Peroneal Nerve Injury Arising from Harvesting a Contralateral Hamstring Autograft for Cruciate Ligament Reconstruction: A Case Report.

CASE: A 22-year-old man underwent a posterior cruciate ligament reconstruction following a sporting injury. Postoperatively, he developed a new foot drop on the hamstring harvest side that was contralateral to the ligamentous injury. On surgical exploration, a lesion in the peroneal nerve fascicles of the sciatic nerve, 20 cm from the fibula head, was neurolyzed with improvement in the foot drop. CONCLUSIONS: Sciatic nerve injury is a rare complication when harvesting the hamstring. Care of orientation of the tendon stripper blade and direction of harvest is required to avoid injury to the sc

Foot drop secondary to peroneal intraneural cyst arising from tibiofibular joint.

UNLABELLED: An 11-year-old boy presented to our outpatient clinic with a three-month history of pain over the fibular head area and gait difficulty. Surgical exploration revealed a mass arising from the superior tibiofibular joint invading the peroneal nerve along the articular branch of the common peroneal nerve. The pathogenesis of the intraneural ganglion of the peroneal nerve may be an articular origin and superior tibiofibular joint is the central point. LEVEL OF EVIDENCE: V.

Adult-acquired flatfoot deformity following posterior tibialis to dorsum transfer: a case report.

A previous study showed that flatfoot deformity does not develop after posterior tibialis to dorsum transfer in patients with peroneal nerve palsy. Their conclusion was that it is the unopposed pull of the peroneus brevis which leads to the flatfoot deformity in posterior tibial tendon dysfunction. This case report presents a patient who developed a flatfoot deformity after posterior tibialis to dorsum transfer despite nonfunctioning peroneal muscles.

Mononeuropathy multiplex as an uncommon presentation of intravascular lymphoma: A case report.

KEY CLINICAL MESSAGE: Although intravascular lymphoma rarely presents with peripheral neuropathy, learning about this presentation can lead to timely diagnosis and improved prognosis in patients with intravascular lymphoma. ABSTRACT: A 64-year-old man presented with asymmetric paresthesia and subsequent weakness of his feet and a 10 kg weight loss over 40 days. Electrodiagnostic studies revealed distal axonal sensory-motor polyneuropathy with ongoing axonal loss. A peroneal nerve biopsy showed intravascular proliferation of CD-20 positive lymphocytes, which suggested intravascular large B-cel

Workup and Management of Persistent Neuralgia following Nerve Block.

Neurological injuries following peripheral nerve blocks are a relatively rare yet potentially devastating complication depending on the type of lesion, affected extremity, and duration of symptoms. Medical management continues to be the treatment modality of choice with multimodal nonopioid analgesics as the cornerstone of this therapy. We report the case of a 28-year-old man who developed a clinical common peroneal and lateral sural cutaneous neuropathy following an uncomplicated popliteal sciatic nerve block. Workup with electrodiagnostic studies and magnetic resonance neurography revealed i

Distal spinal muscular atrophy.

Spinal muscular atrophies (SMA) are clinically heterogenous group of motor system disorders characterised by progressive pure lower motor neuron involvement. The distal form of SMA is an extremely rare disorder, which presents in the adults and has a relatively slow progression with almost no effect on the patients' life-span. Differential diagnosis of this syndrome include other forms of neuromuscular disorders with peroneal muscular atrophy like hereditary motor sensory neuropathy (HMSN) and distal myopathies, which need exclusion before confirming this rare entity. We present a young male w

Electrodiagnostic evaluation of lower extremity neurogenic problems.

Electrodiagnosis is a powerful tool for evaluating lower extremity disorders that stem from the peripheral nervous system. Electrodiagnostic testing can help differentiate neurogenic versus non-neurogenic causes of complaints such as pain, weakness, and paresthesias. It can help practitioners pinpoint the anatomic location and reveal the underlying pathology in peripheral nerve lesions. This article focuses on the electrodiagnostic evaluation of neurogenic processes that present as foot and ankle symptoms.

