Extended rotation-transposition of the pectoralis major myocutaneous flap in the repair of lesions over the shoulder.

An extensive area of radionecrosis over the shoulder was treated by side excision and closed with a pectoralis major myocutaneous flap. To reach the surgical defect the vascular pedicle of the flap had to be rotated 180 degrees. No complications were encountered. This result illustrates another application of the use of this flap. It may be prudent to leave a good deal of muscle around the vascular pedicle in those manoeuvres which require large angles of rotation.

Canadian Physicians' Use of Intramuscular Botulinum Toxin Injections for Shoulder Spasticity: A National Cross-Sectional Survey.

Spasticity of the upper extremity can result in severe pain, along with many complications that can impair a patient's activities of daily living. Failure to treat patients with spasticity of the upper limb can result in a decrease in the range of motion of joints and contracture development, leading to further restriction in daily activities. We aimed to investigate the practice patterns of Canadian physicians who utilize Botulinum toxin type-A (BoNT-A) injections in the management of shoulder spasticity. 50 Canadian Physical Medicine and Rehabilitation (PM&R) physicians completed a survey wi

Sternoclavicular joint osteomyelitis; delayed bone resection with muscle flap: A case report.

INTRODUCTION: Sternoclavicular joint (SCJ) osteomyelitis is a very rare condition. Here, we report an uncommon case of a complicated SCJ osteomyelitis in a patient with an anterior chest wall trauma. PRESENTATION OF CASE: A 61-year-old male a known case of dyslipidemia, hypertension (HTN), and type II diabetes mellitus (T2DM). The patient presented with pain and erythema over the right SCJ following trauma to the same location. Two weeks later, the patient presented with erythematous swelling with a sinus discharging pus, although he was discharged on oral antibiotics, analgesics, and had und

The effectiveness of Botulinum Toxin A for treatment of upper limb impairments and dysfunctions in breast cancer survivors: A randomised controlled trial.

OBJECTIVE: To examine the effectiveness of a single Botulinum Toxin A (BTX-A) infiltration in the pectoralis major muscle, in addition to a standard physical therapy (PT) programme on upper limb impairments and dysfunctions after breast cancer treatment. METHODS: Fifty breast cancer patients with persistent pain 3 months after finishing treatment participated in a double-blinded randomised controlled trial. The intervention group received a single BTX-A infiltration. The control group received a placebo (saline) infiltration. Within one week after the infiltration, all patients attended an in

Breast Reconstruction with Perforator Flaps in Poland Syndrome: Report of a Two-Stage Strategy and Literature Review.

BACKGROUND: Poland syndrome is a congenital deformity presenting unilateral hypoplasia of the breast, pectoralis muscle, and ipsilateral skeletal anomalies. In complex female cases, the significant amount of soft tissue needed for reconstruction continues to be a surgical challenge. Perforator flaps offer alternative methods with sufficient tissue volume, minimal donor site morbidity, and natural cosmetic outcome; however, their role in Poland syndrome breast reconstruction is seldom discussed. This study aims to present a new 2-stage strategy for esthetic restoration of the Poland syndrome ch

Axillary subpectoral approach for pacemaker or defibrillator implantation in patients with ipsilateral prepectoral infection and limited venous access.

PURPOSE: The prepectoral approach is the procedure of choice for pacemaker or defibrillator (device) implantation. Epicardial or transiliac approaches are reserved for patients in whom the pectoral approach is not feasible. We studied the viability of the axillary subpectoral approach for implanting devices in patients in whom the standard prepectoral approach was not feasible. METHODS: Devices and leads were extracted from 16 patients with infected devices in the prepectoral position. The contralateral site was unsuitable for reimplantation because of infection or inadequate venous access. O

Is botulinum toxin type A effective in the treatment of spastic shoulder pain in patients after stroke? A double-blind randomized clinical trial.

OBJECTIVE: To determine the efficacy of botulinum toxin type A for the treatment of spastic shoulder pain in patients after stroke. DESIGN: Double-blind randomized clinical trial. PATIENTS: Of 31 patients enrolled from an acute-care hospital in Spain, 2 cases dropped out (drop-out rate 6.5%). Fourteen subjects were treated with infiltration of 500 units of botulinum toxin type A in the pectoralis major muscle of the paretic side, and 15 with a placebo. METHODS: After infiltration, both groups received transcutaneous electrical nerve stimulation for 6 weeks. Patients were assessed by the use

Inferolateral AlloDerm hammock for implant coverage in breast reconstruction.

