Melnick-Needles syndrome is a congenital syndrome associated with severe architectural disorder of the skeletal system that can cause significant effects on the craniofacial skeleton including the mandible and temporomandibular joint. We report on a case of a young adult patient who experienced chronic dislocation, pain, and dysfunction of her temporomandibular joints related to both a severe bite dysfunction (mandibular hypoplasia) and the underlying bony architectural disorder associated with Melnick-Needles syndrome. The patient underwent bilateral mandibular distraction to correct her skel

A woman with Class II Division 1 long-face syndrome characteristics had a history of facial trauma, dentoalveolar fracture of her mandibular anterior teeth, and temporomandibular joint pain. The pretreatment apical radiograph showed a large area of external root resorption of the mesial surface of the mandibular left central incisor. To arrest the external root resorption, the mandibular left central incisor was extruded. During extrusion, sequential apical radiographs were taken. As the tooth moved away from the site of osteoclastic activity, resorption ceased, and repair took place on the ro

In 132 patients with polysomnographic records suffering from OSAS a cephalometric analysis according to Hasund supplemented by special OSAS-parameters was performed. The statistical analysis showed a correlation between severity of OSAS and width of posterior airway space, length of the soft palate, hyoid position and posterior growth development of the mandible. A principal component analysis showed four groups: 1) prognathic patients, 2) retrognathic patients with narrow PAS, 3) patients with long soft palate and low standing hyoid, and 4) orthognatic obese subjects. Lateral cephalometry is

This study examines the short-term stability of the mandible following mandibular advancement surgery in which skeletal suspension wires were used in addition to dental maxillomandibular fixation. Twenty adults underwent sagittal ramus osteotomies. No concomitant surgical procedures were performed. Maxillomandibular fixation consisted of wiring between the upper and lower orthodontic brackets and circummandibular wires connected to the piriform aperture or anterior nasal spine wires for eight weeks. Cephalograms were analyzed during this period to evaluate skeletal stability. A statistically i

The authors describe a case of Marshall-Smith's syndrome in which there was a facial dysmorphic syndrome and considerable acceleration of bone maturation (24 months to 15 days). The infant had chronic respiratory distress and the aetiology of this condition is discussed, one possibility being a pneumopathy from repeated deglutition provoked by the micro-retrognathism. Hormonal investigations were normal. Contrary to the data in the first published reports, there was a pathological increase in size, as noted in subsequent observations by Weaver. Differential diagnosis is discussed in relation t

Skeletal malocclusions are common phenotypes in humans and have a strong influence on genetic factors. Transforming growth factor beta (TGFβ) controls numerous functions of the human body, including cell proliferation, differentiation, and migration. Thus, this study is aimed at evaluating whether genetic polymorphisms in TGFB1 and its receptor TGFBR2 are associated with mandibular retrognathism in German children and adolescents. Children and teenagers older than 8 years in the mixed or permanent dentition were included in this study. Patients with syndromes and facial trauma and patients wit

The development of craniofacial distraction techniques represents a significant advancement in the management of craniofacial dysplasias. For distraction of the midface after Le Fort III osteotomy, two levels of anchorage to the moving segments are necessary. In this report, the authors connected the distraction forces directly to the infraorbital rims, using a surgical wire for superior anchorage. They performed this technique in four patients with craniofacial dysplasia for the management of severe maxillary hypoplasia using RED II system. This configuration not only simplified the placement

INTRODUCTION: Cases of obstructive sleep apnea syndrome have been described after setback osteotomy. Although such cases are rare, they emphasize the importance of taking into account the structure of the upper airways when performing this type of procedure. We studied the modifications provoked in the upper airways by mandibular setback osteotomy performed for dysmorphism.
MATERIAL AND METHOD: We conducted a retrospective cephalometric study in a series of 25 patients. Preoperative and late postoperative films were digitalized to obtain a precise measurement of the bone displacements and cha

We describe the use of endosseous implants in the autogenous particulate cancellous bone and marrow grafted alveoli after orthognathic surgery for dental rehabilitation of patients with cleft lip and palate. This procedure has been applied to two patients and produced good results functionally and aesthetically. The results are encouraging and indicate that implant placement after orthognathic surgery is useful for patients both with congenital missing teeth and retrognathic maxillae.

Surgery for patients with unilateral (UCLP) and bilateral (BCLP) complete cleft lip, alveolus and palate has a considerable influence upon craniofacial growth. With respect to this, the cleft team at Hannover Medical School has attempted to reduce necessary surgical interventions to labioplasty, palatoplasty and veloplasty. Still, the effects of these operations influence maxillary growth to an extent which requires orthodontic treatment in all patients. This study focuses upon the transverse alterations of the alveolar arch and the deciduous dentition after lip and palate surgery. Dental cast

The use of transbuccal positional screws for stabilisation of bony segments following bilateral sagittal split osteotomy for mandibular advancement was assessed in 700 consecutive cases. In 19 patients (2.7% of cases) screw fixation was not used as the method of fixation. Screw removal was performed in 20 patients (2.8% of cases), in 15 cases due to infection and 5 cases for diverse reasons. The incidence of inferior alveolar nerve damage was increased when compared to a group of patients managed with intermaxillary fixation following advancement BSSO treated in the same unit. In 4 patients (0

A surgical approach to treatment was required for this 35-year-old patient to correct a malocclusion characterized by a bilateral buccal crossbite. Surgical procedures included a LeFort 1 osteotomy to reduce maxillary width and correct the vertical asymmetry. The mandible was lengthened with a bilateral sagittal split osteotomy. Treatment options for the nongrowing patient are limited in the correction of severe skeletal disharmonies.
