Necrotizing fasciitis (NF) is an infection of the deep soft tissues that results in progressive destruction of the muscle fascia and overlying subcutaneous fat. These infections can be sudden, vicious, and fast-spreading. If not treated quickly with antibiotics and debridement of the infected tissue, the patient may develop septic shock, which may lead to multiple organ failure and death. NF is difficult to early diagnose due to the overlying tissue can appear unaffected initially. We report an unusual case of NF of the buttock and left thigh that clinically mimics lumbar disc herniation. This

Laugier-Hunziker syndrome (LHS, also termed idiopathic lenticular mucocutaneous hyperpigmentation) is an unusual condition characterized by progressive pigmentation of the mucous membranes. LHS displays a benign course and is not associated with malignancy. Here we present a case of LHS with a 7-year follow-up. We document metachronous oral melanoacanthomas in this individual. In addition, we found that the oral melanotic macules in this patient waxed and waned in a cyclical manner. To our knowledge, this is the first report of these findings in the context of LHS. Finally, we provide an overv

BACKGROUND: Acute severe nickel carbonyl poisoning is an uncommon yet potentially fatal occupational hazard that produces marked pulmonary injury and rapidly progressive respiratory failure.
CASE PRESENTATION: We describe three middle-aged men who developed severe nickel carbonyl intoxication after workplace exposure at a metal-processing facility. All patients presented with progressive dyspnea, cyanosis, hypoxemia, and pronounced neutrophilia. Chest computed tomography revealed bilateral pulmonary infiltrates with variable early distribution across patients. Prompt recognition permitted imm

Hashimoto encephalopathy or steroid-responsive encephalopathy associated with autoimmune thyroiditis (SREAT) is a rare condition associated with rapidly progressive dementia, stroke-like symptoms, psychosis and positive antithyroid antibodies. In this case report, a woman in her 80s with hypothyroidism and no other medically relevant history presented with a 2-week complaint of progressive hallucinations, short-term memory loss and paranoia. Eventually hospitalised, her hospital course was complicated by severe hospital-acquired delirium, which clouded the clinical picture; however, initial wo

Creutzfeldt-Jakob Disease (CJD) is an incurable and inevitably fatal neurodegenerative disorder. Although CJD has a worldwide distribution, there are no official statistics on CJD in Thailand. A diagnosis of CJD is suspected when a patient develops rapidly progressive dementia with myoclonus. However, CJD may be mistaken for a variety of illnesses because its initial presentation frequently consists of non-specific symptoms. Here, we examined cases of sporadic CJD (sCJD) from Thammasat University Hospital (a tertiary care hospital in Thailand) between January 1, 2012 and December 31, 2014. Thr

A 22-year-old man with a history of cocaine abuse from 2003 to 2005 developed recurrent bleeding of the nasal septum and a progressive cough and dyspnoea. He was admitted to the intensive care unit because of fulminant pneumonia, impaired renal function and progressive general deterioration. While hospitalized, he developed cutaneous vasculitis, thrombosis of the right subclavian and right jugular veins, testicular pain and, eventually, expanding red papules and plaques on the limbs. The symptoms were a diagnostic challenge, until skin biopsy showed immunoglobulin deposits in small vessels and

Pulmonary regurgitation following repair of tetralogy of Fallot is a common postoperative sequela associated with progressive right ventricular enlargement, dysfunction, and is an important determinant of late morbidity and mortality. Although pulmonary regurgitation may be well tolerated for many years following surgery, it can be associated with progressive exercise intolerance, heart failure, tachyarrhythmia, and late sudden death. It also often necessitates re-intervention. Identifying the appropriate timing of such intervention could be very challenging given the risk of prosthetic valve

A woman in her 30s was admitted acutely to the emergency department with severe respiratory failure of unknown cause. On arrival, the patient was severely compromised with a reduced Glasgow Coma scale and arterial pCO2 > 14 kPa. She was intubated and taken to the intensive care unit for respiratory support and further investigations.A detailed patient history revealed multiple admissions in the psychiatric sector with anxiety disorder, as well as numerous admissions to the emergency department with progressive tremors, muscle fatigue and dyspnoea, all of which were interpreted as anxiety attac

BACKGROUND AND OBJECTIVES: Data on care home admission and survival rates of patients with syndromes associated with frontotemporal lobar degeneration (FTLD) are limited. However, their estimation is essential to plan trials and assess the efficacy of intervention. Population-based registers provide unique samples for this estimate. The aim of this study was to assess care home admission rate, survival rate, and their predictors in incident patients with FTLD-associated syndromes from the European FRONTIERS register-based study.
METHODS: We conducted a prospective longitudinal multinational o

