Development and validation of first-ever speech-specific perceptual speech evaluation tool for patients with head and neck cancer: the London speech evaluation (LSE) scale.

BACKGROUND: The aim of this study was to develop and validate the first ever speech-specific perceptual speech-evaluation tool for patients with head and neck cancer. METHODS: Five speech parameters (intelligibility, articulation, speech rate, nasality, and asthenia) and overall grade were included and evaluated. Speech samples of 117 subjects were recorded on electroglottograph equipment using a standard protocol and were independently judged and rated by 3 experienced speech and language therapists and re-rated 12 weeks apart. RESULTS: Among patients the Cronbach's alpha (α) coefficients f

Attendance in voice therapy: can an interdisciplinary care model have an impact?

OBJECTIVES: We sought to determine the effect of referral patterns on attendance in voice therapy. METHODS: Patients who were seen by a laryngologist for vocal concerns and referred for voice therapy comprised the study population. Outcomes were compared between those who were initially evaluated through the interdisciplinary voice clinic (IDC), which combined speech-language pathology and laryngology care, and those who were evaluated by a laryngologist alone. Adherence was measured by completion of the plan of care. RESULTS: There were 79 patients evaluated through the IDC and 100 patients…

Aphasia in Alzheimer's Disease and Other Dementias (ADOD): Evidence From Chinese.

Speech and language impairments (aphasia) are typical of patients with Alzheimer's Disease and other dementias (ADOD) and in some pathologies are diagnostic e.g. Primary Progressive Aphasia (PPA). One question concerns the reliability and validity of symptomatology across typologically different languages. A review of aphasia in ADOD across languages suggests a similar pattern of word comprehension, naming and word finding difficulties but also evidence of language specific features in symptomatology e.g. processing of tone in Chinese languages. Given differences in linguistic impairments acro

Assessment of speech sound disorders: Clinical experiences of Speech-language pathologists in Iran.

BACKGROUND: Speech-language pathologists (SLPs) in non-English speaking countries face challenges when assessing children for speech sound disorders (SSD). Exploring their clinical challenges in service delivery-along with their problem-solving approaches-may contribute to the development of instruments for clinical use in such settings. How do speech-language pathologists assess and treat spoken discourse after TBI? A survey of clinical practice.

BACKGROUND: Spoken discourse is commonly affected after traumatic brain injury (TBI). Although guidelines recommend prioritizing discourse-level skills in cognitive communication management, previous literature has highlighted challenges in managing discourse clinically. Little is known about how speech-language pathologists (SLPs) assess and treat discourse after TBI. AIMS: To investigate current SLP practice to determine the alignment of clinical practice with research evidence and recommendations. METHODS & PROCEDURES: This online survey consisted of 30 questions on SLPs' practice with di

South African speech-language therapists' opinion of their training in cleft lip and palate and craniofacial deformities.

BACKGROUND: Speech care of cleft lip and/or palate (CLP) and craniofacial deformities (CFD) is complex and lengthy and requires collaboration amongst different disciplines. Consequently, it is important to provide academic educational models that include didactics, online learning and clinical exposure in CLP and CFD treatment, and participation in established cleft palate multidisciplinary team management. OBJECTIVES: To obtain information regarding: (1) the perceived adequacy of CLP and CFD academic education of speech-language therapists (SLTs); (2) the professional services that SLTs offe

VoiceRelease.

VoiceRelease is a progressively structured treatment system that aims to release the voice through muscle tension reduction by manual therapy, thereby facilitating healthy voice production. The VoiceRelease treatment should closely interact with functional voice training and should therefore be carried out by the speech-language therapist (SLT). The aim of this paper is not to present the treatment in detail, but to describe the benefits of using the techniques in the voice clinic and to argue that laryngeal manual therapy complements the skills of the SLT and that it in a natural and integrat

Ask a silly question: two decades of troublesome trials.

BACKGROUND: Randomized control trials and the use of meta-analysis in systematic reviews are the basis of evidence-based practice. The paper reviews their use in the development of evidence-based practice in speech and language therapy. AIMS: It is accepted that clinical outcome research should develop in a sequence of phases. A model of this process is described. Examples of outcome research in speech and language therapy are used to illustrate the use of the model and the problems that result when it is not followed. MAIN CONTRIBUTION: Existing research has largely ignored the agreed proce

Why do speech and language therapists stay in, leave and (sometimes) return to the National Health Service (NHS)?

BACKGROUND: Research into recruitment, retention and return of speech and language therapists in the National Health Service (NHS) is relatively limited, particularly in respect of understanding the factors that drive employment choice decisions. AIMS: To identify what factors influence speech and language therapists working in the NHS to stay, and consider leaving, but not do so. To identify what features of the NHS and alternative employers influence speech and language therapists to leave the NHS. To identify why some speech and language therapists return to the NHS after working elsewhere

METHOD: In a web-based questionnaire 178 Swedish speech-language pathologists (SLPs) anonymously answered questions about their perception of typical speech characteristics for CAS. They graded own assessment skills and estimated clinical occurrence. RESULTS: The seven top speech characteristics reported as typical for children with CAS were: inconsis

Progressive oculo-orofacial-speech apraxia (POOSA).

A loss of speech can be related to disorders of the motor units (paresis), language deficits (aphasia), or speech programming deficits (apraxia of speech). Although apraxia of speech has been reported to be associated with degenerative diseases, we observed a patient with a unique constellation of signs that included apraxia of speech, oculo-orofacial apraxia and a supranuclear ophthalmoplegia in the absence of extrapyramidal (Parkinsonian) signs. Post-mortem examination revealed a loss of neurons in the frontal and temporal regions, but there was also a marked loss of neurons and astrogliosis

Early clinical phenotype of late onset Pompe disease: Lessons learned from newborn screening.

PURPOSE: Thoroughly phenotype children with late-onset Pompe disease (LOPD) diagnosed via newborn screening (NBS) to provide guidance for long-term follow up. METHODS: Twenty infants ages 6-21 months with LOPD diagnosed by NBS underwent systematic clinical evaluation at Duke University including cardiac imaging, biomarker testing, physical therapy evaluation, and speech-language pathology evaluation. RESULTS: Of the 20 infants, four were homozygous for the "late-onset" IVS1 splice site variant c.-32-13 T > G, fourteen were compound heterozygous, and two did not have any copies of this varian…

[Pediatric dysphasia. 1. The concept and clinical aspects].

This survey deals with the early diagnosis and treatment of children with developmental dysphasia, which may prevent the progression of learning and behavior disorders. In the pre-verbal and early verbal stage, the severity of the clinical picture is primarily determined by concomitant motor pathology (motor dysfunction, dysarthria, general and oral dyspraxia) and by receptive pathology (hearing and auditory perception). In the verbal period, linguistic problems start to play a role, and often combine with oral motor symptoms to present a mixed picture. Various language syndromes do not become

Student perspectives of simulated learning to improve their dysphagia management.

BACKGROUND: The use of simulation to enhance knowledge translation and bridge the theoretical-clinical gap to enhance clinical training and competency in health professions has received mixed reviews in the literature. OBJECTIVES: This research examined student perspectives of a simulation laboratory in speech therapy to improve students' clinical competency when working with adults with communication and dysphagia impairments. METHOD: An exploratory descriptive pilot study was conducted in 2022 with 16 third-year speech-language therapy students. This mixed-methods study involved students c
