Determining appropriate timing of adaptive radiation therapy for nasopharyngeal carcinoma during intensity-modulated radiation therapy.

BACKGROUND: To determine appropriate timing of an adaptive radiation therapy (ART) replan by evaluating anatomic and dosimetric changes of target volumes and organs at risk (OARs) during intensity-modulated radiation therapy (IMRT) for nasopharyngeal carcinoma (NPC). METHODS: Nineteen NPC patients were recruited. Each patient had repeat computed tomography (CT) scans after each five fractions and at treatment completion. Automatic re-contouring the targets and OARs by using deformable registration algorithm was conducted through CT-CT fusion. Anatomic changes were assessed by comparing the in…

Solar ultraviolet radiation from cancer induction to cancer prevention: solar ultraviolet radiation and cell biology.

Although decades have elapsed, researchers still debate the benefits and hazards of solar ultraviolet radiation (UVR) exposure. On the one hand, humans derive most of their serum 25-hydroxycholecalciferol [25(OH)D3], which has potent anticancer activity, from solar UVB radiation. On the other hand, people are more aware of the risk of cancer incidence associated with harmful levels of solar UVR from daily sunlight exposure. Epidemiological data strongly implicate UV radiation exposure as a major cause of melanoma and other cancers, as UVR promotes mutations in oncogenes and tumor-suppressor ge…

Microbiota and radiation-induced bowel toxicity: lessons from inflammatory bowel disease for the radiation oncologist.

New gastrointestinal symptoms are frequent after pelvic radiotherapy and can greatly affect the quality of life of cancer survivors. The effect of radiation on the intestinal microbiota, and the clinical implications of a modified microbial balance after radiotherapy are now beginning to emerge. In this Personal View, we show the importance of the microbiota for intestinal homoeostasis, and discuss the similarity between inflammatory bowel disease, which has been extensively researched, and radiation-induced gastrointestinal toxicity. By use of microbiota profiles for risk assessment and manip…

Transformation of human mesenchymal stem cells in radiation carcinogenesis: long-term effect of ionizing radiation.

Increasing evidence on cancer stem cells suggest that stem cells are susceptive to carcinogenesis and consequently can be the origin of many cancers. We have recently established a telomerase-transduced human mesenchymal stem cell line and subsequently irradiated this in order to achieve malignant transformation. In the present study, we analyzed the long-term effect of ionizing radiation on these cells and investigate whether radiation can trigger tumor development. The cells were irradiated with a low (2.5 Gy) and a high (15 Gy) dose of gamma-rays and followed for up to 6 months after radiat…

Radiation-induced diffuse brain injury in the neonatal rat model--radiation-induced apoptosis of oligodendrocytes.

The mechanism of radiation-induced diffuse brain injury was investigated using a model of delayed myelination in the irradiated neonatal rat brain in which the number of oligodendrocytes decreases without associated necrosis of the cerebral white matter. Immunohistochemical analysis using antibody against the large myelin-associated glycoprotein, a specific marker of oligodendrocytes at an early stage of development, showed that the number of the oligodendrocytes associated with myelination decreased in the irradiated hemisphere 1 day after irradiation and remained low until 5 days after irrad…

Radiation-Induced Pneumonitis Following Stereotactic Body Radiation Therapy for Left Upper Lobe Squamous Cell Carcinoma.

This report describes the case of a 67-year-old male with a history of treated prostate cancer who developed a left upper lobe (LUL) squamous cell carcinoma, which was treated with stereotactic body radiation therapy (SBRT). After SBRT, radiation pneumonitis together with tumor regression led to bronchial obstruction and post-obstructive volume loss that culminated in a rare complete left lung collapse, underscoring an uncommon but serious complication of SBRT. The patient presented with progressive shortness of breath; imaging demonstrated post-obstructive volume loss and heterogeneous FDG av…

CyberKnife stereotactic radiosurgery, stereotactic radiation therapy and stereotactic body radiation therapy: Technical and clinical updates.

Since its clinical implementation in 1994, the CyberKnife radiosurgical system has transformed the field of stereotactic radiosurgery. Its frameless technology pioneered the role for hypofractionation, delivering radiation in a few high-dose sessions, as well as the application of stereotactic principles for extracranial treatments. Over the years, it has experienced considerable technological evolution, each enhancing imaging precision, treatment efficiency, and system usability. Hypofractionated stereotactic body radiotherapy treatments opened new treatment paradigms across a range of pathol…

Evaluation of radiation protection effectivity in a cardiac angiography room using visualized scattered radiation distribution.

