A distributed, collaborative intelligent agent system approach for proactive postmarketing drug safety surveillance.

Discovering unknown adverse drug reactions (ADRs) in postmarketing surveillance as early as possible is of great importance. The current approach to postmarketing surveillance primarily relies on spontaneous reporting. It is a passive surveillance system and limited by gross underreporting (<10% reporting rate), latency, and inconsistent reporting. We propose a novel team-based intelligent agent software system approach for proactively monitoring and detecting potential ADRs of interest using electronic patient records. We designed such a system and named it ADRMonitor. The intelligent agents,

Coronary artery anomalies: A case of the "malignant" left coronary artery and its surgical management.

BACKGROUND: Any variance of the normal coronary vasculature is regarded as a coronary artery anomaly (CAA). An atypical left coronary artery arising from the right aortic sinus of Valsalva has been identified as the rarest CAA and the second leading cause of sudden cardiac death (SCD) in young people. Prompt identification of the anomalous vessel is essential to mitigate early mortality risk. CASE SUMMARY: A 33-year-old male, ex-smoker presented with exertional breathlessness and retrosternal chest tightness for the past 1 year. Past history also revealed a significant decline in his exercise

Identification of new biomarkers of human endometrial receptivity in the natural cycle.

BACKGROUND: Identification of new markers assessing endometrial receptivity may help in improving the clinical outcome of IVF. This study aimed at identifying genes expressed in human endometrium during the implantation window that could be used as such markers. METHODS: A series of normoresponder patients (n = 31) underwent endometrial biopsies (n = 62, 2 per patient) during the early secretory phase, 2 days after the LH surge (LH + 2) and the mid-secretory phase (LH + 7) of the same natural cycle that preceded a new ICSI attempt for male infertility factor. Samples were analyzed using DNA m

Matrix-assisted laser desorption/ionization time-of-flight mass spectrometric identification and antifungal susceptibility analysis of Candida species isolated from patients with invasive yeast infections in five university hospitals.

In this multicenter study, we compared the performance of the Bruker Biotyper MS system and VITEK 2 YST systems for invasive yeast identification, investigated the distribution of isolated species, and evaluated the antifungal susceptibility profiles of Candida albicans, Candida parapsilosis, and Candida tropicalis. In cases of discrepant results lack of identification with either method, molecular identification techniques were employed. We tested 216 clinical isolates, and concordance between the two methods was observed for 192/216 isolates (88.9%). For five unidentified strains (2.3%), an

Derivation and validation of a practical risk score for prediction of mortality after open repair of ruptured abdominal aortic aneurysms in a US regional cohort and comparison to existing scoring systems.

OBJECTIVE: Scoring systems for predicting mortality after repair of ruptured abdominal aortic aneurysms (RAAAs) have not been developed or tested in a United States population and may not be accurate in the endovascular era. Using prospectively collected data from the Vascular Study Group of New England (VSGNE), we developed a practical risk score for in-hospital mortality after open repair of RAAAs and compared its performance to that of the Glasgow aneurysm score, Hardman index, Vancouver score, and Edinburg ruptured aneurysm score. METHODS: Univariate analysis followed by multivariable ana

Bacteremia due to Rochalimaea henselae in a child: practical identification of isolates in the clinical laboratory.

Two closely related species of Rochalimaea, Rochalimaea quintana and Rochalimaea henselae, are nutritionally fastidious but can be cultivated on bacteriologic media from the blood of patients with diverse clinical presentations. We report a case of culture-proven R. henselae bacteremia in a child with persistent fever. Serologic evidence of infection by R. henselae was ascertained by testing sera at two intervals for immunoglobulin G or immunoglobulin M antibodies by enzyme immunoassay and immunoblot. The case isolate and a collection of other strains (R. henselae, R. quintana, and related org

Prospective randomized study on injury of the external branch of the superior laryngeal nerve during thyroidectomy comparing intraoperative nerve monitoring and a conventional technique.

BACKGROUND: The external branch of the superior laryngeal nerve (SLN) is susceptible to injuries during thyroidectomy, causing voice impairment. Intraoperative nerve monitoring may facilitate identification of the nerve, reducing voice impairment. METHODS: A total of 252 patients undergoing thyroidectomy were randomly assigned to group N (the NIM-Response 3.0 system was used) or group C (the conventional technique using the Vari-Stim 3 was used) to identify the external branch of the SLNs. The primary endpoint was the identification rate of the external branch of the SLN. The secondary endpoi

The identification and management of interstitial lung disease in systemic sclerosis: evidence-based European consensus statements.

BACKGROUND: Systemic sclerosis-associated interstitial lung disease (ILD) carries a high mortality risk; expert guidance is required to aid early recognition and treatment. We aimed to develop the first expert consensus and define an algorithm for the identification and management of the condition through application of well established methods. METHODS: Evidence-based consensus statements for systemic sclerosis-associated ILD management were established for six domains (ie, risk factors, screening, diagnosis and severity assessment, treatment initiation and options, disease progression, and

In-house protocol and performance of MALDI-TOF MS in the early diagnosis of bloodstream infections in a fourth-level hospital in Colombia: Jumping to full use of this technology.

