Duplication.

Congenital limb duplications include pre- and post-axial polydactyly, central polydactyly, and the mirror-hand spectrum. Treatment of these duplications constitutes a significant functional and aesthetic challenge for the reconstructive hand surgeon. This article provides an inclusive review of the embryologic and molecular mechanisms underlying these deformities and focuses on their clinical treatment. The anatomic variation, classification, surgical treatment, and outcomes of surgical intervention are reviewed for each of the disorders of duplication.

The aesthetic jaw line: management of the aging jowl.

The lower third of the face is often an afterthought in aesthetic plastic surgery. Aging across the mandibular border may be described by several mechanisms: fat atrophy and volume loss; shifting of subcutaneous fat compartments; and mandibular septum dehiscence with submandibular fat hypertrophy. Accurate diagnosis of the apparent mechanism of jaw line aging is critical to successful facial rejuvenation. Diagnosis directs the plastic surgeon as to which key anatomic components in this region to manipulate to optimize rejuvenation. The senior author's technique for facial rejuvenation across t

The surgical correction of protuberant ears.

While prominent ears are considered a sign of good fortune in the Far East, Western society looks upon prominent ears in a far less positive manner. Children with prominent ears are often the subjects of verbal and at times physical abuse by their peers, resulting in adverse psychological effects. Advances in otoplasty have made it possible not only to pin back the ears, but also to reshape them, reduce their size, or make them more symmetrical. For a dermatologic surgeon, an otoplasty may be an unfamiliar surgical procedure, however, the surgery itself does not significantly differ from ear w

The Cutler-Beard clamp for lid reconstruction.

PURPOSE: To analyze the use of the Cutler-Beard transmarginal rotation entropion clamp (Karl Ilg & Co., St. Charles, IL) in patients undergoing the Cutler-Beard procedure for repair of eyelid defects after tumor excision or trauma. DESIGN: Prospective, noncomparative case series. PARTICIPANTS: Twenty-five patients undergoing the Cutler-Beard procedure. RESULTS: The Cutler-Beard clamp (Karl Ilg & Co., St. Charles, IL) helps avoid the problems of a commonly used chalazion clamp. The clamp allows a skin orbicularis muscle incision to be combined with a conjunctival tarsal incision without the

[Personal treatment of congenital penile curvature].

Severe type I congenital curvature of the penis is characterised by an almost transparent hypoplasia uretra. Treatment consists of resecetion of the dysplasic urethra and its replacement by tubular grafts of preputial or extragenital skin or of the vesical or buccal mucous membrane. We present our experience of four patient with type I congenital curvature of the penis which we treated with a simple surgical technique consisting of leaving the over-lying skin attached to the hypoplasia urethra and resecting the remaining fibrotic tissue which is connected to and retracts the skin and the caver

Applicability of Yale-Brown Obsessive-Compulsive Scale Modified for Body Dysmorphic Disorder (BDD-YBOCS) for the Assessment of Patients Eligible for Facial Esthetic Surgery: A Scoping Review.

INTRODUCTION: Although the Yale-Brown Obsessive-Compulsive Scale Modified for Body Dysmorphic Disorder (BDD-YBOCS) is a complete questionnaire for evaluating BDD, there are few studies in the literature to investigate or screen for dysmorphism in patients undergoing facial plastic surgery. OBJECTIVE: To map and evaluate facial plastic studies that used this questionnaire to research body dysmorphism in their populations. The secondary objectives were to review the criteria and questions assessed by the questionnaire and their applicability. METHODS: The study selection criteria involved stud

Toxic shock syndrome following latissimus dorsi musculocutaneous flap breast reconstruction.

Acute toxic shock syndrome (TSS) not related to menstruation has comprised an increasing proportion of cases reported to the Center for Disease Control in recent years. We report a patient with TSS resulting from a postoperative donor site infection after bilateral breast reconstruction with latissimus dorsi musculocutaneous flaps. The diagnosis, management, and outcome of TSS are reviewed. The plastic surgeon should be aware of TSS as a potential postoperative complication of routine procedures in plastic surgery.

The Horseshoe Labiaplasty: Problems and Pearls.

Aesthetic vulvovaginal surgery has become the final frontier of cosmetic surgery. Accordingly, there has been an increased interest in plastic surgical procedures for the female genitalia. Labiaplasty is the most common among such procedures, and the number of practitioners and techniques has varied widely in the medical literature. This article will focus on the problems and pearls encountered by the author in treating enlargement of the labia minora and clitoral hood. The content will focus on the technical aspects of the labiaplasty surgery and ways to avoid common complications encountered

[Expert consensus on the clinical diagnosis and treatment of Congenital syndactyly (2025 Edition)].

Syndactyly is one of the most common congenital malformations of the hand/foot, characterized by varying degrees of soft tissue and/or skeletal fusion between adjacent fingers or toes. It may also occur alongside polydactyly, camptodactyly, brachydactyly, or congenital joint fusion. The heterogeneity in its clinical phenotypes and incomplete penetrance makes it challenging to establish clear diagnostic and treatment protocols/procedures. Conventionally, the management of syndactyly has mainly relied on clinical experience, with limited reference to cutting-edge genetic research and systematic

Metoidioplasty.

Many patients who desire metoidioplasty want and need masculine appearing and functioning genitalia. A key part of masculinization involves moving tissue anteriorly. Standing micturition requires urethral lengthening. Vaginectomy, scrotoplasty, and perineal reconstruction are frequent adjunct procedures. Aesthetic and urinary complications are unfortunately common. With the various techniques and options available, shared decision making between surgeons and patients is vital to ensure the surgery needed is achievable with the surgeon's skill and the patient's anatomy.
