of lipid peroxides, resulting from free radicals production increase altered prostacyclin/thromboxane synthesis. Increased production of lipid peroxides, thromboxane and/or cytokines triggered vascular and organic dysfunctions observed in preeclampsia. Changes in lipoprotein metabolism, namely increase in plasma very low density lipoproteins concentrations (VLDL) and oxid

Reactive oxygen metabolites in piglet cryptosporidiosis.

Piglet cryptosporidiosis is characterized by intestinal villous damage and malabsorption, and by reduced NaCl absorption in response to prostaglandins (PGs), which act directly on the epithelium and indirectly through enteric nerves. We hypothesized that phagocyte-derived reactive oxygen metabolite (ROM) production contributed to PG synthesis and altered transport in inflamed ileum. Ileal mucosa from control and infected piglets was analyzed for villous height, PGE2, catalase (an endogenous antioxidant), and malondialdehyde (MDA, a by-product of lipid peroxidation) from d 2-8 after infection.

Chemical toxicity and reactive oxygen species.

Reactive oxygen species (ROS) are cytotoxic, causing inflammatory disease, including tissue necrosis, organ failure, atherosclerosis, infertility, birth defects, premature aging, mutations and malignancy. ROS are produced in the metabolism of drugs and industrial chemicals by (i) one-electron peroxidase oxidations to form cation radicals, (ii) cytochrome P450 metabolism to free radical products, (iii) stabilisation of the ROS-generator, CYP2E1, and (iv) futile cycling of other cytochromes P450. ROS production initiates inflammation which unless quenched may result in chronic inflammatory disea

The giver of oxygen: Hercules Sanche and the Oxydonor.

During the late nineteenth and early twentieth centuries, gas-pipes were among the most popular therapeutic devices available to an unhealthy public. Spurred on by the explosion of print advertising, mail-order gas-pipes were questionable remedies promoted for such diverse conditions as pneumonia and neurasthenia. Though they are an interesting part of the social history of questionable therapeutics, no historian has recently looked in depth at these devices. This paper examines the clinical, social, and economic environment that facilitated the success of the most ubiquitous gas-pipe, the Oxy

[Free oxygen radicals and rheumatic diseases].

The authors assessed in blood some indicators of the reaction of the organism to the formation and action of free radicals in patients with rheumatoid arthritis, with other inflammatory articular diseases selected at random and with degenerative diseases of the joints and spine. From the results ensues that subjects with inflammations of the synovial membrane respond relatively frequently by a rise of indicators of the antioxidant system such as the activity of superoxide dismutase, glutathione peroxidase or the antioxidant capacity of plasma. Obviously a defence reaction of the organism is in

Oxygen radicals and asbestos-mediated disease.

Asbestos fibers are potent elaborators of active oxygen species whether by reactions involving iron on the surface of the fiber, or by attempted phagocytosis of fibers by cell types resident in the lung. The link between production of active oxygen species and the pathogenesis of asbestos-mediated disease has been highlighted by studies outlined here exploring the use of antioxidant scavengers which inhibit the cytotoxic effects of asbestos both in vitro and in vivo. The use of antioxidant enzymes ameliorates the induction of certain genes necessary for cell proliferation, such as ornithine de