Visualization of denervated muscle by gadolinium-enhanced MRI.

Thirty patients presenting with foot drop due to lesions of the peroneal nerve or L5 spinal root were investigated with gadolinium (Gd)-enhanced MRI of the lower leg. Significant enhancement was only seen in the denervated muscles in a pattern appropriate for the distribution of the nerve or root. In a rat model, identical changes in the denervated muscle were reproduced and seen as early as 24 hours after sciatic nerve transection. Thus, Gd-enhanced MRI is a new and sensitive technique to visualize denervated muscle.

Vincristine-Induced Unilateral Foot Drop in a Patient With Diffuse Large B-Cell Lymphoma: A Rare Presentation of Severe Motor Axonopathy.

Vincristine is a cornerstone chemotherapeutic agent in combination regimens for lymphoid malignancies; however, its neurotoxicity remains a major dose-limiting adverse effect. While vincristine-induced peripheral neuropathy (VIPN) commonly presents as a symmetric, sensory-predominant polyneuropathy, isolated unilateral motor neuropathy causing foot drop is exceedingly rare. We report a case of a 64-year-old male with extranodal diffuse large B-cell lymphoma (DLBCL) of the left calf muscle, staged as Ann Arbor stage IE with Revised International Prognostic Index (R-IPI) score of 2, who develope

The Dellon approach to neurolysis in the neuropathy patient with chronic nerve compression.

Impaired glucose tolerance creates a peripheral nervous system that is susceptible to chronic nerve compression. While it is accepted that surgical decompression of the median nerve at the wrist, for carpal tunnel syndrome, is appropriate in the diabetic, application of this concept to the lower extremity has not yet gained widespread recognition. Traditional electrodiagnostic studies demonstrate the presence of neuropathy, but usually are much less able to demonstrate superimposed nerve compression in the lower extremity in the presence of neuropathy. The clinician must rely upon the presence

Neurological manifestations of cytomegalovirus infection in the acquired immunodeficiency syndrome.

Cytomegalovirus (CMV) infection is one of the most important opportunistic infections in AIDS. The most common manifestation of neurological CMV disease in HIV infection is retinitis followed by encephalitis, polyradiculopathy, and multifocal neuropathy. Untreated necrotizing retinitis proceeds to blindness but can readily be diagnosed by ophthalmological examination. CMV polyradiculopathy presents as subacute leg weakness, paraesthesia, and urinary retention. Untreated patients develop ascending paralysis and die within weeks. Multifocal neuropathy commonly affects the radial, ulnar, and pero

Peroneal nerve palsy after total knee arthroplasty. Assessment of predisposing and prognostic factors.

Thirty-two postoperative peroneal-nerve palsies in thirty patients were documented in a retrospective review of 10,361 consecutive total knee arthroplasties performed at one institution from 1979 through 1992. The mean age of the thirty patients was sixty-five years (range, twenty-eight to seventy-eight years). Four of these patients had had a previous proximal tibial osteotomy and five had had a previous lumbar laminectomy. Ten knees (ten patients) had preoperative valgus alignment of 12 degrees or more. A control group of 100 patients who had had total knee arthroplasty during the same perio

Acute painful diabetic neuropathy: an uncommon, remittent type of acute distal small fibre neuropathy.

INTRODUCTION: Acute painful diabetic neuropathy (APDN) is a distinctive diabetic polyneuropathy and consists of two subtypes: treatment-induced neuropathy (TIN) and diabetic neuropathic cachexia (DNC). The characteristics of APDN are (1.) the small-fibre involvement, (2.) occurrence paradoxically after short-term achievement of good glycaemia control, (3.) intense pain sensation and (4.) eventual recovery. In the face of current recommendations to achieve quickly glycaemic targets, it appears necessary to recognise and understand this neuropathy. METHODS AND RESULTS: Over 2009 to 2012, we rep
