Defective iodotyrosine deiodinase activity may benefit from a specific treatment, thus requiring an unequivocal diagnosis. In reported cases this diagnosis has been obtained from an in vivo deiodination test making use of di-iodotyrosine (DIT) labeled either with I-131 or I-125. Dosimetric calculation indicates that such tests may result in unacceptable irradiation of the thyroid of a child wrongly suspected of having defective iodotyrosine deiodinase activity; therefore other methods are needed. Two rare forms of sporotrichosis were seen in one hospital within a two-year period. One, a case of primary sporotrichosis of the lung, represents the eighth case on record, to our knowledge. The other, a case of articular sporotrichosis, is not as rare, but is still uncommon manifestation of the disease. The problems in management of these two patients are of interest because of the challenge of conventional concepts of therapy.

Sporotrichosis is endemic in temperate and tropical regions of the world, including Australia. Although uncommon, it is frequently encountered in Northern Australia, especially in Queensland. All ages are affected, although most cases occur from occupational exposure to vegetation or soil.
