Antecedent history, clinical course, response to treatment, and final visual acuity were documented. RESULTS: Methicillin-resistant S. aureus infectious scleritis required combination systemic, periocular, intraocular, and topical antibiotics for complete resolution. Visual recovery was limited. CONCLU…

Hypopyon uveitis (without scleritis) a manifestation symptom of relapsing polychondritis.

We report an atypical ocular symptom, hypopyon uveitis without scleritis encountered in relapsing polychondritis. Relapsing polychondritis should be considered in the differential diagnosis of sterile hypopyon uveitis.

Surgically induced necrotizing scleritis after pterygium surgery with conjunctival autograft.

PURPOSE: To report a case of surgically induced necrotizing scleritis (SINS) as a complication of conjunctival autograft after pterygium excision. METHODS: A 52-year-old man had undergone nasal pterygium excision with conjunctival autograft in the right eye at another facility. He was treated for suspected infective scleritis before presentation. RESULTS: The sclera, at the site of prior pterygium excision, showed significant thinning with uveal show. There was active inflammation adjacent to the site of thinning. Systemic studies and the examination were noncontributory. The patient was sus…

Therapy insight: scleritis and its relationship to systemic autoimmune disease.

The term scleritis describes a chronic inflammation that involves the outermost coat and skeleton of the eye. Disease can be isolated to the eye, but in up to half of affected individuals it occurs in the context of an immune-mediated systemic inflammatory condition, such as rheumatoid arthritis or Wegener's granulomatosis. Although uncommon, scleritis is often extremely painful, can lead to vision-threatening complications (and involvement of other ocular tissues), and is considered to confer an increased risk of mortality in patients with rheumatoid arthritis. Pathogenic mechanisms in scleri…

Subpalpebral lavage antibiotic treatment for severe infectious scleritis and keratitis.

OBJECTIVE: To report the subpalpebral lavage therapy for the treatment of infectious scleritis and keratitis. METHOD: Six patients were admitted for treatment of severe infectious scleritis and keratitis and were initiated on the subpalpebral lavage system after showing no improvement with topical fortified antibiotics. A continuous antibiotic lavage was applied until clinical sterility was achieved and topical steroids were gradually added to control concomitant inflammation. RESULTS: All 6 patients had resolution of their infections and achieved a stable ocular surface with no inflammation…

Necrotizing scleritis due to varicella zoster infection: a case report.

PURPOSE: To report a case with necrotizing scleritis due to varicella-zoster infection. METHODS: The patient records were evaluated. The present literature was investigated using MEDLINE. A six-year-old boy with varicella infection was admitted to our clinic with redness, pain, and lid edema on the right eye. Slit lamp examination revealed lid edema, purulent secretion, conjunctival injection and chemosis, and inferotemporal scleral necrosis. Sclera was avascular and the conjunctiva was spontaneously detached from sclera in the necrotic region. RESULTS: Systemic and topical acyclovir treatme…

Giant nodular posterior scleritis simulating choroidal melanoma and birdshot retinochoroidopathy.

A case of nodular posterior scleritis simulating both a choroidal tumor and birdshot chorioretinopathy is described. Fluorescein angiography, indocyanine green angiography, and ultrasound were performed. Results of laboratory tests were normal, including HLA-A29, erythrocyte sedimentation rate, rheumatoid factor, and antinuclear antibodies. A chest x-ray was normal. A diagnosis of nodular posterior scleritis was made and the minimally symptomatic patient without ocular pain was observed unless symptoms progressed. In this case, certain clinical features led to the proper diagnosis.

Anterior segment indocyanine green angiography in anterior scleritis and episcleritis.

OBJECTIVE: To evaluate the pattern of anterior segment indocyanine green (ICG) angiography in episcleritis and scleritis. DESIGN: Prospective comparative (paired-eye) observational case series. PARTICIPANTS: Twenty subjects presenting clinical diseases compatible with episcleritis or scleritis. METHODS: Anterior segment ICG angiography was performed according to a standard protocol in subjects presenting either episcleritis or scleritis. Photographs of the anterior segment were taken in the early phase (up to 3 minutes after dye injection), intermediate phase (10-12 minutes) and late phase …

Ocular characteristics and disease associations in scleritis-associated peripheral keratopathy.

OBJECTIVES: To evaluate ocular characteristics and systemic disease associations in patients with scleritis-associated peripheral keratopathy and its different patterns, and to assess any ocular or systemic prognostic significance of the presence of the types of peripheral keratopathy in patients with scleritis. DESIGN: Review of 125 patients with scleritis alone and 47 patients with scleritis-associated peripheral keratopathy; review of patients with scleritis and the different patterns of peripheral keratopathy: peripheral corneal thinning, stromal keratitis, and peripheral ulcerative kerat…

Surgically induced necrotizing scleritis after pterygium excision and conjunctival autograft.

PURPOSE: To report a case of surgically induced necrotizing scleritis (SINS) after pterygium excision and conjunctival autograft. METHODS: A 55-year-old man presented 2 weeks after excision of primary pterygium with conjunctival autograft in the right eye with severe pain in that eye. He had undergone cataract surgery in that eye 8 months before. The graft was pale and white. The underlying sclera was white and avascular. There was marked inflammation adjacent to the site of surgery and graft. A scraping from the graft surface revealed no organisms in smears, and culture revealed no growth. T…

Myelin oligodendrocyte glycoprotein antibody-associated disease presenting with posterior scleritis.

The objective of this article is to describe an unusual presentation of myelin oligodendrocyte glycoprotein antibody-associated disease (MOGAD) with posterior scleritis and optic neuritis. We report the case of a 50-year-old woman who presented with acute visual deterioration, headache, and painful eye movements. Clinical examination revealed profound vision loss, optic disc edema, and posterior scleritis confirmed via B-scan ultrasound. Magnetic resonance imaging demonstrated optic nerve involvement with perineuritis, and myelin oligodendrocyte glycoprotein antibodies were detected. The patie…

Bilateral Corneal Sensory and Nerve Structural Alterations in Unilateral Scleritis.

PURPOSE: The objective of this study was to explore the corneal sensitivity and corneal structure of both eyes in patients with unilateral scleritis. METHODS: This was a cross-sectional case series. We enrolled patients with unilateral scleritis at the Eye Hospital of Wenzhou Medical University between April 2024 and December 2025. Patients with corneal involvement (staining ≥5) were excluded. The corneal sensation of both eyes was examined using a noncontact corneal aesthesiometer and a Cochet-Bonnet aesthesiometer. Then, the corneal structure was examined by in vivo confocal microscopy, and…

Spontaneous Descemet Membrane Detachment After Necrotizing Scleritis: A Case Report.

PURPOSE: We report a case of spontaneous Descemet membrane detachment (DMD) in a patient with a history of necrotizing scleritis and discuss the management approach and outcome. METHODS: This was a case report and review of literature. RESULTS: A 50-year-old man with a history of rheumatoid arthritis, ankylosing spondylitis, and necrotizing scleritis on systemic methotrexate, rituximab, and prednisone presented with 6 weeks of photophobia, pain, and decreased vision in his left eye. Visual acuity on presentation was 20/80 in the left eye, and intraocular pressure was 18 mm Hg. Slit lamp exam…
