The use of the LMA-ProSeal in airway resuscitation.

Insufflation of the stomach with air can be a complication of face mask ventilation in the case of airway obstruction. Although the laryngeal mask airway has proven value in airway resuscitation, it has two major failings: a relatively low seal pressure and lack of access to the alimentary tract. A case is reported in which failed intubation (by multiple techniques) and intermittent face mask ventilation resulted in gastric distension, decreased airway compliance, and compromised gas exchange. The patient experience oxyhemoglobin saturation that did not improve despite laryngeal mask ventilati…

Successful resuscitation of a patient with electrical storm.

A 41 year old woman with type 2 diabetes, hypertension, and hyperlipidaemia but no known heart disease received 130 DC shocks for repeated cardiac arrests due to ventricular tachyarrhythmias over 48 hours. She was stabilised by intravenous amiodarone and had a defibrillator implanted. Serial ECGs did not change, but raised troponin I confirmed myocardial infarction as the underlying cause. Electrical storm is an uncommon and dramatic but usually treatable syndrome of recurrent ventricular arrhythmias. Frequent precipitants of electrical storm include recent worsening heart failure, hypokalaemi

Resuscitation, anaesthesia and analgesia of the burned patient.

Burns resuscitation has evolved over the past few decades towards more evidence-based management. It has been shown that patients with major burns (i.e. involving more than 30% of the body surface) benefit from invasive monitoring, and physiological variable targeted resuscitation using vasoactive agents for cardiovascular support. The invasive approach results in a reduction of mortality rates. Since the introduction of the Parkland formula in 1968, there has been a trend towards the administration of fluid resuscitation far in excess of the volume predicted with this formula. This has led to…

Physiologic reflexes and their impact on resuscitation of the newborn.

Most depressed newborn infants respond promptly to minimal intervention. When that does not happen, a thorough understanding of the physiologic reflexes that promote and prevent, prompt restoration of heart rate, blood pressure, and respiration is crucial. This article will review data demonstrating that lung inflation, in the purely mechanical sense, is a key element in a successful resuscitation. A complicated resuscitation is most often because of the caretaker's lack of awareness of basic cardiopulmonary physiology than to an unusually depressed infant.

[Resuscitation of the burned child in critical condition].

Recent advances in surgical techniques, control of infection and nutritional support have dramatically increased the survival rates of burned children. The characteristics of severely burned pediatric patients dictate that management be different from that required for adults in the intensive care unit. The formulas for fluid replacement should be based on body surface rather than weight in children and adjusted for degree of stress and age, with appropriate monitoring and treatment of hypothermia, pain and associated psychological disorders. Early assessment and treatment of airway obstructio

The delivery room resuscitation of the hydropic neonate.

Hydrops fetalis is a relatively rare disorder that will be encountered occasionally in all obstetric centers. The prognosis for infants with hydrops fetalis is poor, with mortality reported in the range of 50% to 98%. The effectiveness of delivery room resuscitation and the ability to achieve early adequate gas exchange in affected infants may be related to survival. Successful resuscitation requires an understanding of transitional neonatal physiology and the potential impact of hydrops fetalis, an experienced and well-prepared resuscitation team, meticulous attention to technical aspects of …

Death and resuscitation in the pediatric emergency department.

STUDY OBJECTIVE: To describe a population of patients who arrived in a pediatric emergency department with pulse and respirations but then sustained cardiopulmonary or respiratory arrest while in the ED. DESIGN: Retrospective case series of patients from July 1987 to June 1993. SETTING: Urban, tertiary care pediatric ED. PARTICIPANTS: All patients who sustained cardiopulmonary or respiratory arrest while in the ED. RESULTS: Thirty-two cases of cardiopulmonary (n = 18) or respiratory arrest (n = 14) were identified, for an incidence of 1.2 arrests per 10,000 patient visits. Causes of arrest

Resuscitation and senility: a study of patients' opinions.

In the context of 'Do-not-resuscitate' (DNR) decisions, there is a lack of information in the UK on the opinions of patients and prospective patients. Written anonymous responses to questionnaires issued to 322 out-patient subjects showed that 97 per cent would opt for cardiopulmonary resuscitation (CPR) in their current state of health. In the hypothetical circumstance of having advanced senile dementia only 10 per cent would definitely want CPR, with 75 per cent preferring not to have CPR. There were no significant correlations between the responses and sex or age. Of 270 patients asked verb…

Fluid therapy and the resuscitation of traumatic shock.

Fluid management of the traumatized patient begins with assessment of volume status via palpation of pulses; evaluation of mental status; and measurement of urine output, arterial blood pressure, and central pressures. Intravascular line placement and choice of initial resuscitation fluids should be individualized to the clinical situation, although in most situations a crystalloid solution continues to be the initial fluid of choice. Following initial stabilization, the intravenous fluid administered can be tailored to a given situation, chosen only after the deranged fluid balance is sequent…
