A 50-year-old man was admitted with fever and chill sensation 6 months ago. Transthoracic echocardiography (TTE) showed left and right coronary cusp prolapses and a thickened tissue of the aortic curtain. INTRODUCTION: Epicardial access for mapping and ablation of the epicardial substrate may be required in catheter ablation of ventricular tachycardias (VT). However, high complication rates are associated with the standard epicardial access approach. A 25-y-old woman with known neurofibromatosis type 1 and a large anterior mediastinal mass was investigated. 18F-FDG PET/CT revealed a radiotracer-avid anterior mediastinal mass having an SUVmax of 4.3 and demonstrating a hypoactive center. The 123I-metaiodobenzylguanidine SPECT/CT study performed subsequently did not demonstrate any uptake, thereby excluding, for the most part, the diagnosis of paraganglioma or neuroblastoma. At final pathology, a malignant peripheral nerve sheath tumor of the pericardium with areas of chondrosarcomatous and angiosarcomatous differentiation was diagnosed.

The authors report a case of constrictive pericarditis occurring two years after surgical repair of an atrial septal defect in a 37 year-old-man. The diagnosis was made by right heart catheterisation and magnetic resonance imaging. Chronic constrictive epicarditis is a special form of chronic pericarditis. It is characterized by a thickening of the visceral pericardium that becomes adherent to cardiovascular structures, thus impeding the diastolic filling of the ventricles. The parietal pericardium remains soft, without symphysis with the visceral pericardium. We illustrate the diagnostic and therapeutic approaches to this pathology by a clinical case.
