Forensic Psychiatric Patients' Experiences of Personal Recovery: A Wilsonian Concept Analysis.

Psychiatric patients' recovery processes have been studied rather extensively, and a relatively clear consensus on what recovery is already exists. We examined whether the personal recovery of forensic psychiatric patients varies from this definition. We conducted a concept analysis to assess the personal recovery of patients in forensic psychiatric hospitals based on 21 articles and then evaluated how our results compared with existing definitions on psychiatric and forensic recovery. On the basis of this comparison, we concluded that the personal recovery of forensic psychiatric patients doe

Difficulties to differentiate mood disorders co-occurring with compulsive gambling. Discussion based on a case study.

Contemporary literature does not take a clear position on the issue of determining civil and criminal liability of persons diagnosed with pathological gambling, and all the more so in case of possible comorbidity of or interference with other mental disorders. Diagnostic difficulties are demonstrated by a clinical picture of a patient with problem gambling who underwent forensic and psychiatric assessments to evaluate the process of making informed (and independent) decisions in view of numerous concluded civil law (mainly financial) agreements. The patient had been examined 5 times by expert

Sociodemographic and diagnostic characteristics of homicidal and nonhomicidal sexual offenders.

The aims of this study were to compare the prevalence of psychiatric disorders and "psychopathy" in homicidal and nonhomicidal sexual offenders and to investigate the specificity of previous studies on psychiatric morbidity of a sample of sexual murderers. Information from court reports of 166 homicidal and 56 nonhomicidal sex offenders was evaluated using standardized instruments (SCID-II, PCL-R) and classification systems (DSM-IV). Sexual murderers were diagnosed more often with a personality disorder (80.1% vs. 50%; p < 0.001), especially schizoid personality disorder (16.3% vs. 5.4%; p < 0

DSM-III-R personality disorders in parents of schizophrenic patients.

This study examines the frequency of DSM-III-R personality disorders in parents of 58 patients who were admitted consecutively to a New York State psychiatric hospital with a first admission for a schizophrenia-like psychosis. For comparison, a control group of 65 families were randomly recruited who were in the same age group and denied any psychiatric history in their immediate families. Significantly more parents of the patients had a diagnosed personality disorder than controls. These were classified as schizoid, schizotypal, histrionic, and sadistic types by DSM-III-R criteria. While para

Would glucagon-like peptide-1 receptor agonists have efficacy in binge eating disorder and bulimia nervosa? A review of the current literature.

Binge eating, eating an abnormally large amount of food in a discrete period of time with a sense of loss of control over eating, is a defining feature of the eating disorders binge eating disorder (BED) and bulimia nervosa (BN). Both BED and BN are important public health problems for which there are few medical treatments. However, almost all drugs with central nervous system-mediated weight loss properties studied thus far in randomized, placebo-controlled trials in persons with BED or BN have been efficacious for reducing binge eating behavior. Glucagon-like peptide-1 (GLP-1) receptor agon

Mental Illness And Brain Disease.

It has become common to say psychiatric illnesses are brain diseases. This reflects a conception of the mental as being biologically based, though it is also thought that thinking of psychiatric illness this way will reduce the stigma attached to psychiatric illness. If psychiatric illnesses are brain diseases, however, it is not clear why psychiatry should not collapse into neurology, and some argue for this course. Others try to maintain a distinction by saying that neurology deals with abnormalities of neural structure while psychiatry deals with specific abnormalities of neural functioning

A traumatic life experience in childhood increases the risk of a psychiatric disorder in the offspring.

A traumatic life experience in childhood is a fundamental risk factor of numerous psychiatric disorders. Recently, studies try to evaluate the mental health consequences on victims' offspring, but few psychiatric disorders were examined, and whether these results apply to the general population remains unknown. In this study, we use the National Epidemiologic Survey on Alcohol and Related Conditions, a large representative sample of American population, in order to estimate the impact on the offspring of a traumatic experience in parents. Besides, we use a well-known risk factor for offspring'

Psychiatric Disorders in Patients With a Diagnosis of Celiac Disease During Childhood From 1973 to 2016.

BACKGROUND & AIMS: Few studies have explored the link between childhood celiac disease and long-term psychiatric comorbidities. We performed a population-based cohort study of associations between childhood celiac disease and psychiatric disorders and investigated whether risk persists into adulthood. METHODS: We performed a nationwide study in Sweden using data from the Epidemiology Strengthened by histoPathology Reports in Sweden cohort. In this cohort, 19,186 children with a diagnosis of biopsy-verified celiac disease from 1973 through 2016 were identified from Sweden's 28 pathology depart

The contribution of transference-focused psychotherapy in general adult psychiatry training: improving trainees' technical confidence and attitude towards people with personality disorders.

