Nonmetric Variants of Anatolian Crania: A Preliminary Study.

Analysis of nonmetric cranial variants has been essential for identifying the human population through osteologic analysis and genetic affinities. This study aimed to examine the nonmetric cranial variants to evaluate differences among sex and side correlations in Anatolian dry skulls. This study was carried out on 50 Anatolian adult dry human skulls (22 males, 28 females) with suitable features (nonfractured and/or with necessary features) out of a total of 97 dry skulls. After sex analysis, each skull was photographed from the norma frontalis, lateralis, inferior, and superior. Eleven nonmet

Unilateral isolated frontosphenoidal craniosynostosis causing frontal plagiocephaly.

Synostotic frontal plagiocephaly is most commonly caused by frontoparietal (unicoronal) synostosis, but may be caused by other fusions along the coronal hemiring. Frontosphenoidal synostosis is a rare cause of frontal plagiocephaly, with only five previously reported cases. We present the case of an 8-month-old male infant with frontal plagiocephaly caused by unilateral isolated frontosphenoidal synostosis mimicking frontoparietal (unicoronal) synostosis. The clinical and radiological features included flattened ipsilateral forehead, retruded ipsilateral maxilla, a midline chin, contralateral

Diffuse Calvarial Hyperstosis.

Calvarial hyperstosis can be an idiopathic benign finding or secondary to a metabolic pathology. We herein describe a case of diffuse calvarial hyperstosis. A 26-year-old man known to have end-stage renal disease on regular hemodialysis, tertiary hyperparathyroidism, extensive brown tumors, and severe developmental impairment with skeletal deformities was referred to us for macrocephaly. On examination, the patient was chairbound, with speech and motor developmental delay, and frontal bossing. Brain computed tomography revealed diffuse hyperstosis of the calvarium and facial bones expansion wi

Basilar meningitis associated with ethmoid and sphenoid cephaloceles.

Nontuberculous causes of basilar meningitis are rare. This study presents the case of a male who developed fever and meningitis caused by Streptococcus pneumoniae. He developed multiple cranial nerve palsies and imaging findings consistent with basilar meningitis and ventriculitis. Computed tomographic scans of the floor of the anterior fossa were performed after the detection of a cephalocele on magnetic resonance imaging. This imaging revealed a defect in the cribiform plate and fovea ethmoidalis with a large nasoethmoidal cephalocele. There was a second separate defect and cephalocele invol

Unilateral fusion of the frontosphenoidal suture: a rare cause of synostotic frontal plagiocephaly.

Unilateral coronal synostosis is the common appellation for premature, one-sided fusion of the frontoparietal suture-the most common cause of synostotic frontal plagiocephaly. However, frontal asymmetry can also result from isolated fusion across the anterior cranial base without involvement of the frontoparietal suture. This article describes three patients with localized synostosis of the frontosphenoidal suture, the medial extension of the coronal ring. Two patients were initially misdiagnosed as having unilateral coronal synostosis and the other as having deformational frontal plagiocephal

Cholesteatoma of the clivus.

Cholesteatomas (central nervous system epidermoids) can be found intradurally or extradurally in the central nervous system. Extradural intraosseous lesions are most commonly found in the petrous bone. The authors describe a unique case of a clival cholesteatoma in a 64-year-old woman who presented with headaches. No other neurological complaints or physical examination findings were noted. Magnetic resonance imaging showed an expansile lesion centered in the middle portion of the clivus. A large portion of the clivus was eroded. The lesion was explored via a transnasal trans-sphenoidal approa

Sphenoid sinus chondromyxoid fibroma mimicking a mucocele.

We report the case of a 44-year-old man who presented with a chondromyxoid fibroma (CMF) of the sphenoid sinus, which filled the sinus and clinically and radiologically resembled a mucocele. Chondromyxoid fibromas are the least common cartilaginous neoplasms of bone, typically occurring in the metaphysis of long bones. They have occasionally been described in the facial bones, usually the mandible and maxilla, and rarely involve the paranasal sinuses. Chondromyxoid fibroma displays a relatively unique histological appearance, with characteristic lobulation, with spindle to stellate cells embed

Multidisciplinary approach for management of a patient with fibrous dysplasia of maxilla.

