Stress Experiences and Support Needs of Nursing Managers in Patient Safety Incidents: A Phenomenological Qualitative Study.

BACKGROUND: The impact of patient safety incidents on healthcare professionals has drawn increasing attention. As key managerial forces on the clinical frontline, nurse managers not only bear important responsibilities in incident response but may also become "second victims." The pressure they experience and their need for support warrant systematic investigation. OBJECTIVES: To explore the multidimensional stress experiences and support needs of nurse managers during patient safety incidents, providing a theoretical basis and practical guidance for building scientific management and support…

Unreachable patient safety in GP speciality training: a practical intervention to close risk when contact fails.

When patients cannot be reached yet risk exists, GP trainees must make proportionate safety decisions without the stabilising anchor of a live consultation. This 'unreachable patient' problem is common in results handling, radiology follow-up, medication monitoring, and safeguarding, but is rarely taught explicitly as a clinical skill. The Unreachable Patient Algorithm (UPA) is a structured educational intervention that trains trainees to treat reachability as a variable in clinical reasoning. It combines rapid risk stratification, a graded action ladder, and a documentation standard that supp

Preventable Patient Safety Indicators After Lung Resection: Outcomes and Cost at a Large High-Volume Institution.

BackgroundPatient Safety Indicators (PSIs) are quality metrics developed by the Agency for Healthcare Research and Quality (AHRQ) to identify potentially preventable postoperative complications. The rate of PSIs after lung resection remains poorly defined.MethodsWe retrospectively reviewed our institutional database for all lung resection patients age ≥16 from 2014 to 2024. Patient Safety Indicators evaluated were PSI-9 (hemorrhage/hematoma), PSI-10 (acute kidney injury requiring dialysis), PSI-12 (peri-operative pulmonary embolism [PE] or deep vein thrombosis [DVT]), and PSI-13 (post-operativ…

Effectiveness and safety of anti-CGRP monoclonal antibodies in hemiplegic migraine: an individual patient quantitative analysis.

BACKGROUND: Hemiplegic migraine (HM) is a rare and disabling subtype of migraine with aura characterized by reversible motor weakness. Preventive treatment remains challenging, as patients with HM are systematically excluded from RCTs. Monoclonal antibodies targeting the calcitonin gene-related peptide pathway (anti-CGRP mAbs) have shown effectiveness in migraine with typical aura, but evidence in HM is limited. METHODS: This systematic review and quantitative synthesis followed PRISMA guidelines. PubMed, Scopus, and Web of Science were searched for studies reporting patients with HM treated w

Evaluating the Efficacy, Safety, and Patient Selection Criteria for Radiation Therapy in Early-Stage Dupuytren's Contracture.

Dupuytren's contracture (DC) is a fibroproliferative disorder characterized by progressive contractures of the hand. The abnormal thickening of the palmar fascia can significantly impair hand function and lead to progressive pain, significantly reducing patients' quality of life. Surgical interventions are commonly employed for moderate to severe cases and have long been considered the gold standard. Surgery, however, is often associated with morbidities, including complications, infection, and a significant rate of recurrence. Modern-day radiation therapy (RT) has emerged as a promising, thou

Patient Expectations, Safety Concerns, and Adherence to Topical Acne Medications: Findings from a Web-Based Survey.

BACKGROUND: Suboptimal adherence to first-line topical medications (eg, topical retinoids, benzoyl peroxide, and topical antibiotics) is a major barrier to effective acne vulgaris (AV) management. The influence of patients' pre-treatment expectations and concerns about adverse effects on adherence behavior remains inadequately quantified. OBJECTIVE: To evaluate young patients' perceptions of acne and their expectations, concerns, and adherence regarding topical medications. METHODS: A web-based cross-sectional survey was conducted from December 2023 to January 2024. Patients aged 16-29 with

The need for materiovigilance in leprosy-treating hospitals in India: a critical perspective on patient safety.

INTRODUCTION: Leprosy remains a major public health challenge in countries like India, Brazil, and Indonesia. The materiovigilance program was established to monitor the safety of medical devices and helps to protect health-care workers and patients from device-related injuries. Leprosy patients suffer from sensory loss and are at high risk of developing injuries with medical devices. Therefore, integrating materiovigilance program with the national leprosy care program is essential to improve the safety and wellbeing of leprosy patients. AREAS COVERED: This critical perspective covers the ro

Reported patient safety incidents in radiology - understanding the relation between risk areas and their underlying factors.

INTRODUCTION: Learning from errors and underlying causes creates important knowledge, enabling re-evaluation of routines and preventing recurrence of errors. Because of the nature of activities in radiology, measurement of specific risk areas is needed. The aim of this study was to analyse reported events in radiology and identify the underlying causes in relation to the risk. METHODS: Patient Safety Reporting System (PSRS) data from three radiological clinics were retrieved, covering 923 risk events. The reports were studied deductively based on a risk assessment framework (i.e. risk areas) …

Evaluation of single-port TORS tongue base resection for obstructive sleep apnea: safety and patient outcomes.

Tongue base resection for obstructive sleep apnea (OSA) using the da Vinci robotic system has been widely reported as a treatment option. Nevertheless, the relatively high incidence of perioperative hemorrhage associated with the single-port (SP) model may limit its adoption compared with the multi-port system. The study is aiming in evaluating the feasibility and safety of our modified protocol of transoral robotic surgery (TORS) tongue base resection for OSA using the SP da Vinci robotic system. Study design is prospective phase II observational trial. Setting is tertiary referral hospital.

The impact of anti-M antibodies on patient blood typing and transfusion safety: a retrospective study.

BACKGROUND: Anti-M antibodies are a group of frequently detected, naturally occurring antibodies, most of which are immunoglobulin M (IgM) antibodies, inactive at 37 °C, and clinically insignificant in blood transfusion experiments, except for their effect on blood groups. Anti-M antibodies may be clinically significant in some exceptional cases, however, such as cardiac surgery, when the core body temperature drops, resulting in hemolytic transfusion reactions. The present study was conducted to explore the interference caused by the presence of anti-M antibodies on blood group identification

Impact of patient safety on outcomes. From prevention to the treatment of post-intensive care syndrome.

Survivors of critical illness may present physical, psychological, or cognitive symptoms after hospital discharge, encompassed within what is known as post-intensive care syndrome. These alterations result from both the critical illness itself and the medical interventions surrounding it. For its prevention, the implementation of the ABCDEF bundle (Assess/treat pain, Breathing/awakening trials, Choice of sedatives, Delirium reduction, Early mobility and exercise, Family) has been proposed, along with additional strategies grouped under the acronym GHIRN (Good communication, Handout materials,