AIMS AND METHOD: This study evaluated whether brief teaching sessions on transference-focused psychotherapy (TFP) could improve psychiatric trainees' attitudes and clinical confidence in managing patients with personality disorders. A mixed-methods design was used, combining pre- and post-training validated questionnaires with a focus group discussion. Two 4-h workshops covered TFP theory and techniques, and case discussions. RESULTS: Twenty-six participants completed paired questionnaires. Statistically significant improvements were observed in overall attitudes (Attitudes to Personality Dis

Advances in understanding major depressive disorder.

1. Major depressive disorder is recognized by different, frequently recurring patterns of signs and symptoms that are suggestive of specific subtypes of the disorder, including melancholic, atypical, and psychotic. 2. MDD occurs at younger ages, twice as frequently in women, in conjunction with other psychiatric disorders, in persons with a family history of mood disorders, and is associated with high use of medical services. 3. Research on the biological basis of depression has focused on neurotransmission, neuroendocrine dysregulation, and genetic transmission. 4. Advances in understanding M

Hypothalamic-pituitary-adrenal axis targets for the treatment of epilepsy.

Stress is a common seizure trigger in persons with epilepsy. The body's physiological response to stress is mediated by the hypothalamic-pituitary-adrenal (HPA) axis and involves a hormonal cascade that includes corticotropin releasing hormone (CRH), adrenocorticotropin releasing hormone (ACTH) and the release of cortisol (in humans and primates) or corticosterone (in rodents). The prolonged exposure to stress hormones may not only exacerbate pre-existing medical conditions including epilepsy, but may also increase the predisposition to psychiatric comorbidities. Hyperactivity of the HPA axis

[Work rehabilitation in people with severe mental illnesses].

Work and employment are basic human rights. Work therapy has a long-standing tradition in mental health care. Integrating people with severe mental illness into jobs has always been considered indispensable. Germany has a good legal framework for work rehabilitation and there is solid evidence of effectiveness for a range of interventions. International models of work integration have not been implemented to a sufficient degree and this may be one of the reasons for limited success in work rehabilitation for people with mental illnesses. The need for work rehabilitation measures and the availa

[Cryptococcal meningitis and neuropsychiatric consequences of HIV-infection].

A 49-year-old African-born male was admitted to hospital with an acute psychosis. He had been treated by an internist after being found to have hiv; as a result of non-compliance over a period of about four months his cd4-count had dropped to 40. Six months earlier he had developed a cryptococcal meningitis, which left him a number of neurological and psychiatric symptoms. During his stay in hospital there had to be good collaboration with the specialist in internal medicine whose dual task was to manage the patient's dramatically low cd4-account as well as his psychosis. Cryptococcal meningit

Mental Disorders in Individuals With Exercise Addiction-A Cross-Sectional Study.

Background and Aims: Exercise addiction has not yet been designated as an addictive disorder in the DSM-5 due to a lack of detailed research. In particular, associations with other psychiatric diagnoses have received little attention. In this study, individuals with a possible exercise addiction are clinically assessed, in order to establish a profile of co-occurring psychiatric disorders in individuals with exercise addiction. Methods: One hundred and fifty-six individuals who reported exercising more than 10 h a week, and continued to do so despite illness or injury, were recruited for the s

Explaining lifetime criminal arrests among clients of a psychiatric probation and parole service.

This study examines the extent to which sociodemographic characteristics, clinical characteristics, substance abuse problems, and the array of lifetime criminal behavior may explain lifetime arrests among offenders supervised by the psychiatric probation and parole service. Three hundred twenty-five clients with new cases at a psychiatric probation and parole service in a large urban center were screened for major psychiatric disorders. They were also interviewed for socio-demographic characteristics, mental health treatment history, criminal behavior, and arrest history. Hierarchical block mu

The prognostic utility of personality traits versus past psychiatric diagnoses: Predicting future mental health and functioning.

Past psychiatric diagnoses are central to patient case formulation and prognosis. Recently, alternative classification models such as the Hierarchical Taxonomy of Psychopathology (HiTOP) proposed to assess traits to predict clinically-relevant outcomes. The current study directly compared personality traits and past diagnoses as predictors of future mental health and functioning in three independent, prospective samples. Regression analyses found that personality traits significantly predicted future first onsets of psychiatric disorders (ΔR2=06-.15), symptom chronicity (ΔR2=.03-.06), and func
