Intraosseous ganglions (IOGs) are actually quite common but one spanning two adjacent carpal bones is uncommon. We report a case with an IOG spanning the scaphoid and lunate, which was treated surgically. A 16-year-old right-handed female noticed left wrist pain that started spontaneously five years previously. Physical findings indicated carpal instability in the left wrist. Posteroanterior radiographs of the left wrist showed small cysts in the lunate and scaphoid, while the lateral radiograph revealed volar flexion of the lunate. Bone curettage was performed using sharp curettes, and due to

Scapholunate dissociation is a rotatory subluxation of the scaphoid with widening of the gap between the scaphoid and lunate. It is caused by rupture of the ligaments between radius, scaphoid, lunate and capitate bones. The scaphoid has lost its stabilizing function as a bridge between the proximal and distal carpal row, so that the wrist shows the typical "concertina" deformity of dorsal instability. The scapholunate angle measures about 100 degrees (more than 70 degrees). The subluxation of the scaphoid and lunate with the wide gap between these bones is best treated by primary repair of the

The forearm distal bone fracture is the most com-mon fracture in children. Scaphoid frac-ture is rare because the scaphoid is largely cartilaginous in children. Some cases associating both distal radius and scaphoid fractures were reported in the literature. Scaphoid fracture often occurs without displacement and can be easily overlooked. We reported a case of children wrist injury combining scaphoid fracture with ipsilateral distal fracture.

BACKGROUND: The scaphoid bone is the most commonly fractured of the carpal bones. In the Netherlands 90% of all carpal fractures is a fracture of the scaphoid bone. The scaphoid has an essential role in functionality of the wrist, acting as a pivot. Complications in healing can result in poor functional outcome. The scaphoid fracture is a troublesome fracture and failure of treatment can result in avascular necrosis (up to 40%), non-union (5-21%) and early osteo-arthritis (up to 32%) which may seriously impair wrist function. Impaired consolidation of scaphoid fractures results in longer immob…

We tested the hypothesis that the fracture location of scaphoid nonunions relates to the fracture displacement, development of dorsal intercalated segment instability (DISI) deformity, and changes in the contact area of the bones in the radiocarpal joint. Eleven patients with scaphoid nonunions were examined with 3-dimensional computed tomography and a new method of proximity mapping. Two different patterns of displacement of scaphoid nonunions were demonstrated, 1 volar and 1 dorsal. All patients with a volar pattern scaphoid nonunion had a DISI deformity. Only a few of the patients with a do

Individuality, which is so expressed in medicine has found as well its application in fractures and pseudoarthroses of scaphoid bone. In the treatment of fracture and pseudoarthrosis of scaphoid bone in each case, patient's occupation, age, body constitution, intensity of pains, functional status of fist and extent of osteonecrotic changes should be considered. In our work we have obtained the best results in "early" pseudoarthrosis by surgical treatment according to Matti-u and Russe-u and in the "late" ones by the method of complete extirpation of necrotic scaphoid. As fist and wrist-joint a…

BACKGROUND: The treatment of scaphoid nonunion is challenging, with one approach aiming to provide vascularized bone to encourage union. While the iliac crest and medial femoral condyle are well-described donor sites for osseous flaps, they require violation of a separate limb and confer particular donor site morbidities. We investigate the viability of using a proximal olecranon osseous free flap in the setting of scaphoid nonunion.
METHODS: Ten proximal olecranon free flaps were harvested in cadaveric specimens, and the length of the pedicle, diameter of the pedicle, number of perforators a

The numerical study of the scaphoid fracture, although it is relatively unexplored, can be of great clinical interest since it is highly com … computational cost savings. This study provides a comprehensive assessment into the modeling of the scaphoid bone and its interactions with adjacent bones. The findings reveal that various choices of interactions can yield similar results, allowing for flexibility in selecting interaction models based on desired accuracy or computational efficiency. Ultimately, …

OBJECTIVE: To help make diagnosis and treatment of scaphoid fracture more precise by review of published evidence.
QUALITY OF EVIDENCE: MEDLINE was searched using the terms "scaphoid," "carpal navicular," "fracture," "computed tomography," "bone scan," and "scintigraphy." Most papers were case-series observational reports. Papers were cited if the case series was large or if there was a high degree of agreement among several observers. The main recommendation for change in treatment of scaphoid fracture is based on two randomized clinical trials involving more than 1000 patients with proven s

