A technique to demonstrate external haemorrhoids.

Although uncommon in children, haemorrhoids are one of the causes of a protruding anal lesion and may be confused with rectal prolapse or prolapse of a rectal polyp. The lesions may not be obvious when the child is anaesthetised because of lack of straining. This may prevent accurate diagnosis and impede identification of the lesion if surgery is being attempted. The authors report 3 cases where a 20 F Foley catheter with 30 ml balloon was inserted rectally and gentle traction applied to reproduce the raised venous pressure generated during straining. On each occasion external haemorrhoids cou

A case of large mucinous adenocarcinoma arising in a long-standing fistula-in-ano.

BACKGROUND: Chronic anal fistulas are not rare conditions, however, the development of a carcinoma in a long-standing fistula-in-ano is rare. METHODS: The case of a 77-year-old male with a large perianal mucinous adenocarcinoma arising in a long-standing fistula-in-ano is presented. RESULTS: Perianal biopsy revealed mucinous adenocarcinoma. Abdominal CT, double contrast barium examination and flexible sigmoidoscopy revealed no other tumoral lesion in the colon and rectum. CONCLUSION: The patient underwent abdominoperineal resection including wide tumor excision on the gluteal region. The fi

Imaging of the anorectal region.

Imaging of anorectal region has drastically changed during the last decade. Transrectal ultrasound and transrectal MRI can be used for staging the rectal tumours. Endoanal sonography can be applied for the classification of perianal fistulae and identification of anal sphincter defects in patients with faecal incontinence. Due to the limitations of endoanal sonography, endoanal MRI was introduced to assess the pathology related to the anal sphincter complex. Endoanal MRI seems superior to endoanal sonography. This paper describes the new developments of the imaging techniques and presents new

Invasive inflammatory pseudotumor of the pelvis: a case report with review of the literature.

Inflammatory pseudotumor (IPT) is a rare benign lesion of unknown etiology, which mimics malignant neoplasm and may arise from various organs. A 53-year-old woman was submitted to diagnostic evaluation because of bilateral, hydroureteronephrosis and oedema of the left leg after a 3-month history of fever of unknown origin. On bimanual vaginal and rectal examination, a mass was involving the uterus, parametria and mostly left adnexa, while the cervix appeared normal. Computed tomographic (CT) scan revealed a 13x10.5 cm mass in the pelvis, mostly at the place of the left adnexa, uterus and both

CT in the diagnosis of enterovesical fistulae.

Enterovesical fistulae are difficult to demonstrate by conventional radiographic methods. Computed tomography (CT), a sensitive, noninvasive method of documenting the presence of such fistulae, is unique in its ability to outline the extravesical component of the primary disease process. Twenty enterovesical fistulae identified by CT were caused by diverticulitis (nine), carcinoma of the rectosigmoid (two), Crohn disease (three), gynecologic tumors (two), bladder cancer (one), cecal carcinoma (one), prostatic neoplasia (one), and appendiceal abscess (one). The CT findings included intravesical

Overcoming the Challenge: A Comprehensive Review of Neoadjuvant Treatment Resistance in Rectal Cancer.

Colorectal cancer (CRC) is the third most commonly diagnosed cancer and remains a leading cause of cancer-related mortality, particularly among younger men. Approximately one-third of colorectal cancers occur in the rectum. For patients with locally advanced rectal cancer, neoadjuvant therapy is considered the standard treatment approach. Despite advances in therapeutic approaches, improvements in the 5-year survival rate have been modest. Accurate assessment of tumor response to neoadjuvant therapy (NAT) is critical for guiding subsequent treatment strategies, especially when considering elig

[A Case of Rectal Metastasis from Prostate Cancer].

A 78-year-old male was under follow-up after gallbladder cancer surgery when CT imaging revealed thickening of the rectal wall, along with enlarged mesenteric and left-sided lymph nodes. Colonoscopy identified a semi-circumferential, tumor-like lesion in the lower rectum, and biopsy results confirmed a diagnosis of poorly differentiated adenocarcinoma. Pelvic MRI further revealed a separate tumor in the prostate, leading to a diagnosis of concurrent rectal and prostate cancer. The prostate cancer was managed with endocrine therapy following rectal resection. For the rectal cancer, neoadjuvant

Primary staging of rectal cancer on MRI: an updated pictorial review with focus on common pitfalls and current controversies.

Magnetic resonance imaging (MRI) plays a pivotal role in primary staging of rectal cancer, enabling the determination of appropriate management strategies and prediction of patient outcomes. However, inconsistencies and pitfalls exist in various aspects, including rectal anatomy, MRI protocols and strategies for artifact resolution, as well as in T- and N-staging, all of which limit the diagnostic value of MRI. This narrative and pictorial review offers a comprehensive overview of factors influencing primary staging of rectal cancer and the role of MRI in assessing them. It highlights the sign

Not all Rectal Cancer Patients Could Benefit From the Surgery on the Primary Site.

AIM: Previous studies have provided evidence that primary site surgery can improve the prognosis of rectal cancer patients, even in those with advanced age and distant metastasis, though results have been inconsistent. The current study aims to determine if all rectal cancer patients are likely to benefit from surgery in terms of overall survival. METHODS: This study examined the impact of primary site surgery on the prognosis of rectal cancer patients diagnosed between 2010 and 2019 using multivariable Cox regression analysis. The study also stratified patients by age group, M stage, chemoth

Anorectal Melanoma-Brownish Black Mass Not Always a Hemorrhoid.

The non-specific clinical symptoms of anorectal brownish-black mass do not help to differentiate colorectal cancer, hemorrhoids, rectal ulcers which result in a delayed diagnosis or lead to inadequate management of lethal anorectal melanoma. Primary malignant melanoma of the anorectal region is an uncommon tumor, constituting approximately 1% of anal canal tumors which may be misdiagnosed clinically as hemorrhoids. Because of aggressive behavior and poor prognosis, efficient and prompt diagnosis is required in these cases. We report 2 cases of this rare tumor.

The rectosigmoid problem.

The lack of consensus over the transition point for the end of the sigmoid and beginning of the rectum is a problem for the colorectal multidisciplinary team. In this review, we survey the wide number of landmarks for the rectosigmoid junction, and describe the theoretical and evidence-based strengths and weaknesses of each one. Without a reliable definition of the rectum, sigmoid and rectal cancers will be classified inconsistently. As the treatment strategies for sigmoid and rectal cancers are radically different, incorrect tumour localisation has a substantial impact on patient management,

[A case of metastatic ureteral tumor from rectal cancer].

A 67-year-old man was referred for further examination of left hydronephrosis. He had undergone anterior resection for rectal cancer 2 years previously and also right lobectomy for a solitary hepatic metastasis one year postoperatively. Antegrade pyelography demonstrated a filling defect in middle portion of the left ureter. Cytology of the aspirated urine was class V. Left nephrourete-rectomy was performed. Histologically metastatic adenocarcinoma with intact ureteral mucosa was demonstrated.

Endoscopic ultrasonography in the evaluation of colon and rectal disease.

Endosonography of the colon has until recently been limited to examination of the rectum. Rigid and flexible instruments are currently available which allow for detailed images of the rectal wall. Comparative studies suggest that endosonography may be more accurate than computed tomography in the preoperative staging of rectal carcinoma. Recently, an ultrasound colonoscope has been developed which allows for visual examination, biopsy, and endosonography throughout the colon. Experience with this technique is limited and its role in the evaluation of colonic disease has not yet been determined
