Immunotherapy for the Treatment of Acute Lymphoblastic Leukemia.

PURPOSE OF REVIEW: Immunotherapy for the treatment of acute lymphoblastic leukemia (ALL) broadens therapeutic options beyond chemotherapy and targeted therapy. Here, we review the use of monoclonal antibody-based drugs and cellular therapies to treat ALL. We discuss the challenges facing the field regarding the optimal timing and sequencing of these therapies in relation to other treatment options as well as considerations of cost effectiveness. RECENT FINDINGS: By early identification of patients at risk for leukemic relapse, monoclonal antibody and cellular immunotherapies can be brought to

BCR-ABL1-like B-Acute Lymphoblastic Leukemia/Lymphoma: A Comprehensive Review.

CONTEXT.—: In the 2016 update of the World Health Organization (WHO) classification of hematopoietic neoplasms, BCR-ABL1-like B-acute lymphoblastic leukemia/lymphoma (B-ALL) is added as a new provisional entity that lacks the BCR-ABL1 translocation but shows a pattern of gene expression very similar to that seen in B-ALL with BCR-ABL1. OBJECTIVE.—: To review the kinase-activating alterations and the diagnostic approach for BCR-ABL1-like B-ALL. DATA SOURCES.—: We provide a comprehensive review of BCR-ABL1-like B-ALL based on recent literature and the 2016 update of the World Health Organizati

The effect of co-occurring lesions on leukaemogenesis and drug response in T-ALL and ETP-ALL.

Despite advances in the management of acute lymphoblastic leukaemia (ALL), current regimens fail to significantly transform outcomes for patients with high-risk subtypes. Advances in genomic analyses have identified novel lesions including mutations in genes that encode chromatin modifiers and those that influence cytokine and kinase signalling, rendering many of these alterations potentially targetable by tyrosine kinase and epigenetic inhibitors currently in clinical use. Although specific genomic lesions, gene expression patterns, and immunophenotypic profiles have been associated with spec

Addressing Administration Challenges Associated With Blinatumomab Infusions: A Multidisciplinary Approach.

Blinatumomab has shown great potential for patients with chemotherapy-resistant B-cell acute lymphocytic leukemia. Blinatumomab's toxicity profile includes central nervous system toxicities, as well as cytokine release syndrome. Although neurological toxicities associated with blinatumomab are almost always reversible, early detection and intervention of these toxicities is vital to ensure that patients continue their full course of treatment. Guidelines for the preparation and administration of blinatumomab in both inpatient and outpatient settings, as well as a standardized neurological nurs

Genetic susceptibility in childhood acute lymphoblastic leukemia.

Acute lymphoblastic leukemia (ALL) is the most common childhood malignancy and a leading cause of death due to disease in children. The genetic basis of ALL susceptibility has been supported by its association with certain congenital disorders and, more recently, by several genome-wide association studies (GWAS). These GWAS identified common variants in ARID5B, IKZF1, CEBPE, CDKN2A, PIP4K2A, LHPP and ELK3 influencing ALL risk. However, the risk variants of these SNPs were not validated in all populations, suggesting that some of the loci could be population specific. On the other hand, the cur

Acute lymphoblastic leukemia: a comprehensive review and 2017 update.

Acute lymphoblastic leukemia (ALL) is the second most common acute leukemia in adults, with an incidence of over 6500 cases per year in the United States alone. The hallmark of ALL is chromosomal abnormalities and genetic alterations involved in differentiation and proliferation of lymphoid precursor cells. In adults, 75% of cases develop from precursors of the B-cell lineage, with the remainder of cases consisting of malignant T-cell precursors. Traditionally, risk stratification has been based on clinical factors such age, white blood cell count and response to chemotherapy; however, the ide

Unusual Presentation of Pre-T Cell Acute Lymphoblastic Leukemia: Massive Pericardial Effusion Only.

Despite the infectious and connective tissue diseases are most encountered, etiological factors, pericardial effusions, can also rarely emerge because of malign disease in childhood and it often develops during the course of chemotherapy and radiotherapy. Unfortunately, pericardial involvement is one of the frequently encountered symptoms of pediatric malignancies. Herein, we present a rare case who was admitted with pericardial effusion and whose disease was diagnosed as pre-T cell acute lymphoblastic leukemia without peripheral blood involvement.

[Childhood eruptive nevi: a case report].