Avascular necrosis of bone may affect specific areas of the body where the blood supply to the bone or part of the bone is vulnerable to injury or disease. In the carpal scaphoid, it can find this entity consequent a trauma, microtrauma, or fractures, especially in the proximal pole (Preiser in Fortschr Geb Roentgenstr 15:189-197, 1910), associated with a systemic disease, steroid ingestion (Ferlic and Morin in J Hand Surg-A 14:13-16, 1989), chemotherapy (Green in Hand Clinic 3:163-168, 1987) or a hypoplastic scaphoid (Gunal et al., in J Hand Surg-B 20(6):736-740, 1995). We retrospectively analyzed the radiographic findings in 76 patients with proven scaphoid (navicular) fractures. The purpose of the study was to determine the incidence of the impacted buckle fracture of the scaphoid bone in children. Fourteen of the 76 patients (17%) demonstrated such a fracture, and all of the fractures occurred in the middle or distal third of the scaphoid bone. The mean age of this group of patients was 13.7 years, as compared to the mean age of 15.6 years in the patients with overt transverse fractures. One out of 14 of the patients with a buckle fracture was female. None…

The purpose of this paper is to retrospectively review 234 consecutive cases of scaphoid fractures and nonunions treated using arthroscopy with the dorsal percutaneous implantation of a headless compression screw for healing and complications. Solid union of fracture is determined by CT scan. We identified 126 acute injuries, including 65 proximal pole fractures; 67 grossly displaced fractures; 12 trans-scaphoid perilunate dislocations including four trans-scaphoid trans-capitate fractures; and ten combined scaphoid and distal radius fractures. 108 scaphoid nonunions were identified. 98 were c

BACKGROUND: Our goal was to analyze the movement of acute scaphoid waist fracture fragments and adjacent bones in a common coordinate system. Our hypothesis was that the distal scaphoid fragment flexes and pronates and the proximal fragment extends.
METHODS: Computed tomography (CT) scans of patients diagnosed with an acute scaphoid waist fracture were evaluated using a 3-dimensional (3D) model. The scans of 57 nondisplaced and 23 displaced fractures were compared with a control group of 27 scans showing no pathological involvement of the wrist. Three anatomical landmarks were labeled on the

BACKGROUND: Rapid diagnosis of scaphoid bone fracture in the wrist is important so that appropriate treatment can be started. If a fracture is clinically suspected without being visible on radiography, further investigation has to be conducted to reveal a fracture or to rule it out. The objective of this study was to investigate the validity of computed tomographic (CT) scanning and bone scintigraphy compared with the clinical fracture rate during follow-up of 1 year for examining patients with a suspected scaphoid fracture.
METHODS: Bone scintigraphy and CT scanning were performed in 29 pati…

BACKGROUND: Radiography alone will not detect all scaphoid fractures. There is a reported prevalence between 9 and 33% of occult scaphoid fractures. The evidence-based literature suggests that magnetic resonance imaging (MRI) is the most suitable secondary imaging modality due to the ability to evaluate the bone marrow directly and to also identify other injuries. However, there is no consensus on the choice of follow-up imaging strategy-computed tomography, MRI, or bone scan-across different institutions. Tomosynthesis is a new digital tomographic method creating multiple thin tomographic sec

Scaphoid fractures represent the most common carpal fracture and carry a risk of nonunion, particularly when diagnosis is delayed, fixation is inadequate, or the proximal pole is involved because of limited vascularity. Nonunion can result in carpal collapse and degenerative wrist arthritis if untreated. Traditional open repair techniques provide access for debridement and bone grafting but often require capsular and ligamentous dissection that may compromise vascularity and increase postoperative stiffness. Arthroscopic-assisted scaphoid nonunion repair has emerged as an alternative that pres

The kinematics of five fresh frozen wrist specimens were studied before and after a simulated scaphoid waist fracture. To determine the change in wrist motion and fracture site characteristics associated with an unstable wrist, the relative motion of each carpal bone was determined from the movement of implanted carpal markers on biplanar radiographs obtained in neutral and the four extreme wrist positions. The kinematics of the wrist in our specimens before the osteotomy were similar to previous studies. After the osteotomy, the proximal and distal segments of the scaphoid moved independently

A case of bilateral congenital aplasia of the carpal scaphoid bone is presented. In contrast to previously reported cases of congenital hypoplasia of the scaphoid, this case was not associated with hypoplasia or absence of thenar and forearm muscles, absence of the sesamoid bones of the thumb, abnormalities of the skeleton of the thumb ray or hypoplasia of the forearm bones.

Clinically suspected scaphoid fractures are common injuries. The authors prospectively followed up, by serial radiography and bone scanning, 23 patients who had a history of trauma to the carpus and associated localized tenderness to the anatomic "snuff box": in 15 patients the radiographs and bone scans were negative; in 5 the radiograph was equivocal and the bone scan positive. The authors concluded that patients with clinically suspected scaphoid fractures and negative radiographs can be treated symptomatically without immobilization in a cast. Patients with equivocal radiographs should be
