Self-exclusion program: a longitudinal evaluation study.

Few self-exclusion programs have been evaluated and their long-term impact remains unknown. This study has two main goals: (1) to assess changes in gambling behaviour and gambling problems for self-excluded patrons, and (2) to follow self-excluded gamblers for a two-year period (during and after the self-exclusion period). Individuals who excluded themselves (N = 161 at the initial stage) participated in telephone interviews after signing the self-exclusion agreement and were followed at 6, 12, 18 and 24-months. Results show that according to the DSM-IV, 73.1% of the participants were patholog

Smokeless tobacco cessation in military personnel: a randomized controlled trial.

INTRODUCTION: Military personnel are twice as likely as civilians to use smokeless tobacco (ST). This study evaluated the efficacy of a minimal-contact ST cessation program in military personnel. METHODS: Participants were recruited from 24 military dental clinics across the United States during annual dental examinations. Participants were 785 active-duty military personnel who were randomly assigned to receive a minimal-contact behavioral treatment (n = 392) or usual care (n = 393). The behavioral treatment included an ST cessation manual, a videotape cessation guide tailored for military p

The stepped-care approach in anorexia nervosa and bulimia nervosa: progress and problems.

The stepped-care approach is based on the assumption that treatment should be less intensive and hence less expensive and intrusive at the start. Current data suggest that cognitive behaviour therapy (CBT) should be the preferred first treatment for bulimia nervosa (BN) patients. By comparison with the numerous trials supporting its efficacy, in fact, the evidence sustaining similar therapies, e.g. interpersonal psychotherapy (IPT) or pharmacological therapy, is weaker. There are now sufficient data to justify the use of less intensive and shorter treatment (i.e. psychoeducational groups and s

Using therapeutic groups to support women with faecal incontinence.

In this paper, the authors describe the use of facilitated patient groups, in the management of women with faecal incontinence (FI). Two types of groups are discussed--a psychoeducational group and a psychotherapy group. Detailed descriptions of some of the themes which emerged in these groups are provided. The effectiveness of such groups is described, with regard to both psychological and physical functioning. Further investigation into the use of groups for this patient population is recommended.

Continuous positive airway pressure machines and the work of coordinating technologies at home.

OBJECTIVES: Research on the use of health technology at home has consistently reported that it is underpinned by users' evaluations of the technology in relation to their illness trajectory, their social network, and the place where it is used. Recent studies have also revealed that the efficacy of health technologies depends upon users' work that is largely invisible to professionals, managers and designers. This paper draws on the most widely used therapy for the management of sleep apnoea, continuous positive airway pressure (CPAP), to show how users maintain workable relationships between

Sexual assault within intimate partner violence: impact on helpseeking in a national sample.

Within intimate partner violence (IPV), sexual assault is often subsumed under the heading of physical abuse, but evidence suggests qualitative differences in outcomes when both types of abuse occur. This study explores the cumulative effect of sexual assault and physical abuse by a current or former intimate partner on helpseeking. Using a dataset of 1,072 IPV victims from 8 states, we found that women who had experienced sexual assault in addition to physical abuse (44%) used more help, but were also more likely to say that they did not seek help when they needed it. Among those who were awa

A randomized clinical trial of the Recovery Line among methadone treatment patients with ongoing illicit drug use.

BACKGROUND: Relapse, drug use, and treatment dropout are common challenges facing patients receiving methadone. Though effective, multiple barriers to face-to-face counseling exist. The Recovery Line (RL), an automated, self-management system based on Cognitive Behavioral Therapy, is a phone-based adjunctive treatment that provides low cost, consistent delivery and immediate therapeutic availability 24 h a day. METHODS: The current study was a 12-week randomized clinical efficacy trial of treatment-as-usual (TAU) only or RL + TAU for methadone treatment patients with continued illicit drug us

Self-Guided Psychological Interventions for the Treatment and Prevention of Eating Disorders: A Meta-Analysis of Randomized Controlled Trials.

