The putative involvement of the transabdominal muscles in dysphonia: a preliminary study and thoughts.

OBJECTIVES/HYPOTHESIS: Real-time ultrasound was used as an adjunct to assess patterns of periabdominal musculature in 14 individual with dysphonia and muscle tension dysphonia. MATERIALS: Fourteen individuals with muscle tension dysphonia were evaluated with real-time ultrasound as a part of their initial evaluation and management. RESULTS: In 13 of 14 individuals, there was an imbalance found during phonation between the transversus abdominis muscles (TAs) and internal oblique muscles (IOs), whereby the IOs were found to be overactive and the TAs underactive. After physiotherapy, this patte…

Biomechanical modeling of register transitions and the role of vocal tract resonators.

Biomechanical modeling and bifurcation theory are applied to study phonation onset and register transition. A four-mass body-cover model with a smooth geometry is introduced to reproduce characteristic features of chest and falsetto registers. Sub- and supraglottal resonances are modeled using a wave-reflection model. Simulations for increasing and decreasing subglottal pressure reveal that the phonation onset exhibits amplitude jumps and hysteresis referring to a subcritical Hopf bifurcation. The onset pressure is reduced due to vocal tract resonances. Hysteresis is observed also for the voic…

Adductor spastic dysphonia: heterogeneity of physiologic and phonatory characteristics.

Physiologic and phonatory characteristics of 23 subjects with adductor spastic dysphonia were studied, including examination of the laryngeal appearance by fiberoptic nasoendoscopy, neurologic examination, and measurement of phonatory airflow and speaking fundamental frequency. These characteristics displayed considerable heterogeneity among the subjects investigated. Three patterns of laryngeal appearance during phonation were observed: vocal fold adduction, associated ventricular fold constriction, and approximation of the laryngeal inlet. Eight of 13 subjects demonstrated some neurologic ab…

Functional restoration for recurrent laryngeal paralysis: an experimental study.

The purpose of this investigation was to exclude abductor disturbance and to maintain phonation during recurrent laryngeal nerve paralysis through a new surgical design. This new method consists of implantation of the ansa hypoglossi and sternothyroid muscle pedicle which has been shown to provide sufficient abductor function in a recent publication, and the selective severance of the abductor branch after neurorrhaphy of the recurrent laryngeal which is shown to be a sufficient adductor function in another study. The recovery of sufficient abductor function during inspiration and dyspnea was …

Differences in Glottal Closure and Visibility of the Anterior Commissure during Rigid-90°, Rigid-70°, and Flexible Laryngostroboscopy.

INTRODUCTION: The conventional rigid-90° and rigid-70° laryngostroboscopy has been so far considered the gold standard in assessing the vibratory behavior of the vocal folds and the glottal closure configuration during phonation. Meanwhile, this rigid laryngostroboscopy is more and more replaced by flexible chip-on-tip systems. The aim of this study was to evaluate the influence of these different endoscopic techniques on glottal closure configuration and on visibility of the complete focal fold length including anterior commissure during phonation. METHODS: Twenty-one euphonic subjects were …

Estimation of vocal fold plane in 3D CT images for diagnosis of vocal fold abnormalities.

Vocal folds are the key body structures that are responsible for phonation and regulating air movement into and out of lungs. Various vocal fold disorders may seriously impact the quality of life. When diagnosing vocal fold disorders, CT of the neck is the commonly used imaging method. However, vocal folds do not align with the normal axial plane of a neck and the plane containing vocal cords and arytenoids does vary during phonation. It is therefore important to generate an algorithm for detecting the actual plane containing vocal folds. In this paper, we propose a method to automatically est…

[Velopharyngeal sequels in labial-alveolar-velopalatine clefts. Analysis, conclusions and speech therapy management].

