A Clinical Quality-Assurance Panel for Patient-Facing Black-Box Large Language Models in Healthcare: Anchored Forced-Choice Drift Monitoring for Sensitive Communication.

Background Patient-facing large language models (LLMs) are being integrated into patient portals, automated health messaging, psychoeducation, and mental health-related support. Because many deployments are vendor-hosted black boxes that may change without local visibility, health systems need practical quality-assurance methods that can establish a baseline behavioral signature and flag candidate changes in how sensitive topics are framed. Methods This feasibility study applied an anchored two-alternative forced-choice (2AFC) protocol to ChatGPT 5.4 (OpenAI, San Francisco, California, United

Micro- and nanoplastics are elevated in femoral atherosclerotic plaques compared with undiseased arteries.

OBJECTIVE: As plastic production continues to accelerate, the byproducts increasingly fill the environment. Once degraded into micronanoplastics (MNPs), particles may circulate into food, drinking water, or air. Nascent literature has demonstrated MNP bioaccumulation within human tissues, such as the blood, brain, and solid organs. Only recently have MNPs been identified within thrombi and atherosclerotic plaques of diseased blood vessels, and these findings have been associated with adverse clinical outcomes. Data on MNP content in infrainguinal arterial occlusive disease is currently lacking

[Method for assessment of distribution of UHMWPE wear particles in periprosthetic tissues in total hip arthroplasty].

PURPOSE OF THE STUDY: Aseptic loosening of implants is the main complication affecting the longevity of joint prostheses. The highest proportion of loosening occurs due to osteolysis produced by the presence of ultra-high molecular weight polyethylene (UHMWPE) wear particles smaller than 1 microm. These can be identified by microscopic, spectroscopic or light-scattering methods. Here we describe our method for counting wear particles, based on the principle of light scattering. MATERIAL AND METHODS: Between 2002 and 2004, we collected samples of polyethylene granuloma in 19 patients who under

[Simultaneous determination of 15 bisphenols in prepared dishes by soild phase extraction purification-ultra performance liquid chromatography-tandem mass spectrometry].

Bisphenols (BPs), which include bisphenol A and its analogs (such as bisphenol S, bisphenol AF, and bisphenol B), are chemical substances that are synthesized artificially. BPs are used in epoxy resins and polycarbonate plastics and are widely found in food packaging and beverage containers. BPs are endocrine disruptors; hence, they can disrupt the natural hormonal activities of the human body, impact the neurological development of children, and affect intestinal microbial communities in the body, leading to obesity. Consequently, BPs pose threats to human health. The prepared dishes are usua

Association of mixed exposure to microplastics with sperm dysfunction: a multi-site study in China.

BACKGROUND: Microplastics are environmental pollutants detected in various human organs and tissues. These particles originate from multiple sources including the degradation of larger plastic items and the intentional inclusion in consumer goods. Potential risks for human health resulting from microplastics exposure have also been reported. However, the distribution in the male reproductive system and its effect remains largely unknown. This study aims to investigate the presence of multiple microplastics in human semen and urine and their association with sperm quality in a multi-site study

Lymphoid tissue targeting of anti-HIV drugs using liposomes.

Considering that HIV-1 accumulates and replicates actively within lymphoid tissues, any strategy that will decrease viral stores in these tissues might be beneficial to the infected host. Follicular dendritic cells (FDC), B lymphocytes, antigen-presenting cells like macrophages, and activated CD4(+) T cells are abundant in lymphoid tissues, and all express substantial levels of the HLA-DR determinant of the major histocompatibility complex class II (MHC-II). Monocyte-derived macrophages, which are also CD4(+) and express HLA-DR, are considered to be the most frequent hosts of HIV-1 in tissues

Ameliorative mechanism of dietary vitamin d and magnesium on newborn's pulmonary toxicity induced by cadmium.

