Loneliness and living arrangements.

Adults with ID/DD live in increasingly small community settings, where the risk of loneliness may be greater. We examined self-reported loneliness among 1,002 individuals with ID/DD from 5 states in relation to community residence size, personal characteristics, social contact, and social climate. One third reported being lonely sometimes and one sixth said they were often lonely, but loneliness was not more common for people living alone or in very small settings. More loneliness was reported by residents of larger community living settings of 7 to 15 people. More social contact and liking wh

Frequency of orthostatic hypotension in a community based cohort of patients with Parkinson's disease.

The frequency of orthostatic hypotension (OH) in cohorts of patients recruited through hospital Parkinson's disease (PD) clinics ranges from 16% to 58%. However, hospital based cohorts may be subject to ascertainment bias. The aim of this study was to determine the frequency of OH in a community based population of PD patients and to determine the demographic features of patients with and without OH. Forty two (47%) of patients met the criteria for OH. Subjects with OH were older than those without OH, but there was no difference in PD disease duration or severity, MMSE or depression rating be

Cross-cultural issues in forensic psychiatry training.

OBJECTIVE: Forensic psychiatry was officially recognized as a subspecialty by the American Board of Medical Specialties in the 1990's. In 1994, the American Board of Psychiatry and Neurology (ABPN) gave its first written examination to certify forensic psychiatrists. In 1996, the Accreditation Council for Graduate Medical Education (ACGME) began to officially accredit one-year residency experiences in forensic psychiatry, which follow a 4-year residency in general psychiatry. The extra year of training, colloquially known as a fellowship, is required for candidates who wish to receive certific

Cutaneous malignant melanoma in Norway: variation by region of residence before and after the age 17.

OBJECTIVE: The aim of this study was to examine indirectly the importance of sun exposure during childhood and adolescence for the development of cutaneous malignant melanoma (CMM) by using a migration approach. METHODS: A nested case-control study was carried out based on all inhabitants living in Norway at any time between 1960 and 1980. The cases were all diagnosed between 1960 and 1992 (n = 13,934) and 10 controls were added for each case (n = 130,507). Analyses were conducted on residence history in the southeast, mid-west and northern part of Norway, before and after age 17, also includ

Prevalence of skin disease in a cohort of shelter-based homeless men.

BACKGROUND: Cutaneous disease is a frequent cause of morbidity in homeless people. A variety of infectious and noninfectious conditions have been described in this population that are exacerbated by malnutrition, exposure to hazardous environment, psychiatric disease, physical injuries, and limited access to health care. OBJECTIVE: Our purpose was to study the prevalence of skin disease in a selected group of the homeless population. METHODS: We evaluated skin diseases in a cohort of 142 homeless men who were staying overnight in a major Boston homeless shelter for men over a period of 3 mon

Precautionary measures for the wise traveler. Some common hazards.

Several risks can be posed by travel, even when the stay is not protracted nor the destination exotic. Air travelers should consider the potential problems associated with decreased cabin pressure, hypoxia, and circadian dysrhythmia, problems that can contraindicate flying for some. Motion sickness can affect those traveling by air, sea, or land; medications are available to combat the symptoms, and several other remedies such as changing visual stimuli are also worth trying. Persons staying in hotels, particularly high rises, should review several general guidelines for defensive action in ca

Nutrition of elderly men living alone. I. Intakes of energy and nutrients.

The energy and nutrient intakes of 35 men aged 70 years and over, living alone at home, were studied using five-day records. The mean energy intake was 1628 (SD 545) kcal/day. Protein and calcium intakes were generally adequate, but 60% of the old men took less than 10mg of iron per day. Intakes of vitamin C were below 15 mg/day in 34%. Thirty-four percent took less than 0.8 mg thiamine daily. Intakes of vitamin A and riboflavin were, in general, adequate, except for a small number of subjects. It is concluded that about one-third of the men had deficient vitamin C or thiamine intakes.

Association of Park Size, Access and Neighbourhood Walkability with Physical Activity and Obesity: A Cross-Sectional Analysis.

BACKGROUND: We examined whether higher access to parks and greenspace is independently associated with an increase in physical activity and lower rates of obesity when neighbourhood walkability is accounted for and whether neighbourhood walkability and park access have synergistic effects on these outcomes. MATERIALS AND METHODS: We used cross-sectional data from the Canadian Community Health Survey between 2007 and 2014 for adults aged 20 to 74 in Ontario, Canada. Neighbourhood-level park access exposures included size of parks and number of parks within 800 m of residential areas, and neigh

Psychological heterogeneity and residential demands in early-stage aging: evidence from a human-environment systems framework.

BACKGROUND: Research on home-based eldercare has increasingly moved beyond physical safety and accessibility to consider psychological wellbeing and everyday activity. However, older adults are still often treated as a homogeneous group. Less is known about how different psychological profiles are associated with residential preferences, activity patterns, and perceived environmental support among early-stage aging adults. METHODS: This study focused on early-stage older adults and applied a human-environment systems framework to examine the relationships among psychological characteristics,

Multi-level disparities in survival among patients diagnosed with oral cavity cancer in Texas between 1995 and 2020.

