[Management of preterm premature rupture of membranes (except for antibiotherapy): CNGOF preterm premature rupture of membranes guidelines].

OBJECTIVE: To review the different parts of therapeutic management of viable preterm premature rupture of membranes (PPROM), except the antibiotherapy and birth modalities. METHODS: The Medline, Cochrane Library, and Google Scholar databases over a period from 1980 to September 2018 have been consulted. RESULTS: When the diagnostic of viable PPROM is reached, the woman should be hospitalized and signs of intrauterine infection (IUI) should be sought (Professional consensus). If cervical assessment appears necessary, speculum, digital examination or cervical ultrasound may be performed (Profe…

Penile fracture: a rare case of simultaneous rupture of the one corpus cavernosum and complete urethral rupture.

INTRODUCTION: Penile fracture is a traumatic rupture of tunica albuginea and the tumescent corpora cavernosa due to the nonphysiological bending of the penile shaft, presenting with or without rupture of corpus pongiosum and urethra. The incidence of concomitant injury of the urethra is 0-38%. Complete urethral rupture is rare, but it is almost always associated with bilateral corporeal injury. CASE REPORT: We presented a patient with complete urethral rupture, and rupture of the right cavernous body. According to the available literature, this case is extremely rare. CONCLUSION: Fracture of…

Increased 18F-FDG uptake is predictive of rupture in a novel rat abdominal aortic aneurysm rupture model.

OBJECTIVE: To determine whether F-fluorodeoxyglucose (F-FDG) micro-positron emission tomography (micro-PET) can predict abdominal aortic aneurysm (AAA) rupture. BACKGROUND: An infrarenal AAA model is needed to study inflammatory mechanisms that drive rupture. F-FDG PET can detect vascular inflammation in animal models and patients. METHODS: After exposing Sprague-Dawley rats to intra-aortic porcine pancreatic elastase (PPE) (12 U/mL), AAA rupture was induced by daily, subcutaneous, β-aminopropionitrile (BAPN, 300 mg/kg, N = 24) administration. Negative control AAA animals (N = 15) underwent …

[Acute myocardial infarction with left ventricular free wall rupture and papillary muscle rupture caused by infectious endocarditis].

Infectious endocarditis sometimes causes coronary embolism which induces acute myocardial infarction. A 59-year-old man was admitted to our hospital with a diagnosis of acute myocardial infarction accompanied by left ventricular free wall rupture and papillary muscle rupture. We perfomed mitral valve replacement combined with repair of left ventricular free wall rupture. The anterior mitral leaflet had perforation and vegetation, which suggested that acute myocardial infarction was caused by septic embolus originated from infectious endocarditis in this case.

Bladder rupture associated with uterine rupture. A report of two cases occurring during vaginal birth after cesarean.

BACKGROUND: Uterine rupture occurs in < 1% of patients undergoing a trial of labor after cesarean section. Associated injury to adjacent organs within the maternal pelvis has likewise been very rarely reported. CASE: Two cases of posterior bladder wall rupture occurred in association with rupture of low transverse uterine incisions. CONCLUSION: Bladder rupture may be associated with uterine rupture during attempted vaginal birth after cesarean. The potential for bladder injury should be included in the patient's antepartum counseling.

Atraumatic Splenic Rupture: A Case Report.

Atraumatic splenic rupture (ASR) is a rare but potentially fatal emergency, accounting for less than 0.5% of all splenic ruptures. Its nonspecific presentation often complicates diagnosis and management. True idiopathic ASR, occurring without an underlying splenic disease, is extremely rare. We report the case of a 61-year-old woman with diffuse large B-cell lymphoma in complete metabolic remission who developed sudden, severe epigastric and left upper quadrant pain 7 days after receiving high-dose methotrexate for central nervous system prophylaxis. There was no evidence of splenic involvemen…

Osmotically-induced rupture of viral capsids.