PURPOSE: Implant reconstruction is commonly performed to reconstruct mastectomy defects or to correct breast hypoplasia. We have been using an inferolateral AlloDerm hammock as an inferior extension of the pectoralis major muscle to provide a mechanical barrier between the implant and skin and to control implant position. METHODS: The inferior border of the AlloDerm hammock is attached inferiorly to the rectus abdominis fascia and laterally to the serratus anterior fascia to create the borders of the implant pocket. The AlloDerm is then sewn to the pectoralis major muscle to enclose the impla

Pectorobrachioepicondylaris musculoaponeurotic band: case description with evidence of median nerve compression.

Upper limb muscle variations can be encountered on imaging or at surgery. We report an unusual muscle and band found during routine dissection of the arm in a cadaver. This case is described and salient literature reviewed. A band was found that traveled from the insertion of the pectoralis major tendon distally and obliquely toward the medial intermuscular septum and medical epicondyle. Fibers of the brachialis were found to interdigitate into the band. A tunnel was formed that carried the median nerve and brachial vessels. Evidence of median nerve compression was observed. We considered this

Integrated transcriptome and metabolome analysis of E17 embryonic pectoral muscle development regulated by 1 % CO2 during the first 10 days of incubation.

Maintaining CO₂ concentration at 1 % during the first 10 days of incubation represents a potential strategy for enhancing hatch quality. However, the effects of this specific CO₂ regulation strategy on embryonic skeletal muscle development have not yet been systematically evaluated. In this study, 400 fertilized Wod 188 broiler eggs were randomly divided into control and treatment groups. The control group was maintained under normal ventilation, with no CO2 introduced. The treatment group maintained the 1 % CO₂ concentration using dynamic carbon dioxide supplementation during the first 10 day

63-year-old man with right biceps and right pectoralis major abscesses: an unusual case of pyomyositis.

Pyomyositis is considered a great mimicker and masquerader. This case is of a 63-year-old man with diabetes who initially presented to the outpatient clinic afebrile with right shoulder pain. His work-up was negative, and he was discharged home. He subsequently presented to the emergency room (ER) two times for worsening right shoulder pain. During his first visit to the ER, his work-up was unremarkable, and he was discharged home. On his return to the ER, he was now febrile with inflammation involving his right upper extremity and right chest wall. Imaging studies of his right upper extremity

Mapping the Existing Return-to-Sport Criteria After Pectoralis Major Tendon Injury in Different Sports: A Scoping Review for Getting Back to the Game.

CONTEXT: It is estimated that only 50% of athletes who sustain pectoralis major tendon injuries return to sport (RTS) despite successful treatment. This limited RTS rate may be due to absence of standardized RTS criteria for such patients. Evidence suggests a significant heterogeneity in RTS guidance across various studies, leading to inconsistency in clinical advice. OBJECTIVE: To examine the existing RTS criteria used in pectoralis major tendon injury in different sporting populations. DATA SOURCES: A comprehensive literature search with defined keywords was conducted in electronic databas

Comparison of two suturing techniques in terms of lead stabilizing efficiency in acute and chronic terms.

BACKGROUND AND AIMS: Cardiac implantable electronic devices (CIED) are frequently used in the treatment of arrhythmias. Maintenance of lead position is a key element for proper functioning of the CIEDs. There are two suturing techniques that are commonly used to anchor the leads to pectoral muscle (simple knot and anchor knot techniques). While there is one in vitro study comparing lead stabilizing efficacy of these two techniques, there is no in vivo study in the literature. In this in vivo study, the efficacy of lead stabilization between these two techniques was compared. METHODS: Twenty r

Hypoplasia of the latissimus dorsi muscle complicating breast reconstruction in Poland's syndrome.

Poland's syndrome includes anomalies of the breast, thorax, and upper extremity. Unilateral hypoplasia of the breast and pectoral muscle are seen most frequently, and the resulting problem of breast reconstruction must be addressed. The latissimus dorsi myocutaneous flap is one of the most useful reconstructive tools in this situation. A case is presented of Poland's syndrome with hypoplasia of the latissimus dorsi muscle-an abnormality not generally associated with Poland's syndrome. The latissimus dorsi muscle should be carefully evaluated preoperatively in such patients, to make sure it is
