Renal venous congestion following hemorrhagic shock due to traumatic liver injury.

A 78-year-old woman who sustained traumatic liver injury with hemorrhagic shock was hospitalized. She was admitted to the ICU after blood transfusion and emergent angiography. AKI was observed on the following day. Blood transfusion was continued because initial assessment was prerenal AKI due to hypovolemia. Despite transfusion of blood products and administration of diuretics, aggravated renal dysfunction, and low urine output continued, resulting in respiratory failure due to pulmonary edema. Renal venous congestion was suspected as the primary cause of AKI, since IVC compression from a hem

Acadesine and lipopolysaccharide-evoked pulmonary dysfunction after resuscitation from traumatic shock.

BACKGROUND: We have reported that the purine precursor acadesine (AICAR) improved the microcirculation, repleted adenosine triphosphate, and attenuated local and lung neutrophil infiltration after intestinal reperfusion and that it quickly improved systemic hemodynamics after resuscitation from hemorrhagic shock. This study evaluated the therapeutic potential of AICAR after fluid resuscitated trauma. METHODS: Anesthetized (fentanyl) mongrel pigs were subjected to tissue injury plus hemorrhage and randomized to receive resuscitation fluids comprised of shed blood plus either lactated Ringer's

Neutrophil CD18 expression and blockade after traumatic shock and endotoxin challenge.

OBJECTIVE: The expression of the leukocyte CD18 adhesion complex on polymorphonuclear leukocytes (PMNs) was measured, and the physiologic effects of blockade of the complex were studied after trauma and sepsis. SUMMARY BACKGROUND DATA: Margination of PMNs occurs early during inflammation and depends, in part, on expression of the CD18 adhesion complex. Blockade of this adherence complex can reduce PMN-mediated damage. This study tests the hypothesis that PMN activation after resuscitated trauma produces an occult endothelial injury that increases the vulnerability to a delayed inflammatory st

[Transfusion therapy of traumatic shock using plasma substitutes oxyamal and volecam].

The work is devoted to the investigation of efficiency of antishock transfusion therapy by new soviet drugs on the basis of hydroxyethyl starch--Oxyamal and Volecam. The investigation was performed on 30 patients with trauma shock and hypovolemic syndrome. The clinico-laboratory data obtained have shown that transfusion of Oxyamal and Volecam allow to rapidly normalize disturbed parameters of the central hemodynamics and continuously maintain them at the due level.

A pragmatic clinical definition of traumatic shock using systolic blood pressure and shock index: A national registry study.

BACKGROUND: Traumatic shock (TS) is implicated in up to 40% of early trauma deaths; however, no consensus definition exists, and its epidemiology and outcomes have not been characterised at a population level in Australia. AIMS: To apply a pragmatic clinical definition of TS, based on systolic blood pressure (SBP) and shock index (SI), to a national trauma registry, and to describe the epidemiology and outcomes of major trauma (MT) patients with and without clinical signs of shock. METHODS: A retrospective cohort study was conducted using Australia New Zealand Trauma Registry (ANZTR) data (2

[Features of the morphofunctional response of Peyer's patches in rats during the course of traumatic shock and the early post-shock period].

The reaction of Peyer's patches to trauma manifests in: 1) vascular changes; 2) primary or stressor cells shifts recordable in traumatic shock, characterized by depletion of patch follicles by minor lymphocytes; 3) secondary changes associated with the onset of reparative processes; 4) the increased macrophagal activity detectable during the entire period of the traumatic shock and in the early post-shock period. It is suggested that changes seen may be due to endotoxin that penetrates from the intestine.

Obstructive shock induced by internal thoracic artery injury with traumatic sternal fracture.

Internal thoracic artery (ITA) injuries associated with sternal fractures can lead to shock. Several studies have documented injuries resulting in hemorrhagic shock, yet there is limited reporting on obstructive shock. Opinions differ regarding which is superior between transcatheter arterial embolization (TAE) and open thoracotomy. We report the case of an 80-year-old female patient presented with blunt chest trauma when driving. Her vital signs were normal. However, ultrasonography revealed a hypoechoic anterior mediastinal lesion. Her blood pressure decreased immediately before undergoing a

Danger signals in traumatic hemorrhagic shock and new lines for clinical applications.

Hemorrhage is the leading cause of death in severe trauma injuries. When organs or tissues are subjected to prolonged hypoxia, danger signals-known as damage-associated molecular patterns (DAMPs)-are released into the intercellular environment. The endothelium is both the target and a major provider of damage-associated molecular patterns, which are directly involved in immuno-inflammatory dysregulation and the associated tissue suffering. Although damage-associated molecular patterns release begins very early after trauma, this release and its consequences continue beyond the initial treatmen

Modeling Acute Traumatic Hemorrhagic Shock Injury: Challenges and Guidelines for Preclinical Studies.

Trauma is responsible for a large proportion of the world's burden of disease, and is by far the biggest killer of young adults. Hemorrhage is the leading cause of preventable death and its effects are directly correlated with the incidence multi-organ failure in survivors. Trauma research is challenging due to patient heterogeneity, limited randomized controlled trials, and in vitro studies that fail to mimic the systemic injury response. Preclinical research remains essential for mechanistic and therapeutic discovery. Yet modeling the multifaceted nature of traumatic injury poses important e

Alcohol's Burden on Immunity Following Burn, Hemorrhagic Shock, or Traumatic Brain Injury.

Alcohol consumption contributes to increased incidence and severity of traumatic injury. Compared with patients who do not consume alcohol, alcohol-consuming patients have higher rates of long-term morbidity and mortality during recovery from injury. This can be attributed in part to an impaired immune response in individuals who consume alcohol. Acute and chronic alcohol use can affect both the innate and adaptive immune defense responses within multiple organ systems; the combination of alcohol use and injury results in increased susceptibility to bacterial and viral pathogens. This review e