"Eruptive nevi" is a phenomenon characterized by a rapid appearance of multiple melanocytic nevi. It is mainly developed in three groups of patients: those with systemic immunosuppression, bullous cutaneous disorders, and a melanocytic stimulation drug. We report on the case of an 11-year-old boy who was diagnosed with acute lymphoblastic leukemia. A few months after the beginning of the chemotherapy, he developed multiple pigmented lesions over the skin. Eruptive nevi syndrome has been described in the literature in 29 cases in the context of severe bullous disease and in immunosuppression. N

Minimal residual disease in acute lymphoblastic leukemia: optimal methods and clinical relevance, pitfalls and recent approaches.

After advances in experimental and clinical testing, minimal residual disease (MRD) assay results are considered a determining factor in treatment of acute lymphoblastic leukemia patients. According to MRD assay results, bone marrow (BM) leukemic burden and the rate of its decline after treatment can be directly evaluated. Detailed knowledge of the leukemic burden in BM can minimize toxicity and treatment complications in patients by tailoring the therapeutic dose based on patients' conditions. In addition, reduction of MRD before allo-HSCT is an important prerequisite for reception of transpl

Philadelphia chromosome-negative acute lymphoblastic leukemia: therapies under development.

Although the prognosis of adults with Philadelphia chromosome-negative acute lymphoblastic leukemia (ALL) has steadily improved over the past decade, less than 50% of patients maintain their remission at 5 years. Several approaches have been explored in the past few years including: monoclonal antibodies - either 'naked' (rituximab) or in combination with an immunotoxin (calicheamicin or maytansin), plant toxin (ricin), or bacterial toxin (Pseudomonas or diphtheria), and a novel bispecific T-cell-engaging antibody (blinatumomab); chimeric antigen therapy using autologous T cells that target CD

Multiple relapses in extramedullary localization of acute lymphoblastic leukemia.

Extramedullary relapses of acute lymphoblastic leukemia in children and young adults are rare and in most cases are usually related to the central nervous system or testes. We describe a case of a 25-year-old man with an acute lymphoblastic leukemia with multiple extramedullary relapses in the testes, gallbladder, breast, parotid gland and eye at various times (Fig. 5, Ref. 21).

Acute lymphoblastic leukemia: monitoring minimal residual disease as a therapeutic principle.

Measurement of submicroscopic (minimal) levels of residual disease (MRD) can be used to monitor treatment response much more precisely than morphological screening of bone marrow slides. Several studies have demonstrated that MRD assessment in childhood and adult acute lymphoblastic leukemia (ALL) significantly correlates with clinical outcome. MRD detection is particularly useful for evaluation of early treatment response, but also to monitor disease before and after stem cell transplantation, for early assessment of an impending relapse and in the setting of salvage treatment. Currently, thr

Acute lymphoblastic leukaemia in children with Down syndrome: an updated review.

Down syndrome (DS) is the most common chromosomal abnormality in children and also carries a marked increased incidence of acute leukaemia. Current therapy for acute lymphoblastic leukaemia (ALL) in children with DS offers similar treatment outcome as compared to children without DS. However, in general, there is increased morbidity associated with chemotherapy in children with DS. In recent years, a better understanding of the biology of ALL has led to the elucidation of differences in DS ALL vs non-DS ALL. Further research in these differences may provide a more targeted therapy for DS ALL,

Minimal residual disease in acute lymphoblastic leukemia.

In patients with acute lymphoblastic leukemia (ALL), treatment response is increasingly evaluated with minimal residual disease (MRD) assays. ALL cells can be recognized by their clonal rearrangement of immunoglobulin and T-cell receptor genes, expression of gene fusions, and leukemia-associated immunophenotypes. Assays based on polymerase chain reaction or flow cytometry can detect one ALL cell among 10,000 to 100,000 normal cells in clinical samples. The vast majority of cases have antigen-receptor gene rearrangements and leukemia immunophenotypes for MRD monitoring; about half of the cases

Sacroiliitis as an initial presentation of acute lymphoblastic leukaemia.

A 17-year-old male was admitted to the hospital because of fever, low back pain and knee pain which had started a couple of months ago. Pelvic CT-scan confirmed left sacroiliitis. Complete blood count revealed pancytopenia.A bone marrow biopsy and aspiration was performed under local anaesthesia. The pathologist reported Acute Lymphoblastic Leukaemia. The reported patient is the first case of acute lymphoblastic leukaemia, accompanied by sacroiliitis.
