Limb dependency activates the venoarteriolar reflex (VAR), resulting in reflex vasoconstriction (RV). Since others have reported the VAR to fail in diabetic neuropathy, we examined the reliability of this test in detecting sympathetic failure in diabetic and other neuropathies. We studied 40 controls, 49 diabetic subjects, and 29 patients with other neuropathies. Six male patients with narcolepsy for several years, were studied without and with amphetamine in order to evaluate possible abnormalities in autonomic control of the cardiovascular system. Studies were made of (1) heart rate and blood flow in the resting forearm during contralateral isometric handgrip, (2) respiratory sinus arrhythmia and (3) heart rate response to the Valsalva manoeuvre. To determine the effects of the intra-aural muscle reflex on the spondee threshold, 12 normal-hearing adults were tested monaurally with two contralaterally presented stimuli, a 2000 Hz pure tone and a broad-band noise, at two presentation levels: -10 dB and +10 dB re each subject's acoustic reflex threshold for each stimulus. The acoustic reflex thresholds for broadband noise and 500-, 1000-, and 2000-Hz activating signals were measured in a group of young normal hearing adults and a group of elderly normal hearing subjects. Blink reflexes were studied in 21 patients in coma after severe head injury. Our observations suggest that blink reflexes are a simple, objective, neurophysiological test to evaluate brainstem function. Correlations between these reflexes and the anatomoclinical stanges of coma and the Glasgow coma scale have been established. Presence of the early R1 component shows the integrity of at least a part of the pontine structures. The appearance of the late R2 component is correlated with a better chance of recovery from coma.