The association of voice and speech defines phonation. Phonation depends on various organs: the lungs for air, the larynx as a vibration generator, and cavities of resonance modeling articulation and tone. The role of the soft palate is important in French and for everyday conversation. Velopharyngeal incompetence is defined as a voice and articulation disruption by anatomic and/or functional deficiency between the soft palate and pharynx. Hypernasality is studied on an acoustic, phonetic, and vocal level and graded in various degrees of hypernasality. The various clinical and perceptive tests…

[Laryngoscopic findings in dysphonia following traumatic midbrain syndrome].

26 patients with traumatic voice disorders were examined laryngoscopically. During the initial mute stage, reflex vocal cord movement could be detected but no intentional movement. A gradual approximation of the vocal cords led to a narrowing of the glottic aperture so that whispering but not voiced phonation, was possible (stage of whispering). The third stage was characterized by the rapid development of a laryngeal (and pharyngeal) spasticity, identifiable by shortened and thickened vocal folds, an increased approximation of the ventricular folds and in a more dorsal position of the epiglot…

Evolutionary loss of complexity in human vocal anatomy as an adaptation for speech.

Human speech production obeys the same acoustic principles as vocal production in other animals but has distinctive features: A stable vocal source is filtered by rapidly changing formant frequencies. To understand speech evolution, we examined a wide range of primates, combining observations of phonation with mathematical modeling. We found that source stability relies upon simplifications in laryngeal anatomy, specifically the loss of air sacs and vocal membranes. We conclude that the evolutionary loss of vocal membranes allows human speech to mostly avoid the spontaneous nonlinear phenomena…

[Electromyography of the larynx with skin surface electrodes].

A new method of skin surface electromyography (EMG) was evaluated. Twelve electrodes were placed symmetrically in the cricothyroid and thyroid lamina region. Raw EMG signals were continuously recorded, filtered, and rectified. Phonation of the vowel /a/ triggered a computer, which thereafter stored a 625-ms EMG recording, starting 375 ms before and ending 250 ms after onset of the audible phonation. Sixteen to 64 of these recordings were averaged to eliminate random noise. Twenty-six healthy subjects and 9 patients with unilateral laryngeal paralysis were studied. EMG activity exhibited great …

Directional Perilaryngeal Functional Muscle Network Indicates Sequential Patterns of Muscle Coordination During Vocal Tasks.

OBJECTIVE: This study, for the first time, investigates the response of a directional perilaryngeal muscle network to a variety of vocal tasks. METHOD: Twelve surface electromyography (sEMG) signals were recorded bilaterally from six muscles around the larynx during ten vocal tasks. Eight subjects (four females) without a history of voice disorders participated in the study. The vocal tasks were divided into three groups, including (i) phonation at distinct levels of loudness and pitch, (ii) single-repetition tasks such as pitch glide, and (iii) reading at different levels of loudness. RESUL…

Regulation of Transglottal Airflow in Speakers With Parkinson's Disease.

The purpose of this study was to investigate how speakers with Parkinson's disease (PD) regulate transglottal airflow during phonation within and across breath groups, compared to healthy older adult (HOA) speakers. Aerodynamic recordings from 22 speakers with PD and 22 HOA speakers were compared. Transglottal airflow was measured from vocalic portions of consonant-vowel syllables and an all-voiced sentence using a pneumotachograph. Mean airflow in vowels and sentences in addition to the regularity (standard deviation) of airflow within and across separate productions (different breath groups)…

Analysis of vocal fold function from acoustic data simultaneously recorded with high-speed endoscopy.

SUMMARY: Acoustic and endoscopic voice assessments are routinely performed to determine the vocal fold vibratory function as part of the voice assessment protocol in clinics. More often than not these data are separately recorded, resulting in information being obtained from two different phonation segments and an increase of time for the voice evaluation process. This study explores the use of acoustic data, simultaneously recorded during high-speed endoscopy (HSE), for the evaluation of vocal fold function. PATIENTS AND METHODS: HSE and acoustic data were recorded from the subjects simultan…