Fibrous dysplasia is a non-neoplastic hamartomatous developmental fibro-osseous lesion of bone. Monostotic fibrous dysplasia is more common than the polyostotic form and usually involves jaw bones, ribs and femur. Maxillary lesions may extend to involve the maxillary sinus, zygoma, sphenoid bone and floor of the orbit and require surgical intervention resulting in an acquired defect of the involved site. A multidisciplinary team approach involving an oral pathologist, oral surgeon, oral medicine expert and maxillofacial prosthodontist is required for successful treatment and rehabilitation of …

Surgical removal of giant acoustic neurinomas involving the skull base. Report of two cases.

Neurinomas arising from the peripheral branch of the acoustic nerve distal to the internal auditory canal in the temporal bone are rare. Two advanced skull-base neurinomas are described which were situated mainly in the temporal petrous bone, and extended to the parapharyngeal space anteriorly, to the lateral cervical portion inferiorly, into the sphenoidal sinus medially, and into the middle and posterior cranial fossae compressing the brain stem. Both patients had been deaf for several years without other neurological deficits. The operative findings revealed that the fifth, seventh, and cau

Giant cell tumour and central giant cell reparative granuloma of the skull: do these represent ends of a spectrum? A case report and literature review.

Giant cell tumour (GCT) of bone is an uncommon primary bone neoplasm typically occurring at the epiphyses of long bones in young adults. They are osteolytic neoplasms with approximate local recurrence rates of 25%, and 2% of patients develop pulmonary metastases. These tumours appear very rarely in the skull, with those few reported cases arising predominantly in the sphenoid and occasionally the temporal bones. They demonstrate benign histological features, but are locally aggressive and surgical excision is the treatment of choice. It is widely believed that giant cell tumours should be dist

The sphenoid sinus, foramen rotundum and vidian canal: a radiological study of anatomical relationships.

INTRODUCTION: The sphenoid sinus is an important structure in ventral skull base surgeries that is surrounded by several vital anatomical structures including the internal carotid arteries, optic nerve and cranial nerves inside the cavernous sinus. In addition, the foramen rotundum is a small canal deeply situated in the base of the skull, which represents the way for exit of the maxillary nerve. Understanding of the sphenoid bone anatomical relationships is central to the expanded endonasal approaches to the skull base. OBJECTIVE: To record and analyze the measurement indexes of the sphenoid…

Expanded endonasal endoscopic approach for resection of a large skull base aneurysmal bone cyst in a pediatric patient with extensive cranial fibrous dysplasia.

Aneurysmal bone cysts (ABCs) are uncommon non-neoplastic, hemorrhagic, and expansile osseous lesions. These lesions most commonly occur in the first two decades of life and affect the long bones and spinal column. Skull base involvement is rare. The authors report the case of a 16-year-old boy who presented with acute visual decline and was found to have a large skull base ABC centered in the sphenoid sinus. In addition, the patient had extensive cranial fibrous dysplasia. The patient underwent a staged expanded endonasal endoscopic approach for complete resection of this lesion with excellent

Craniopharyngioma arising in the pharyngeal hypophysis.

Four cases of nasopharyngeal craniopharyngiomas have been previously reported in the medical literature. They were, in fact, only nasopharyngeal extensions of tumors originating in the anterior pituitary of the sella turcica. Pituitary adenomas can arise from any part of the craniopharyngeal canal. Supporting this theory are four reported cases of pituitary adenomas in the body of the sphenoid bone separate from both the sella turcica and the nasopharynx. The discovery by Erdheim in 1904 of the pharyngeal hypophysis located on the posterior edge of the vomerine bone raises the possibility of a…

Optic nerve compression due to aneurysmal bone cyst.

A 10-year-old boy developed loss of central vision in both eyes due to compression of the optic nerves by a mass arising from the sphenoid and ethmoid sinuses. Histopathologic examination of biopsy specimens showed a fibrous matrix composed of spindle-shaped cells interspersed with small and large channels, characteristic of an aneurysmal bons cyst. One year after partial excision of the intracranial and extracranial portions fo the lesions, vision had returned to nearly normal levels. Aneurysmal bone cysts rarely involve the orbits or other cranial bones to produce ocular signs and symptoms.
