BACKGROUND: Q fever is a world-wide condition caused by the rickettsia Coxiella burnetii. It appears more prevalent in agrarian communities and may have serious sequelae.

The prevalence of Q fever pneumonia among children with atypical pneumonia from whom only an acute-phase serum sample was available was traced by using an indirect immunofluorescence (IF) test, nested PCR, and isolation. Twenty (34.5%) of 58 sera were found to have both polyvalent and immunoglobulin M antibodies to the phase II antigen of Coxiella burnetii by the IF test. The author has reviewed different aspects of the role of immunocompetence in the development of Q fever. Coxiella burnetii lives within the phagolysosomes of infected cells. In animals, the immunosuppression caused by either cortisone or X-irradiation reactivates Q fever. Q-fever occurs in The Netherlands in childhood more often than used to be believed. We treated an 8-month-old infant with congestive myocarditis associated with a seroconversion against Coxiella burnetii. The infection was probably caused by the father, who a few weeks earlier had visited his native Surinam, where he frequently was surrounded by domestic animals and drank raw cow's milk. After 2.5 years the child's general and cardial conditions are good although digoxin therapy is still necessary.

In Britain, Q fever in man was first reported in 1949. Two years later a serological survey of cattle revealed a prevalence of infection of 2.1%. In the 1970's prevalences of 4.5% and 3.1% were encountered respectively in cows that had aborted or calved normally in central England. In current practice, only serology, by the titration of anti-Coxiella burnetii antibodies, permits the diagnosis of acute or chronic Q fever.
