We report on a 25-year-old man who had scrub typhus with the unusual complication of acute renal failure. The clinical features of fever, headache, high Weil-Felix OX-K agglutination and Rickettsia tsutsugamushi immunofluorescence titers confirmed the diagnosis of scrub typhus. Acute renal failure was proven by oliguria, urinary diagnostic indices and renal biopsy. The patient had a complete recovery after adequate medical treatment.

We report on a 20-year-old man who had scrub typhus with the unusual neurologic complication of brachial plexus neuropathy. The clinical features of fever, headache, pneumonitis, eschar, high Weil-Felix OX-K agglutination and Rickettsia tsutsugamushi immunofluorescence titers confirmed the diagnosis of scrub typhus. Brachial plexus neuropathy was proven by an electrophysiologic examination. He had a nearly complete recovery after adequate medical treatment.

The indirect immunoperoxidase technique was assessed for the serodiagnosis of tsutsugamushi fever (scrub typhus). The antigens were peritoneal smears prepared from mice infected intraperitoneally with the Karp, Kato, and Gilliam strains of Rickettsia tsutsugamushi. A man in his late 30s presented with a prolonged febrile illness and recurrent syncopal episodes. Physical examination revealed hypotension, bradycardia and eschars on the bilateral thighs. A 12-lead ECG showed complete heart block with wide QRS escape rate of 30 beats per minute. Laboratory tests confirmed scrub typhus with elevated cardiac markers.
