Glenoid exposure in total shoulder arthroplasty.

Glenoid exposure is agreed to be a difficult step, but is also a key step in total shoulder arthroplasty, both anatomic and reverse. It conditions unhindered use of the ancillary instrumentation and thus correct glenoid component positioning. The main stages comprise arthrotomy, by opening the rotator cuff, humeral head cut, and inferior glenohumeral release, enabling shifting of the humerus and good exposure of the glenoid cavity. The two main approaches are deltopectoral and anterosuperior transdeltoid. Using the deltopectoral approach, arthrotomy is performed through the subscapularis muscl

Does stress shielding after radial head arthroplasty affect functional outcomes?

BACKGROUND: Various complications related to the prosthesis, such as implant loosening and stress shielding phenomenon, could develop after prosthetic replacement of the radial head. Stress shielding is known to occur around rigidly fixed implants. The purpose of this study was to evaluate the clinical influence and causative factors of the stress shielding phenomenon after radial head arthroplasty (RHA). METHODS: Clinical records and radiographs of 56 patients with unreconstructable radial head fractures who received radial head replacement between 2009 and 2019 were reviewed. Exclusion crit

The Effect of Technological Innovations in Reverse Total Shoulder Arthroplasty on the Rate of Periprosthetic Acromion and Scapular Spine Fractures.

INTRODUCTION: Innovations in reverse total shoulder arthroplasty (rTSA) have improved outcomes and mitigated complications through improvements in implant design, surgical techniques, and enabling technologies. The purpose of this study was to evaluate the association between technologic evolutions in rTSA and the incidence of acromion and scapular spine fracture (ASF). The authors hypothesize that ASF incidence has decreased over time and that this decrease is temporally associated with successive changes in implant design, surgical technique, and enabling technology within a single-design pl

Outcome and Failure Analysis of 132 Episodes of Hematogenous Periprosthetic Joint Infections-A Cohort Study.

BACKGROUND: The outcomes of hematogenous periprosthetic joint infection (PJI) and reasons for failure are largely unknown. METHODS: The outcomes of consecutive patients with hematogenous PJI treated at our institution between 2010 and 2019 were evaluated. Failure was classified as persistence or relapse of infection or new infection. Failure-free survival was assessed using Kaplan-Meier analysis. Proportions between groups were compared with the Fisher exact test. RESULTS: One hundred thirty-two hematogenous PJI episodes involving knee (n = 76), hip (n = 54), shoulder (n = 1), or elbow (n =

Retrospective cohort study on radial head arthroplasty comparing long-term outcomes between valgus type injury and fracture dislocation.

BACKGROUND: Radial head arthroplasty (RHA) has been commonly adopted for irreparable radial head fractures while little information is addressed on valgus type injury. The purpose of this study is to report long-term outcomes and radiographic analysis in RHA for valgus type injury with comparison to fracture dislocation injury. METHODS: A retrospective cohort study was conducted in patients receiving unilateral RHA with loose-fit, modular metal prosthesis for irreparable radial head fractures between 2004 and 2012. Totally, 33 patients with a mean follow up of 9 years (range, 7 to 15 years) w

Accuracy in predicting 135° neck-shaft angle short-stem and glenoid components size in reverse shoulder arthroplasty with 3D preoperative planning.

BACKGROUND: Reverse shoulder arthroplasty (RSA) is a widely used procedure for glenohumeral joint degeneration. Despite advancements in 3D preoperative planning, the concordance between planned and intraoperatively implanted components remains unclear. This study evaluates the accuracy of 3D preoperative planning in predicting humeral and glenoid components size in RSA with a 135° neck-shaft angle (NSA) short-stem. MATERIALS AND METHODS: A retrospective study was conducted on 84 RSA cases performed between October 2023 and September 2024 at a specialized shoulder surgery center. All cases und

Implant survival at four years for hemiarthroplasty and total shoulder arthroplasty in the treatment of atraumatic avascular necrosis of the humeral head.

AIMS: Hemiarthroplasty (HA) and total shoulder arthroplasty (TSA) are often the preferred forms of treatment for patients with atraumatic avascular necrosis of the humeral head when conservative treatment fails. Little has been reported about the survival of HA and TSA for this indication. The aim of this study was to investigate the differences in revision rates between HA and TSA in these patients, to determine whether one of these implants has a superior survival and may be a better choice in the treatment of this condition. METHODS: Data from 280 shoulders with 159 primary HAs and 121 TSA

Comparing clinical and functional outcomes following anatomic total shoulder arthroplasty based on humeral component length (Equinoxe; Exactech, Inc.).

BACKGROUND: There has been a trend toward the use of shorter humeral components with the goal of preserving bone, decreasing operative time, and a theoretical decrease in stress shielding. The purpose of this study was to compare the clinical and functional outcomes of anatomic total shoulder arthroplasty among standard-length stem, short-stem, and stemless humeral components, with a minimum of 2-year follow-up. METHODS: A retrospective review of a multicenter international database was performed for anatomic total shoulder arthroplasty with a minimum of 2-year follow-up between 2018 and 2021

Mid-term outcomes of pyrolytic carbon humeral resurfacing hemiarthroplasty compared with metal humeral resurfacing and metal stemmed hemiarthroplasty for osteoarthritis in young patients: analysis from the Australian Orthopaedic Association National Joint Replacement Registry.

BACKGROUND: The optimal surgical management of glenohumeral osteoarthritis in young patients remains an unsolved problem. Humeral resurfacing hemiarthroplasty and stemmed hemiarthroplasty using metallic heads are 2 surgical options that avoid the complications of loosening or wear of the glenoid component seen in total shoulder arthroplasty. Despite the potential benefits, improvement in survivorship has not been demonstrated from joint registry studies or other studies at mid-term follow-up. This is predominantly because of glenoid erosion and pain that occur when the metal resurfaced head ar

Numerical simulation and analysis of fatigue performance for the humeral stem.

BACKGROUND AND OBJECTIVE: Fatigue failure of the humeral stem is a severe long-term failure after shoulder arthroplasty, causing harm to patients and resulting in complex revision surgeries. However, there are few studies on humeral stem fatigue testing, and corresponding testing standards have not been established. Therefore, this study aims to investigate the fatigue performance of the humeral stem by establishing an efficient numerical simulation method. METHODS: Material properties are obtained by uniaxial tensile and fatigue tests. A parameterized static analysis program was written, and

Time-elapsed microstructural imaging of failure of the reverse shoulder implant.

BACKGROUND: Reverse Shoulder Arthroplasties (RSA) have become a primary choice for improving shoulder function and pain. However, the biomechanical failure mechanism of the humeral component is still unclear. The present study reports a novel protocol for microstructural imaging of the entire humerus implant under load before and after fracture. METHODS: A humerus specimen was obtained from a 75-year-old male donor. An expert surgeon implanted the specimen with a commonly used RSA implant (Aequalis reversed II, Stryker Orthopaedics, USA) and surgical procedure. The physiological glenohumeral

A multi-rigid-body system model for thorax and upper-limb joint kinematics estimation designed for prosthetic users with trans-radial amputation.

Upper-limb joint kinematics has often been estimated using customized approaches tailored for specific research questions. While multiple features are critical for an accurate estimate of upper-limb kinematics, only a few models combine them, and none has been applied to trans-radial prosthetic users. This study aimed to develop a multi-body system model of the thorax and upper-limb joint kinematics, aligned with the International Society of Biomechanics guidelines, applicable to both trans-radial prosthetic users and able-bodied individuals. The model's performance was compared to a commonly