A simple model is proposed aimed to investigate how the amount of dissociated ions influences the mechanical stability of viral capsids. After an osmotic and mechanical equilibrium is established with the outer solution, a non-adiabatic change in salt concentration at the external environment is considered, which results in a significant solvent inflow across the capsid surface, eventually leading to its rupture. The key assumption behind such an osmotic shock mechanism is that solvent flow takes place at timescales much shorter than the ones typical of ionic diffusion. In order to theoretical…

[Quadriceps Tendon and Patellar Tendon Rupture].

The vascularity of the quadriceps tendon influences the risk of tendon rupture. Quadriceps and patellar tendon ruptures are rare but serious injuries, with an incidence of 1.37/100000 and 0.68/100000, respectively. Risk factors include age, degeneration, and systemic diseases such as kidney disease and rheumatoid arthritis. While quadriceps tendon ruptures predominantly affect people over 50, patellar tendon ruptures are more common in younger, physically active people, often with pre-existing tendon pathologies. The most common mechanism of rupture of the quadriceps tendon is indirect trauma,…

[A rare cause of splenic rupture].

Splenic rupture after a colonoscopy is a rare, but potentially life-threatening complication. It should be considered in patients with acute pain in the left upper abdomen, arterial hypotension or even hemorrhagic shock. The diagnosis is made using computer tomography or sonography. Treatment depends on the severity and includes conservative management, arterial embolization or splenectomy.

Intracranial aneurysms, a major cause of subarachnoid hemorrhage(SAH), pose a significant social burden due to their poor patient outcomes. … nd turbulent flow take over, maintaining macrophage infiltration. As the disease progresses towards rupture, infiltration creates hypoxic conditions that exacerbate the situation. These conditions, in turn, induce the formation of neovessels at the weakest point of the aneurysm and promote specific inflammatory microenvironments rich in neutrophils. The e …

Complete aortic rupture following wakeboarding accident.

Wakeboarding is a sport associated with various types of injuries, primarily affecting the upper and lower limbs. In this case, a 44-year-old man fell from a ramp while wakeboarding and barely managed to reach the shore before dying shortly afterward. An autopsy revealed a complete rupture of the thoracic aorta along with a fracture of the fourth thoracic vertebra. It is likely that several mechanisms contributed to these injuries, either individually or in combination: concussive, rotational, and tensile forces. Additionally, it is plausible that the rupture occurred in two stages, with an in…

Delayed Diagnosis of Quadriceps Tendon Rupture.

Quadriceps tendon (QT) injury classically presents with pain, inability to actively extend the knee, and a palpable defect in the suprapatellar region. Unilateral eccentric contraction and hyperflexion after ground level fall are the most common mechanisms of injury. Delayed QT repair may require additional operative procedures and is associated with poorer post-operative functional outcomes, persistent knee extensor weakness, and decreased range of motion. This report demonstrates a case of delayed diagnosis of QT rupture after a fall with injury to the contralateral extremity. Initial evalua…

An Unusual Presentation of Diaphragmatic Rupture.

Representing less than 1% of all traumatic injuries, diaphragmatic injuries are uncommon and are usually associated with injuries in other thoracic and abdominal organs. We report a case of a diaphragmatic injury in a 38-year-old man who presented to the Emergency Department due to a pedestrian-vehicle accident. He had a massive hemothorax on the left due to a ruptured spleen. An exploratory laparotomy was done to manage the bleeding, restore the diaphragmatic hernia contents in their right anatomical position, conduct a splenectomy, and repair the diaphragmatic defect. Although the majority o…

Clinical Practice Update: Achilles Tendon Rupture.

Achilles tendon rupture is a common injury. It most often occurs in middle aged men who participate in recreational sports. The injury classically presents with a loud popping noise and immediate pain and weakness of the lower extremity during actions such as jumping or running. The diagnosis is made clinically, but an MRI is often obtained for confirmation of rupture and to aid in surgical planning. Treatment is either operative, with open or minimally invasive approaches, or non-operative, with functional bracing or plaster casting. Surgical treatment was preferred for much of the 20th centu…
