The non-functioning or dysfunctional sinus is completely isolated from the remainder of the nasal cavity with no hope of normal ventilation despite the most aggressive medical therapy. Most often these sinuses are the result of mucosal stripping/removal during prior radical surgeries. The reason for these radical operations include treatment of neoplasm, but most often is for revision of inflammatory disease when prior procedures have not been successful at restoring ventilation and maintaining patent ostia. When faced with a dysfunctional sinus, rhinologists typically have two choices: repeat

Schwannoma is a benign tumor arising from the sheath of myelinated nerve fibers and may occur in any part of the body. Ancient schwannoma, a variant of schwannomas, in the head and neck region is uncommon, and its occurrence in the nasal cavity and paranasal sinuses is extremely rare with only one case reported to date. Herein, we describe a rare case of sinonasal ancient schwannoma. The tumor arose from the nasal septum and extended to the sphenoid sinus. It was successfully treated by endoscopic sinus surgery, and the patient showed no evidence of recurrence 24 months postoperatively.

A fatal rhinocerebral zygomycotic infection caused by Saksenaea vasiformis in a 71-year-old man was diagnosed based on the presence of broad, infrequently septate, branched, hyaline hyphae in tissue obtained from the right and left base of the skull, soft tissue, both maxillary sinuses and the sphenoid sinus; isolation of S. vasiformis from the tissue; and demonstration of mucoraceous antibodies by enzyme-linked immunosorbent assay. Fluorescent antibody studies carried out with a Rhizopus arrhizus conjugate, stained hyphal fragments in tissue with a 2+ intensity.

BACKGROUND: Spontaneous sphenoid sinus cerebrospinal fluid (CSF) encephaloceles have been postulated to arise from a persistent Sternberg's canal. However, recent evidence has questioned this embryological etiology. We examined the anatomic location of a series of lateral sphenoid sinus encephaloceles to determine if they corresponded with the location of Sternberg's canal.
METHODS: We queried a prospectively acquired database of surgically treated spontaneous CSF leaks and identified those arising from the sphenoidal sinus. Images were reviewed to characterize the leaks with respect to the f

A 51-year-old woman with headache was referred to our hospital. She had a history of endonasal pituitary surgery 22 years prior and hypertension 10 years prior. The endonasal transsphenoidal pituitary approach was performed via microscopic transseptal approach. Nasal endoscopy revealed whitish cystic wall protruded from the right sphenoid sinus. Functional endoscopic sinus surgery was performed under general anesthesia. After incision of the right sphenoid ostium, yellowish mucoid discharge in the right sphenoid sinus was pushed out from the sphenoid sinus. Histopathology confirmed fungal ball

BACKGROUND: Anomalies in the anatomical structure of the nasal cavity and paranasal sinuses often serve as a potential cause of spontaneous cerebrospinal fluid (CSF) leakage and may result in the development of a meningoencephalocele. In this report, the authors present a case of surgically treated intrasphenoidal meningoencephalocele attributed to the persistence of the lateral craniopharyngeal canal, which was further complicated by the occurrence of an intracerebral hematoma.
OBSERVATIONS: A temporal lobe meningoencephalocele located in the lateral recess of the sphenoid sinus was successf

We reported a case of cavernous sinus aspergillosis. A 62-year-old man complained of trigeminal neuralgia in the right V1 region. Neurological examination on admission showed ptosis, loss of light reflex and ophthalmoplegia externa in the right side. MRI enhanced with gadolinium demonstrated sphenoid sinusitis and mass lesion in the right cavernous sinus. MRA revealed right internal carotid artery occlusion. An open biopsy using the extradural temporopolar approach was performed. Pus discharge was observed from the cavernous sinus and histological examination showed hypha of Aspergillus. With …

Penetrating injuries of the paranasal sinuses due to foreign bodies are rare, especially as a result of a traffic accident. Here we report a patient with a ballpoint pen lodged in his left eye following a traffic accident. The pen extended from the medial aspect of the left orbit, through the left ethmoid sinus and the nasal septum, to the right sphenoid sinus. We removed the pen uneventfully using endoscopic sinus surgery. There are no similar reports in the English literature and we therefore present this case because of its rarity.

