Study of the human liver by ultrasound examination to search for variations of venous anatomy.

INTRODUCTION: Knowledge about venous patterns (especially portal and hepatic venous) and their variations are extremely crucial for liver transplantation and other surgical procedures on liver. Studies have been done at national or international level on variations of hepatic venous patterns by CT or MRI or by dissection on cadaveric liver. No definitive or authentic study on this topic could be noted in electronic and print media, as well as in standard textbook. Considering low cost and noninvasive nature, we utilized ultrasonography to bridge this lacuna. AIM: To estimate prevalence and ty

Budd-Chiari Syndrome With Spontaneous Intrahepatic Portosystemic Shunts: A Case Series.

Intra-hepatic portal-venous collaterals are characteristic of Budd-Chiari syndrome (BCS) and are usually of small caliber and seen on Doppler. Creation of large portal-systemic shunt, either radiologically (Transjugular intrahepatic porto-systemic shunt) or surgically results in excellent long term outcomes in BCS. Here, we report a series of three rare cases of asymptomatic BCS, who had spontaneous large intra-hepatic portal-systemic shunts.

Noncirrhotic portal vein thrombosis exhibits neuropsychological and MR changes consistent with minimal hepatic encephalopathy.

Hepatic encephalopathy can arise from portal-systemic shunting in the absence of intrinsic liver disease. However, there are few descriptions of this form of encephalopathy. Portal vein thrombosis is an infrequent disease that causes portal-systemic shunting. Episodic hepatic encephalopathy has been described in patients with portal vein thrombosis, but it is not known if these patients develop minimal hepatic encephalopathy. We designed a study to investigate the neurological consequences of portal vein thrombosis in patients without cirrhosis and no clinical signs of encephalopathy. For this

Role of interventional radiology in the diagnosis and management of congenital extrahepatic portosystemic shunts: Two case reports.

Congenital extrahepatic portosystemic shunt (CEPS) is a rare splanchnic venous malformation, wherein the portal venous outflow drains into t … ith an asymptomatic hyperammonemia and was found to have a Type 1 CEPS with absence of intrahepatic portal system. The second patient was asymptomatic and was incidentally found to have a Type 2 CEPS on imaging with normal intrahepatic portal system. Both patients were successfully treated with endovascular occlusion of the CEPS.

Cholestasis in children with portal vein obstruction.

We describe cholestasis as a result of bile duct abnormalities in 8 children with portal vein obstruction. In a clinical, biochemical and radiological investigation of 121 children with cavernous transformation of the portal vein seen between 1986 and 2000, 8 presented with jaundice, pruritus, and/or raised serum aminotransferases and/or gamma glutamyl transpeptidase (gamma GT) activities. Each displayed dilation and narrowing of intra- and/or extrahepatic bile ducts. Surgical decompression of the portal system (portal-systemic or Rex anastomosis) resulted in the regression of the signs of cho…

Angiographic evaluation after portal blood flow preserving operations.

We carried out a prospective study comparing postoperative portal angiographic characteristics in patients with hemorrhagic portal hypertension treated electively by two different portal blood flow preserving procedures. Between 1986 and 1991, a total of 81 patients underwent operation: 38 selective shunts and 43 Sugiura-Futagawa procedures. Cirrhosis was shown by biopsy in 50 cases, 26 of them secondary to alcoholism. We found rebleeding in 5% of the patients, incapacitating postoperative encephalopathy in 8%, and an operative mortality of 7%. Fifty-two cases were evaluated in the postoperati

Portal Venous and Mesenteric Wall Calcifications along with Intra-splenic Vascular Calcification: A Rare Case Report and Literature Review.

BACKGROUND: Cross-sectional imaging findings of portal vein calcification were found to be an indicator of portal vein thrombosis. Calcification of the portal vein and its tributaries in portal hypertension is an uncommon finding and the mechanical stress due to long-standing portal hypertension on the vessel wall is considered leading to sclerosis and calcification in the intima and media of the vessel wall. Intra-splenic vascular calcification is a rare occurrence and has been seen in patients with portal hypertension. CASE PRESENTATION: A 60-year-old male was referred for an ultrasound sca

TIPS Placement via Combined Transjugular and Transhepatic Approach for Cavernous Portal Vein Occlusion: Targeted Approach.

Purpose. We report a novel technique which aided recanalization of an occluded portal vein for transjugular intrahepatic portosystemic shunt … e balloon was targeted and punctured from the transjugular approach, and a wire was passed into the portal system. TIPS placement then proceeded routinely. Conclusion. Although occlusion of the portal vein increases difficulty of performing TIPS, it should not be considered an absolute contraindication. Congenital mesenterico-portal (Rex) shunt.

Congenital anomalies of the portal and umbilical-venous system are rare, with few reported cases. We describe a case where the native umbilical vein had acquired mesenteric venous outflow to bypass an in-utero portal-vein (PV) obstruction and restore normal venous return to the liver. This is analogous to the recently described Rex shunt procedure used in some cases of extrahepatic portal hypertension due to PV occlusion.

[Complications of transcutaneous transhepatic endovascular interventions in patients with portal hypertension].

Results of endovasal treatment of 196 patients with portal hypertension and its complications are analysed. The total rate of complications is 23.4%, the most serious of them were intraabdominal bleeding (2.5%), portal vein thrombosis (2.1%). The rate of mortality, related to the complications of transhepatic procedures, was 2.5%. The majority of the complications were specific to the character of the disease: liver cirrhosis, ascites, high portal pressure, coagulating system disorders.

