Internal Superior Laryngeal Nerve Injections Through the Piriform Sinus for Laryngeal Neurosensory Disturbances: A Prospective Analysis.

OBJECTIVE: Internal superior laryngeal nerve (iSLN) injections demonstrate short term improvements in patients with laryngeal neurosensory disturbances (ie, chronic refractory cough and globus pharyngeus) that can require multiple injections. A more targeted iSLN injection approach could provide greater effectiveness. This study prospectively evaluated whether transnasal endoscopic iSLN injections that target the internal iSLN as it transverses the piriform sinuses was effective at treating laryngeal neurosensory disturbances. METHODS: This was a prospective cohort analysis performed at a sin

Pharyngeal Residue Severity Rating Scales Based on Fiberoptic Endoscopic Evaluation of Swallowing: A Systematic Review.

Identification of pharyngeal residue severity located in the valleculae and pyriform sinuses has always been a primary goal during fiberoptic endoscopic evaluation of swallowing (FEES). Pharyngeal residue is a clinical sign of potential prandial aspiration making an accurate description of its severity an important but difficult challenge. A reliable, validated, and generalizable pharyngeal residue severity rating scale for FEES would be beneficial. A systematic review of the published English language literature since 1995 was conducted to determine the quality of existing pharyngeal residue …

Acute Suppurative Thyroiditis Extending to Retropharyngeal Space: Report of 2 Cases.

Acute suppurative thyroiditis (AST) is a rare endocrine emergency, typically of bacterial origin, that can lead to significant morbidity and life-threatening complications. It is far less common than other inflammatory thyroid conditions, such as subacute thyroiditis, and may mimic noninfectious inflammatory disorders. Case 1 describes a 40-year-old man with a 5-day history of anterior neck swelling and odynophagia. Physical examination and imaging confirmed AST with retropharyngeal space involvement. He was treated with broad-spectrum intravenous antibiotics and thyroid abscess aspiration. Ca

Leiomyosarcoma of the hypopharynx: a case report.

Cases of leiomyosarcoma in the head and neck are very rare with only four reports of a leiomyosarcoma originating in the hypopharynx. A rare case of leiomyosarcoma of the hypopharynx is described. A 62-year-old woman, who had experienced slight difficulty of swallowing and hoarseness, visited our hospital. A smooth-surfaced tumor was found extending from the postcricoid to an area in the left pyriform sinus, and the vocal cord on the left side was fixed. Imaging studies, including FDG-PET CT imaging showed the tumor in the postcricoid, but no distinguishable distant metastases. A total larynge…

Lipomas of the larynx and hypopharynx: a review of the literature with the addition of three new cases.

Lipomas of the larynx and hypopharynx are uncommon mesenchymal neoplasms. This report discusses the clinical and pathological features of three cases of laryngeal and hypopharyngeal lipomas. Two of the cases occurred in females and one in a male. The ages of the patients were 28, 51 and 51 years respectively. Two of the cases involved the supraglottic larynx (left arytenoid and left vestibular fold); the third involved the pyriform sinus. Symptoms included airway obstruction, dysphagia, throat discomfort, a sensation of excessive secretions in the throat and an increase in snoring. The complai…
