[Arthroscopy-guided treatment of fractures of the distal radius: 16 wrists].

PURPOSE OF THE STUDY: We report our experience with arthroscopy-guided treatment of fractures of the distal radius. MATERIAL AND METHODS: Between November 2001 and June 2002, 16 patients (10 men, 6 women, mean age 51 years, age range 24-75 years) underwent arthroscopy-guided treatment of articular fractures of the distal radius. Patients were installed with the arm held in a horizontal position with a brace, the wrist under traction. The same procedure was used in all cases: introduction of the arthroscope, joint cleaning and shaving, search for lesions, arthroscopy-guided treatment. Kirschne

Intra-articular distal radius fractures: arthroscopic assessment of radiographically assisted reduction.

The optimal means of assessing articular displacement during closed reduction of distal radius fractures is unknown. The purpose of this study was to evaluate the in vivo accuracy of fluoroscopy (C-arm) and plain radiographs (XR) in measuring articular step-off and gap and to determine if postreduction arthroscopy can identify malreduced intra-articular fractures that would benefit from reduction. Fifteen intra-articular distal radius fractures underwent closed manipulation and percutaneous pinning. Reduction was assessed sequentially by C-arm, XR, and wrist arthroscopy. The width of gapping b…

Principles of external fixation and supplementary techniques in distal radius fractures.

External fixation for fractures of the distal radius has been used for almost 80 years. The main objective is to gain reduction and maintain … e grafting, fixation wires and stabilization of the radioulnar joint. Associated injuries in distal radius fractures need to be identified and treated. The possible complications of external fixation and the means to prevent them are discussed. External fixation of the distal radius has found its place as an established method in treating certain types of this com …

[External fixation of distal radius fractures: indication, technique and functional results].

A series of 56 consecutive patients with 59 fractures of the distal end of the radius, treated with an unilateral external fixator of Hoffmann between 1980 and 1988, is presented, 47 of these patients were examined clinically at home in the last months of 1989. We can distinguish two different groups of patients: the older females with an average of 59.2 years and the younger males with an average of 40.1 years. The fractures of the females were caused by a lower energy trauma than those of the males (p less than 0.05). The males had significantly more complex fractures (Frykman type VII or VI

[A new classification of distal radius fractures and corresponding treatment methods].

Based on previously published reports on fractures of the distal radius and on the analysis of 400 fresh fractures treated in two accident hospitals affiliated with our university hospital, we propose a new classification as shown in Table 2. This new classification includes a type of combined Chauffeur-Barton fracture which has never been described before. Each type of fracture of the distal radius requires a specific treatment as shown in Fig. 1.

Treatment of unstable Colles' fractures with a new radius mini-fixator.

An external fixator has been developed that is confined to the radius. It is lightweight, has a low profile, and its application is uncomplicated. Because the fixator provides rigid fixation of the fracture fragments, it has the important advantage of allowing immediate active wrist exercises, thus facilitating early restoration of mobility. The radius mini-fixator has been effective in the treatment of unstable Frykman Type I and II Colles' fractures that account for 36.3% in Frykman's series. It has also been used to stabilize osteotomies of the radius for malunited Colles' fractures.

Extra-articular low-velocity gunshot fractures of the radius and ulna.

Low-velocity gunshot fractures of the forearm are complex injuries and previously published studies have not emphasized the problems particular to these forearm lesions. Of the twenty-nine patients in this series, thirteen had peripheral nerve injuries, three had impending Voklmann's ischemia, and ten had delayed union or malunion of fractures after treatment by closed methods. Only thirteen had none of these problems. Eight patients had long-term disability resulting either from permanent nerve damage with loss of sensation or weakness of grip, or from significant loss of motion following del

Quantitative Analysis of the Volar Ulnar Corner of the Distal Radius: A Reference for Intraoperative Distal Radius Fracture Reduction.

PURPOSE: The reduction of distal radius fractures using volar locking plate fixation can be performed by securing the plate to the distal fragments and then levering the plate to the radial shaft. Oblique placement of the plate on the radial shaft may lead to malreduction. The aim of this study was to evaluate parameters that can be used for the assessment of intraoperative distal radius fracture reduction using "distal-first" volar plate fixation, especially the geometry of the ulnar corner. The prevalence of Tolat distal radioulnar joint (DRUJ) types was determined, and the angles of the vol…

The effect of distal radius translation in the coronal plane on forearm rotation: a cadaveric study of distal radius fractures.

PURPOSE: To determine the effect of lateral translation of the distal radius in the coronal plane on forearm rotation after distal radius fracture. METHODS: Ten fresh cadaveric limbs underwent distal radius osteotomy just proximal to the distal radial-ulnar joint to simulate an extra-articular distal radius fracture. We used an Agee Wrist Jack external fixator to create increasing magnitudes of distal fragment lateral translation in 2-mm increments. Forearm rotation was measured using a 3-dimensional camera at each magnitude of lateral translation. RESULTS: Total forearm rotation for the int

Differences in Function and Fracture Risk in Postmenopausal Women With and Without a Recent Distal Radius Fracture.

A distal radius fracture (DRF) is commonly the first fracture to occur in early postmenopausal women. The reasons for sustaining a DRF may be related to fall risk, bone fragility, or both. The objective of this study was to compare functional and fracture risk status in postmenopausal women with and without a recent DRF and explore the relationships between function, grip strength, and fracture risk status. Seventy-seven women a ges 50-78 with (n = 32) and without (n = 45) a history of DRF in the past 2 years participated. Balance, timed up and go (TUG), gait velocity, balance confidence, sit

Inadequate management for secondary fracture prevention in patients with distal radius fracture by trauma surgeons.

UNLABELLED: We evaluated the secondary fracture prevention in 1445 patients with distal radius fracture by trauma surgeons. The rate of patients with distal radius fracture who underwent bone mineral density (BMD) examination was low, suggesting that appropriate treatment for osteoporosis by trauma surgeons is not performed at present. INTRODUCTION: To clarify the status of osteoporosis interventions after distal radial fractures by trauma surgeons who play the main role in treatment for these fractures, we performed a survey involving multiple institutions in Japan. METHODS: We asked 155 bo

Fracture of the ulnar styloid process negatively influences the outcome of paediatric fractures of the distal radius.

In paediatric patients with fractures of the distal radius, the consequences of associated ulnar styloid fractures are often underestimated. These may include persisting pain or functional deficits. The aim of the present study was to report the outcome of these fractures using a modified DASH-Score. All children with distal radius fractures treated in a two years period were analysed; only patients with a concomitant fracture of the ulnar styloid were included in the study. In addition, children with a non-union of the styloid at cast removal were asked to complete a postal questionnaire; the …