BACKGROUND: Calpainopathies, including limb-girdle muscular dystrophy recessive type 1 (LGMD R1) and the rare dominant type 4 (LGMD D4), are genetic neuromuscular disorders caused by pathogenic variants in the CAPN3 gene, which encodes calpain-3, a muscle-specific cysteine protease. This protein plays a crucial role in muscle remodelling, calcium homeostasis, and myogenesis regulation. MAIN BODY: LGMD R1, the most common form of LGMD, is characterized by progressive, symmetrical muscle weakness primarily affecting the shoulder and pelvic girdles, with an onset ranging from childhood to adultho

BACKGROUND AND OBJECTIVE: Moyamoya disease (MMD) is a progressive cerebrovascular disorder treated primarily with surgical revascularization to prevent stroke recurrence. Despite established surgical techniques, standardized perioperative care protocols remain limited, potentially leading to variable outcomes and perioperative complications. Enhanced Recovery After Surgery (ERAS) protocols have demonstrated improved outcomes across surgical disciplines but have not been systematically applied to cerebral bypass surgery for MMD. The objective was to develop evidence-based ERAS recommendations f

Bronchiectasis is a chronic, often progressive respiratory disease characterized by irreversible dilation of the bronchi. It is etiologically heterogeneous and frequently associated with a significant symptom burden, multiple complications, and reduced quality of life. In recent years, the global prevalence of bronchiectasis has increased markedly, placing a substantial economic burden on healthcare systems. These consensus-based guidelines are the first German-language guidelines focused on the management of bronchiectasis in adults. They underscore the critical role of thoracic imaging - par

Thrombocytopenia, anasarca, fever, reticulin fibrosis, and organomegaly (TAFRO) syndrome is an extremely rare and potentially life-threatening inflammatory disorder of unknown cause and sometimes necessitates intensive care due to complex pathological conditions, including respiratory failure, renal dysfunction, circulatory failure, and infection. However, reports detailing intensive care management of TAFRO syndrome remain limited. A 65-year-old woman was diagnosed with TAFRO syndrome, presenting with progressive edema, significant pleural and ascitic effusions, thrombocytopenia, fever around

UNLABELLED: Systemic sclerosis (SSc) is a chronic autoimmune disease connective tissue and one of the most common collagen diseases. There are several clinical types of scleroderma which differ in their course, possible complications and prognosis. The most characteristic form SSc is limited and diffuse systemic sclerosis. The SSc is characterized by the progressive fibrosis of the skin and internal organs, leading to their failure, morphology and blood vessels disorders.
PURPOSE: The aim of our work is to identify the main health problems of patients suffering from systemic sclerosis dependi

Advances in critical care medicine have increased survival for victims of myocardial infarction and other acute cardiac events so that increasingly patients are receiving long-term, labor-intensive, and costly medical care. Innovations in drug delivery systems and skyrocketing health care costs have fostered the growth of home health care which has blossomed into a $2.8 billion industry. There is evidence that outpatient dobutamine therapy produces definite physical and possibly psychological improvements of variable degree and duration. Hemodynamic improvements are generally associated with i

In spinal muscular atrophy (SMA), degeneration of motor neurons causes progressive muscular weakness, which is caused by homozygous deletion of the SMN1 gene. Available epidemiological data on SMA are scarce, often outdated, and limited to relatively small regions or populations. Combining data from different sources including genetic laboratories and patient registries may provide better insight of the disease epidemiology. To investigate the incidence of genetically confirmed SMA, and the number of patients who are able and approachable to participate in new clinical trials and observational

BACKGROUND: Transthyretin amyloidosis cardiomyopathy (ATTR-CM) is a progressive, underdiagnosed cause of heart failure, particularly among older adults. Despite updated Japanese Circulation Society (JCS) guidelines and advances in imaging and therapies, diagnostic delays and variability persist. Understanding real-world barriers is essential for improving patient outcomes.
METHODS AND RESULTS: A mixed-methods approach was used to develop an evidenced care pathway for ATTR-CM in Japan. A literature review of guidelines and epidemiological data informed a draft pathway. Semistructured interview

Lymphedema is a chronic and progressive condition due to an imbalance between lymphatic filtration and transport capacity. A relative overload of fluid is caused by lymphatic impairment (afterload impairment); an absolute overload is caused by increased filtration (preload raised). Lymphedema can result in considerable disability and loss of quality of life. The Dutch Society of Dermatology organized a task force to create guidelines using the International Classification of Functioning, Disability, and Health. As lymphedema is a chronic condition, a new approach was chosen according to the ch