BACKGROUND: Bloodstream infections (BSIs) are a major cause of mortality in hospitalized patients. Rapid diagnosis is crucial because any delay in the antimicrobial treatment is associated with an increase in adverse patient outcomes. The application of matrix-assisted laser desorption ionization-time of flight (MALDI-TOF) technology directly to blood cultures permits earlier identification of BSIs and facilitates treatment management. METHODS: A total of 470 positive blood cultures from patient samples were analyzed using Standard Aerobic/F and Anaerobic/F blood culture media. Isolates were

Channel-Adaptive Temporal Modeling with Grouped Convolutional Network and Informer for Seizure Detection and Prediction.

Reliable identification and early prediction of epileptic seizures play a critical role in improving patient outcomes and supporting timely therapeutic decision-making. To address the challenges of spatiotemporal feature coupling and long-range dependency modeling in multi-channel EEG signals, this paper proposes a channel-adaptive temporal modeling framework named GCNN-Informer. The framework follows a three-stage pipeline: a grouped convolutional network (GCNN) extracts local features and selects the five most discriminative channels; a convolutional block attention module (CBAM) performs du

Lessons from the domestic Ebola response: Improving health care system resilience to high consequence infectious diseases.

BACKGROUND: The domestic response to the West Africa Ebola virus disease (EVD) epidemic from 2014-2016 provides a unique opportunity to distill lessons learned about health sector planning and operations from those individuals directly involved. This research project aimed to identify and integrate these lessons into an actionable checklist that can improve health sector resilience to future high-consequence infectious disease (HCID) events. METHODS: Interviews (N = 73) were completed with individuals involved in the domestic EVD response in 4 cities (Atlanta, Dallas, New York, and Omaha), an

Diagnosing Hunter syndrome in pediatric practice: practical considerations and common pitfalls.

UNLABELLED: Mucopolysaccharidosis II (MPS II), or Hunter syndrome, is an X-linked lysosomal storage disorder caused by a deficiency in the enzyme iduronate-2-sulfatase. Affected patients suffer progressive damage to multiple organ systems and early mortality. Two thirds of patients also manifest cognitive impairment and developmental delays. MPS II can be extremely difficult to diagnose before irreversible organ and tissue damage has occurred because of an insidious onset and the overlap in signs and symptoms with common childhood complaints. This is particularly true of patients without cogni

Gastroenteropancreatic neuroendocrine tumours.

Gastroenteropancreatic (GEP) neuroendocrine tumours (NETs) are fairly rare neoplasms that present many clinical challenges. They secrete peptides and neuroamines that cause distinct clinical syndromes, including carcinoid syndrome. However, many are clinically silent until late presentation with mass effects. Investigation and management should be highly individualised for a patient, taking into consideration the likely natural history of the tumour and general health of the patient. Management strategies include surgery for cure (which is achieved rarely) or for cytoreduction, radiological in

Incidence and significance of errors in a patient 'track and trigger' system during an epidemic of Legionnaires' disease: retrospective casenote analysis.

Early warning scoring is designed to be an objective tool to aid identification of hospital patients at risk of deterioration. 'Track and trigger' systems using such scores are widely used but many aspects of scoring have not been clarified. We aimed to document how observations and scores are used in practice as part of a typical track and trigger system. We extracted patient observations and early warning scores from the casenotes of 189 patients admitted to Furness General Hospital during a large outbreak of Legionnaires' disease in 2002. We used these 3739 sets of primary observations to r

Artificial Intelligence in Non-Obstructive Azoospermia: State of the Art in Sperm Retrieval.

Non-obstructive azoospermia (NOA) is the most severe form of male infertility, characterized by defective spermatogenesis and a complete absence of sperm in the ejaculate. Biological fatherhood for affected patients relies on identifying rare foci of active spermatogenesis within the testes through microdissection testicular sperm extraction (micro-TESE) which is the current gold standard approach. Optimizing sperm retrieval is challenging since success depends on both surgical precision and laboratory detection accuracy. Artificial intelligence (AI) has recently emerged as a promising tool to

Findings From Electronic Medical Record Review of 2500 Patients Identified With Lyme Disease Diagnoses, Test Orders, and Antibiotic Orders in the SubLyme Network.

BACKGROUND: Electronic health records provide opportunities for improving Lyme disease surveillance by collecting detailed clinical information and timely, scalable disease incidence estimation. Medical record review can improve understanding of clinical manifestations, how likely and possible cases compare, and potential misclassification of possible cases. METHODS: The SubLyme Network is a collaboration among 5 healthcare systems, a coordinating center, and the Centers for Disease Control and Prevention. The sites each conducted stratified random sampling of 500 potential acute Lyme disease