In this study, we devised a radiation protection tool specifically designed for healthcare professionals and students engaged in cardiac catheterization to easily monitor and evaluate scattered radiation distribution across diverse C-arm angles and arbitrary physician associated staff positions-scrub nurse and technologist positions. In this study, scattered radiation distributions in an angiography room were calculated using the Monte Carlo simulation of particle and heavy ion transport code system (PHITS) code. Four visualizations were performed under different C-arm angles with and without …

Towards sustainable human space exploration-priorities for radiation research to quantify and mitigate radiation risks.

Human spaceflight is entering a new era of sustainable human space exploration. By 2030 humans will regularly fly to the Moon's orbit, return to the Moon's surface and preparations for crewed Mars missions will intensify. In planning these undertakings, several challenges will need to be addressed in order to ensure the safety of astronauts during their space travels. One of the important challenges to overcome, that could be a major showstopper of the space endeavor, is the exposure to the space radiation environment. There is an urgent need for quantifying, managing and limiting the detrimen…

68Ga-DOTATATE PET-Based Radiation Contouring Creates More Precise Radiation Volumes for Patients With Meningioma.

PURPOSE: Radiation treatment planning for meningiomas traditionally involves magnetic resonance imaging (MRI) contrast enhanced images to define residual tumor. However, the gross tumor volume may be difficult to delineate for patients with a meningioma in the skull base sagittal sinus or after resection. Advanced positron emission tomography (PET) imaging using 68Ga-DOTATATE, which has been shown to be more sensitive and specific than MRI imaging, can be used for target volume delineation in these circumstances. We hypothesized that 68Ga-DOTATATE PET scan-based treatment planning would lead t…

[Radiation Protection Against Exposure from Patients Receiving 131I-MIBG and Released from Radiation Treatment Room].

131I-3-iodobenzylguanidine or 131I-iobenguane (3-(131I) iodobenzylguanidine or 131I-iobenguane [131I-MIBG]) is a radioactive agent that is specifically accumulated in tumor cells such as pheochromocytoma and neuroblastoma. Due to its cytotoxic beta ray emitted from 131I, it has been developed as an agent for radioisotope therapy and some researchers have reported its effectiveness. In this study, based on the patients' data from previous clinical trials of 131I-MIBG therapy, we evaluated the radiation safety for public exposure caused by radiation emitted from patients who received 131I-MIBG. …

[Clinical examination of cases of radiation proctitis after intensity-modulated radiation therapy for prostate cancer].

In our series of 366 patients who received intensity-modulated radiation therapy (IMRT) for prostate cancer, radiation proctitis developed in 24 patients. We examined the endoscopic evidence and clinical characteristics of radiation proctitis in these patients. The onset time was 2-29 months after treatment, with bloody bowel discharge being the most common symptom (22 cases). Colonoscopy revealed that the lesions tended to be concentrated on the rectal right anterior wall. Severity, according to the Tada classification, was 0b or Ia in 70.8% of all cases. The incidence of radiation proctitis …

Estimate of organ radiation absorbed doses in clinical CT using the radiation treatment planning system.

Organ absorbed doses in computed tomography (CT) scans can be measured with anatomical phantoms but not inside the human body. In this study, a straightforward method was investigated to estimate organ doses in clinical CT using the radiation treatment planning system (RTPS) and compared them with experimental results of photoluminescence dosemeters (PLD). In a heterogeneous phantom, the average difference between PLD and RTPS values were -5.0% for the body and 7.1% for the lung. Using CT data, organ doses in 30 clinical cases were then calculated. There was a significant inverse correlation b…

A multicenter randomized trial of breast intensity-modulated radiation therapy to reduce acute radiation dermatitis.

PURPOSE: Dermatitis is a frequent adverse effect of adjuvant breast radiotherapy. It is more likely in full-breasted women and when the radiation is distributed nonhomogeneously in the breast. Breast intensity-modulated radiation therapy (IMRT) is a technique that ensures a more homogeneous dose distribution. PATIENTS AND METHODS: A multicenter, double-blind, randomized clinical trial was performed to test if breast IMRT would reduce the rate of acute skin reaction (notably moist desquamation), decrease pain, and improve quality of life compared with standard radiotherapy using wedges. Patien…