Thrombolytic treatment of portal thrombosis.

BACKGROUND/AIMS: The authors present their experience with thrombolytic treatment of "acute" portal thrombosis. METHODOLOGY: Since 1980, po … nts are alive, the time of follow-up ranging from 9 months to 6 years. In 8 cases, five years after portal system recanalization the first symptoms of portal hypertension occurred. CONCLUSIONS: Thrombolytic treatment of acute portal thrombosis, if administered promptly, appears to be the only way to improve, or even restore, the portal system patency.

[Upper abdominal sonography in total abnormal pulmonary vein confluence into the portal vein].

Upper abdominal sonography demonstrated an asplenia-syndrome and a massive dilatation of the portal vein and its intrahepatic branches in a newborn infant with angiocardiographically confirmed total anomalous pulmonary venous return to the portal vein. A common venous trunk (vertical vein) could be traced from behind the heart to the sinus-like dilated portal vein. Because demonstration of the vertical vein may not be possible with the sector scanner below the diaphragm, we consider sonographic demonstration of a dilated portal vein as an important sign for total anomalous pulmonary venous ret

Parallel trajectories in the discovery of the SCN-OVLT and pituitary portal pathways: Legacies of Geoffrey Harris.

A map of central nervous system organization based on vascular networks provides a layer of organization distinct from familiar neural networks or connectomes. As a well-established example, the capillary networks of the pituitary portal system enable a route for small amounts of neurochemical signals to reach local targets by traveling along specialized pathways, thereby avoiding dilution in the systemic circulation. The first evidence of such a pathway in the brain came from anatomical studies identifying a portal pathway linking the hypothalamus and the pituitary gland. Almost a century lat…

Congenital absence of the portal vein with splenomegaly and hypersplenism in a young woman.

Congenital absence of portal vein (CAPV) with a systemic shunt of enteric blood is a rare malformation. Twenty-seven cases have been reported to date; with the exception of 4 patients all cases had either liver anomalies or cardiac anomalies. CAPV is usually diagnosed before the age of 18 (25/27 cases). Here we present the case of a 33-year-old woman with CAPV without further anomalies. Beside suffering from circumscript scleroderma, splenomegaly and hypersplenism, the patient was otherwise healthy. Diagnosis was based on histological findings, angiography and CT scan. The liver biopsy showed

Portal hypertension and variceal hemorrhage.

Portal hypertension, a major hallmark of cirrhosis, is defined as a portal pressure gradient exceeding 5 mm Hg. In portal hypertension, porto-systemic collaterals decompress the portal circulation and give rise to varices. Successful management of portal hypertension and its complications requires knowledge of the underlying pathophysiology, the pertinent anatomy, and the natural history of the collateral circulation, particularly the gastroesophageal varices.

Intrahepatic portal vein sclerosis in patients without a history of liver disease. An autopsy study.

Portal fibrosis with portal venous obliteration is characteristic of noncirrhotic portal hypertension, but similar lesions are also commonly seen in patients without clinical evidence of liver disease. Thus, the ability to predict the presence of portal hypertension with histologic criteria will probably depend on a quantitative assessment of liver tissue. The purpose of this study is to provide a quantitative basis in a "normal" population for evaluation of portal vein obliteration. We reviewed 414 consecutive autopsies of patients without known history of alcoholism or clinical liver disease

A pathological scoring system in the diagnosis and judgment of prognosis of biliary atresia.

OBJECTIVE: The purpose of this study was to evaluate the diagnostic and prognostic value of a histological scoring system in biliary atresia (BA). METHODS: From June 2013 to July 2014, 86 wedge liver biopsy specimens were obtained from infants with neonatal cholestasis (58 patients with biliary atresia and 28 patients with non-obstructive cholestasis as control) in our center. A pathologist, single-blinded to the final diagnosis, made the histological diagnosis individually based on an 8-feature (liver fibrosis, portal ductal proliferation, bile plugs in portal ductules, cholestasis, hepatoce

Transjugular intrahepatic portosystemic shunt in patients with cirrhosis: Indications and posttransjugular intrahepatic portosystemic shunt complications in 2020.

Transjugular intrahepatic portosystemic shunt is a percutaneous radiologic-guided procedure that aims to reduce portal hypertension by creating a shunt between the portal venous system and the hepatic venous system. The most common cause of portal hypertension is liver cirrhosis in Western countries. Two main indications of transjugular intrahepatic portosystemic shunt are validated by randomised controlled studies in patients with cirrhosis and variceal bleeding (salvage transjugular intrahepatic portosystemic shunt, early transjugular intrahepatic portosystemic shunt or rebleeding despite an

Portal hypertension.

Portal hypertension is the main complication of cirrhosis and is responsible for its most common complications: variceal hemorrhage, ascites, and portosystemic encephalopathy. Portal hypertension is the result of increased intrahepatic resistance and increased portal venous inflow. Vasodilatation (splanchnic and systemic) and the hyperdynamic circulation are hemodynamic abnormalities typical of cirrhosis and portal hypertension. Gastroesophageal varices result almost solely from portal hypertension, although the hyperdynamic circulation contributes to variceal growth and hemorrhage. Ascites re