BACKGROUND: Self-guided interventions may broaden the dissemination of evidence-based prevention and treatment protocols for eating disorders. We conducted a meta-analysis comparing self-guided prevention and treatment approaches for eating disorders to (1) control groups and (2) professionally guided self-help programs. METHODS: Forty-six trials were included. Interventions ranged from web, to app, to CD-ROM to book-based programs. Random effects meta-analyses were conducted on numerous symptom and risk outcomes. RESULTS: Only four trials recruited an unselected sample, with self-guided pro

Tailoring cognitive assistance for individuals with a traumatic brain injury using assistive technology for cognition: translating clinical reasoning into ontologies.

PURPOSE: Individuals with a traumatic brain injury (TBI) frequently require cognitive assistance to engage in complex activities, emphasising the need for personalised support to enhance their independence. This study explored how occupational therapists use evaluation results to tailor cognitive assistance and translated these results into clinical reasoning ontologies that could be integrated into assistive technologies for cognition (ATCs). MATERIAL AND METHODS: Fifteen occupational therapists and occupational therapy master's students participated in focus groups presenting case studies o

It takes a village: A pilot study of a group telehealth intervention for support persons affected by opioid use disorder.

INTRODUCTION: Opioid use disorder (OUD) has devastating effects on individuals, families, and communities. The Community Reinforcement and Family Training (CRAFT) is a Support Person (SP)-focused intervention that aims to increase SPs' communication strategies, positive reinforcement/rewards, and social support. This pilot study, called eINSPIRE (INtegrating Support Persons Into REcovery), adapted CRAFT for delivery via group telehealth. The aims were to evaluate the feasibility, acceptability, and preliminary effectiveness of this intervention on patient buprenorphine retention and SP mental

Randomized trial of internet-delivered self-help with telephone support for pathological gamblers.

Although effective therapies for pathological gambling exist, their uptake is limited to 10% of the target population. To lower the barriers for help seeking, the authors tested an online alternative in a randomized trial (N = 66). The participants were pathological gamblers not presenting with severe comorbid depression. A wait-list control was compared with an 8-week Internet-based cognitive behavior therapy program with minimal therapist contact via e-mail and weekly telephone calls of less than 15 min. Average time spent on each participant, including phone conversations, e-mail, and admin

Life stories of depressed adult women in peri-urban Namibia.

The problems women in peri-urban Namibia are faced with are multi-dimensional. Like women in other communities they face the pressure of having a number of responsibilities, namely working, being a wife and mother, taking care of their families and perhaps caring for aging parents. Sometimes the pressure can be too overwhelming to manage. As a result, many women become depressed. Studies on depression among black African women in Namibia could not be traced. It was therefore considered to find out how women suffering from depression from this part of the world tell their life stories. The purp

Alternative psychotherapy approaches for social anxiety disorder.

Alternative therapies and therapy modalities for SAD are needed because: Established treatments (CBT and pharmacologic) do not help everyone who seeks help. Established treatments provide only partial decrease in symptoms for many patients. Patients may experience recurrence of symptoms in long-term follow-up. CBT does not reach enough patients in need. Alternative treatment approaches and modalities may also be needed to address the successful outcomes of CBT. Success in overcoming social anxiety symptoms can generate a whole new set of challenges. For example, a 31-year-old man who overcomes

Outpatient management of patients with alcohol problems.

This paper addresses the clinical presentation of patients with alcohol problems including screening, diagnosis, detoxification, treatment, referral, and longitudinal follow-up. The case-based discussion focuses on the clinical management of a patient whose hazardous drinking progresses to alcohol dependence and requires coordinated care. Ways in which internists can aid patients with alcohol problems by screening, giving brief advice, and providing appropriate referrals when indicated are discussed.

Online support and co-rumination experiences among adults in substance use recovery.

OBJECTIVE: This study examined patterns of perceived social support and co-rumination (e.g., repeatedly discussing problems) in social media recovery groups and associated whole-person wellness among adults in recovery from substance use disorder (SUD). METHODS: A cross-sectional online survey (n = 468) was conducted in collaboration with a Community Advisory Board (CAB). The CAB reviewed and refined the survey, led recruitment, interpreted results, and co-authored this manuscript. Latent profile analysis (LPA) identified subgroups based on four online social support domains and three co-rumi