BACKGROUND: Multi-level factors, including demographic and neighborhood social conditions, related to disparities in oral cavity cancer (OCC) are not fully understood. We examined the association between these factors and survival outcomes among patients diagnosed with OCC. METHODS: We identified 14,416 patients diagnosed with OCC from 1995 to 2020 using population-based data from the Texas Cancer Registry. Neighborhood-level social vulnerability was measured using the 2010 US Centers for Disease Control and Prevention's Neighborhood Social Vulnerability Index (CDC-SVI). We examined race and

Hurricanes, Neighborhood Disadvantage, and Cardiopulmonary Health in US Veterans.

IMPORTANCE: Global climate change is increasing the frequency and severity of hurricanes, which are associated with adverse health outcomes. Veterans may be more susceptible to the health impacts of hurricanes because of prior military exposures. OBJECTIVE: To determine whether hurricane exposure is associated with cardiovascular and respiratory events among US veterans. DESIGN, SETTING, AND PARTICIPANTS: This retrospective cohort study included US veterans enrolled in Veteran Health Administration (VHA) primary care who lived in areas affected by Hurricane Sandy (2011-2013) or Hurricane Har

Assessing Upstream Structural and Social Drivers of Health in Breast Cancer Survivorship: The Pathways Study Neighborhood Resource.

BACKGROUND: Research has linked neighborhood factors (e.g., air pollution, obesogenic environment) often associated with residential segregation to cancer disparities, including mortality. Limited research has explored these multilevel drivers of health, recognizing residential segregation as upstream of neighborhood social, built, and physical environment attributes. In this study, we describe the neighborhood data resource of the Pathways Study, a diverse cohort of women diagnosed with breast cancer. METHODS: Using data from multiple sources, including the 2010 US Census, we appended census

The Association Between Neighbourhood Leisure Amenities and Chinese Older Adults' Depressive Symptoms: The Mediating Role of Social Capital.

With the growing prevalence of mental health problems among older adults, this paper investigates the association between neighbourhood leisure amenities and depressive symptoms among older adults in China, and further explores the mediating role of social capital in this relationship. The results suggest that neighbourhood leisure amenities significantly reduce older adults' depressive symptoms (α = -0.124, 95% CI [-0.206, -0.042]), with stronger associations observed among those who are low-income, aged 75 years and above, female, and living in rural areas. Mediation analyses show that socia

The Social Ecology of Psychotherapy: Community-Level Influences on Adolescent Mental Health Treatment.

OBJECTIVE: Effective treatment for depression and anxiety in adolescence is a public health concern. Although psychotherapies for depression and anxiety exist, youth psychotherapies are less effective when delivered in community settings compared to research settings. A social-ecological model, which considers individual-, interpersonal-, and community-level influences on wellbeing, posits that understanding multi-level factors can help us understand this drop in treatment effectiveness. This study employs a socioecological model to examine the influence of neighborhood factors on depression a

Hospital Formula Supplementation Postbreastfeeding Initiation, Neighborhood Economy, and Race.

IMPORTANCE: Breastfeeding supports lifelong health, but socioeconomic and racial disparities persist. Biases in hospital formula supplementation practices may be an underlying contributor. OBJECTIVE: To examine whether nonmedically indicated hospital formula supplementation of term-born breastfed newborns is associated with neighborhood socioeconomic status and/or maternal race. DESIGN, SETTING, AND PARTICIPANTS: Provincial registry data were used to build a cohort of all live births of term-born singleton infants who initiated breastfeeding in Ontario, Canada, hospitals from April 1, 2015 t

The relationship of neighborhood socioeconomic disadvantage and pretreatment cancer-related cognitive impairment in women with breast cancer: A post hoc analysis of a randomized controlled trial.

BACKGROUND: Up to 30% of women with breast cancer experience cancer-related cognitive impairment (CRCI) even before they receive treatment. The authors conducted a post hoc analysis to examine the relationship between neighborhood socioeconomic disadvantage and CRCI in treatment-naive postmenopausal women with early stage breast cancer. METHODS: The Exercise Program in Cancer and Cognition was a single-blind, 6-month, randomized controlled trial evaluating moderate-intensity aerobic exercise versus usual activity on neurocognitive function. By using baseline data from treatment-naive particip

Neighborhood social fragmentation in relation to impaired mismatch negativity among youth at clinical high risk for psychosis and healthy comparisons.

Impairments in mismatch negativity (MMN) are well-established in schizophrenia and have been observed in youth at clinical high-risk for psychosis (CHR-P). Prior animal studies have shown that social isolation may be related to neurobiological changes, including reduced MMN-like responses and schizophrenia-like behaviors. In parallel, neighborhood social fragmentation has been shown to be associated with the onset of psychosis. This study investigates the association between neighborhood social fragmentation and MMN impairment among CHR-P youth and healthy comparisons (HC). Data were collected

Neighborhood Social Disorganization in Early Adolescence and Substance Use Trajectories into Young Adulthood: The Moderating Role of Effortful Control and Parental Substance Use.

Adolescents living in socially disorganized neighborhoods may consume substances more frequently. Although some evidence suggests that such potential neighborhood influences persist into young adulthood, findings remain inconsistent. More research is needed that includes different indicators of neighborhood social disorganization, substance use outcomes, and family- and individual-level moderating factors. To address these gaps, this study examined the associations between three indicators of neighborhood social disorganization—social fragmentation, socioeconomic deprivation, and disorder—in e