Eleven cases are chosen to illustrate the differential diagnosis of deforming lesions of the bony orbit and its vicinity, which cause unilateral proptosis or other cranio-facial disfigurement. Three distinct varieties of orbito-cranial neurofibromatosis are distinguished, and they are contrasted with other deforming lesions of childhood--unilateral craniosynostosis, chronic juvenile subdural hygroma of the temporal fossa and congenital facial hemihypertrophy. Adult lesions are represented by intrinisic tumours of the orbit and paranasal sinuses, and by the commonest two processes deforming the

Fourteen cases of mucocele of the sphenoid sinus are reviewed to assess differentiating radiographic features. Recognition is usually possible using detailed tomography in the frontal, lateral, and submentovertex projections. Two constant findings were opacification of the sinus (usually bilateral) and expansion and thinning of the sinus walls. Contiguous opacification of the ethmoid sinuses was present in most cases, and pansinusitis was frequent. Distinction from intrasellar lesions is based on the absence of sellar enlargement. Early and correct diagnosis is important to prevent permanent v

BACKGROUND: Endoscopic vidian neurectomy has been performed for decades on patients with refractory allergic rhinitis and vasomotor rhinitis. Although the surgical approach has evolved continuously, it still has deficiencies in terms of accuracy and safety. To overcome these deficiencies, we conducted anatomical study to propose a modified endoscopic vidian neurectomy. METHODS: Five adult dry skulls and seven fresh skull specimens were used to identify landmarks for locating the vidian nerve. The modified surgical approach was applied to 9 patients with allergic or vasomotor rhinitis to test i

Capillary hemangiomas are benign vascular tumors usually diagnosed in infancy and involving skin and soft tissues. We report a rare case of an adult capillary hemangioma of the left sphenoid sinus extending into the intrasellar and parasellar regions. A 71-year-old woman presented with complaints of headaches and diplopia. Magnetic resonance imaging revealed a T2-hyperintense mass-like lesion involving the left posterior ethmoid and sphenoid sinuses and left cavernous sinus, that had extended into the intrasellar and parasellar regions. Dynamic contrast-enhanced computed tomography revealed en

OBJECTIVES: This study aimed to investigate the prevalence of optic nerve protrusion (ONP) and its clinical indicators by using sinus computed tomography (CT) scan.
STUDY DESIGN: Sinus CT scans of 260 consecutive patients with chronic inflammatory sinus disease were reviewed.
RESULTS: The prevalence of ONP in our study population was 28%. Nineteen percent of the optic nerves protruded into the sphenoid sinuses including indentation of the sinus wall (12%) and coursing through the sphenoid sinus (8%). In the presence of contralateral ONP and/or ipsilateral anterior clinoid process pneumatizat

Accurate segmentation of the paranasal sinuses, including the frontal sinus (FS), ethmoid sinus (ES), sphenoid sinus (SS), and maxillary sinus (MS), plays an important role in supporting image-guided surgery (IGS) for sinusitis, facilitating safer intraoperative navigation by identifying anatomical variations and delineating surgical landmarks on CT imaging. To the best of our knowledge, no comparative studies of convolutional neural networks (CNNs), vision transformers (ViTs), and hybrid networks for segmenting each paranasal sinus in patients with sinusitis have been conducted. Therefore, th

A 35-year-old woman presented with a headache lasting 2 weeks and complained of recent diplopia. Neurologic examination showed limited left eye abduction. Initial brain CT scan was considered normal (figure, A). Brain MRI showed left sphenoiditis (figure, B). The patient was treated with amoxicillin/clavulanic acid and corticosteroids with rapid symptom improvement which deferred endoscopy that could provide a bacteriologic diagnosis.