Cadmium (Cd) exposure in mothers can cause respiratory issues in newborns, but the exact toxicity mechanisms are not fully understood. Vitamin D deficiency in Cd-exposed rats is associated with increased cadmium accumulation in tissues. Finding a cost-effective medication that is vital for the body while also reducing the effects of poisoning is crucial in treating poisonings. To investigate the mechanisms of Cd-induced lung toxicity, we examined the impact of prolonged Cd exposure in female rats before pregnancy on newborn lung health, focusing on sera TNF-α level, lung P53, Foxo1 mRNA, and l

Transferrin-conjugated SNALPs encapsulating 2'-O-methylated miR-34a for the treatment of multiple myeloma.

Stable nucleic acid lipid vesicles (SNALPs) encapsulating miR-34a to treat multiple myeloma (MM) were developed. Wild type or completely 2'-O-methylated (OMet) MiR-34a was used in this study. Moreover, SNALPs were conjugated with transferrin (Tf) in order to target MM cells overexpressing transferrin receptors (TfRs). The type of miR-34a chemical backbone did not significantly affect the characteristics of SNALPs in terms of mean size, polydispersity index, and zeta potential, while the encapsulation of an OMet miR-34a resulted in a significant increase of miRNA encapsulation into the SNALPs.

Biocompatibility in hemodialysis: artificial membrane and human blood interactions.

Hemodialysis, a cornerstone therapy for chronic kidney disease, represented a crucial advance in the evolution of artificial organs. While its success is largely due to its efficiency in removing uremic toxins, an equally important challenge is to uphold the primum non nocere principle by minimizing the harmful effects of membrane-blood interactions. This review examines the complex mechanisms and key interactions underlying membrane biocompatibility, including complement activation, inflammation, and coagulation disturbances, paving the way for their clinical implications. We also summarize r

Nanoparticle T-cell engagers as a modular platform for cancer immunotherapy.

T-cell-based immunotherapy, such as CAR-T cells and bispecific T-cell engagers (BiTEs), has shown promising clinical outcomes in many cancers; however, these therapies have significant limitations, such as poor pharmacokinetics and the ability to target only one antigen on the cancer cells. In multiclonal diseases, these therapies confer the development of antigen-less clones, causing tumor escape and relapse. In this study, we developed nanoparticle-based bispecific T-cell engagers (nanoBiTEs), which are liposomes decorated with anti-CD3 monoclonal antibodies (mAbs) targeting T cells, and mAb

In vivo activity of miR-34a mimics delivered by stable nucleic acid lipid particles (SNALPs) against multiple myeloma.

Multiple myeloma (MM) is a disease with an adverse outcome and new therapeutic strategies are urgently awaited. A rising body of evidence supports the notion that microRNAs (miRNAs), master regulators of eukaryotic gene expression, may exert anti-MM activity. Here, we evaluated the activity of synthetic miR-34a in MM cells. We found that transfection of miR-34a mimics in MM cells induces a significant change of gene expression with relevant effects on multiple signal transduction pathways. We detected early inactivation of pro-survival and proliferative kinases Erk-2 and Akt followed at later

Recent advances in biomimetic nanodelivery systems for cancer Immunotherapy.

Tumor immunotherapy is a developing and promising therapeutic method. However, the mechanism of tumor immune microenvironment and individual differences of patients make the clinical application of immunotherapy still very limited. The resulting targeting of the tumor environment and immune system is a suitable strategy for tumor therapy. Biomimetic nanodelivery systems (BNDS) coated with nanoparticles has brought new hope for tumor immunotherapy. Due to its high targeting, maximum drug delivery efficiency and immune escape, BNDS has become one of the options for tumor immunotherapy in the fut

Applications of nanotechnologies for miRNA-based cancer therapeutics: current advances and future perspectives.

MicroRNAs (miRNAs) are short (18-25 nt), non-coding, widely conserved RNA molecules responsible for regulating gene expression via sequence-specific post-transcriptional mechanisms. Since the human miRNA transcriptome regulates the expression of a number of tumor suppressors and oncogenes, its dysregulation is associated with the clinical onset of different types of cancer. Despite the fact that numerous therapeutic approaches have been designed in recent years to treat cancer, the complexity of the disease manifested by each patient has prevented the development of a highly effective disease
