PURPOSE: The authors have reviewed the use of muscle pedicle flaps for the treatment of failed bladder neck closure in exstrophy spectrum patients.

An 82-year-old female patient presented to our clinic with refractory nailbed ulceration accompanied by pain in her left fourth toe. Six months prior, she had undergone nail removal surgery to address a persistent paronychia that had not resolved for four months. Crust, necrotic tissue, and secretion on the nailbed were taken for microbiological examination, and Candida parapsilosis was discovered in culture. Normal Pressure Hydrocephalus (NPH) occurs when there is an accumulation of cerebrospinal fluid due to impeded flow or excess production, resulting in gait and memory impairment and urinary incontinence. The authors present the case of a 67-year-old male, who had symptoms for a year prior to being diagnosed. His neurological exam was significant for a slow, and unsteady wide-based gait. No underlying cause for his NPH was found. He underwent a shunt procedure following which he made a complete recovery.

Retinal detachments constitute an emergency ocular condition when the neurosensory retina separates from the retinal pigment epithelium, leading to the death of the tissue. Prompt diagnosis and treatment are essential to avoid significant morbidity, including vision loss and/or blindness associated with this condition. This case report describes the author's challenging journey from a non-ophthalmologist perspective through the terrifying experience of bilateral rhegmatogenous retinal detachments involving seven surgical procedures prior to return to full clinical function.

In BriefIn a retrospective study the authors examined 1269 patients who underwent spinal surgery and were admitted to the intensive care unit (ICU) and identified factors that are associated with venous thromboembolic events (VTEs) in this "high risk" group. Amongst these high-risk factors were: surgeries longer than 4 hours, comorbid disease, patients needing an osteotomy, and patients undergoing spinal stabilization for fractures. Identification of factors that can be optimized prior to surgery will decrease the rates of VTE.

The authors present a 16-year-old patient with no known history of cleft palate who developed velopharyngeal insufficiency after a Le Fort I osteotomy performed for the correction of maxillary hypoplasia and a Class III malocclusion. Postoperative evaluation revealed the presence of velopharyngeal insufficiency and subtle findings of an occult submucous cleft palate. She had a pharyngeal flap 6 months later with successful correction of the velopharyngeal insufficiency. This case illustrates the need to screen for submucous cleft palate prior to orthognathic surgery.

A case of a left temporo-occipital arteriovenous malformation associated with a pleomorphic xanthoastrocytoma is described. Nine patients with nonunion of the tibia associated with angular deformity were treated using the Ilizarov device. Eight of these went on to heal the nonunion and had acceptable correction of the angular deformity. One patient with an atrophic nonunion and severe bone loss received a below knee amputation. The authors recommend the use of circular ring fixation as an alternative in the treatment of selected cases of stiff nonunion of the tibia combined with angular deformity, particularly if there is active infection, prior use of an external fixator, or poor soft tissue coverage.

There are only scattered case reports of intracranial tuberculosis in industrialized nations; brainstem tuberculoma is even more unusual, accounting for 2.5% to 8% of all intracranial tuberculoma. In developing nations, however, central nervous system tuberculosis (CNS-TB) is not rare and intracranial tuberculoma may account for 5% to 30% of all intracranial masses. The authors determined the prevalence of certain major congenital disorders among live-born infants of 6509 mothers in a prepaid health plan for the 30-month period of January 1, 1980 through June 30, 1982 who used a wide variety of drugs during the first trimester of pregnancy. The results were similar to those obtained in this population in a prior 30-month study. No strong associations between any of the commonly used drugs and the congenital disorders studied were present.
