Hemivertebra resection in the cervical spine.

STUDY DESIGN: Retrospective study with clinical and radiologic evaluation of hemivertebra resection in children with congenital cervical scoliosis. OBJECTIVE: Assessment of hemivertebra resection in the treatment of congenital cervical scoliosis. SUMMARY OF BACKGROUND DATA: To our knowledge, this is the first report on hemivertebra resection in the midcervical spine. METHODS: Three patients with torticollis due to a cervical hemivertebra were operated on by hemivertebra resection and fusion of the adjacent vertebrae. Resection was performed by a posterior-anterior (-posterior) approach. Mea…

Aneurysmal bone cyst of the spine.

The authors present the case of an 11-year-old boy with a painful, rapidly expanding lesion in the posterior triangle of the neck. There was no history of cervical trauma. Computerized tomography of the neck revealed a unicameral (single-chambered) aneurysmal bone cyst involving the C3 vertebra. Treatment was by open resection and curettage; no recurrence was seen at six months. We discuss the natural history, differential diagnosis, radiographic appearance and treatment modalities for this unusual, benign, expanding, osteolytic lesion containing blood-filled cystic cavities.

Tuberculosis of the spine and spinal cord.

Tuberculosis remains an important problem in underdeveloped countries, especially in individuals under the age of 20 years. In developed countries, there was an increase of the disease due to several factors, especially the appearance of A.I.D.S. The arterial haematogenous seeding is the most important way to the propagation of the disease to the rachis. The bacillus mycobacterium tuberculosis is characterized by a slow growth, propention for an oxygen rich environment and the absence of proteolytic enzymes. These characteristics are responsible for the imaging particularities of tuberculous o

Natural history of the aging spine.

The unrelenting changes associated with aging progressively affects all structures of the spinal units. The degenerative process starts early during the first decade of life at the disc level. Discal degeneration is associated with biochemical changes followed by macroscopic alterations including tears and fissures, which may lead to discal herniation, the main cause of radiculopathy in the young adult. Moreover, nociceptive nerve fibers have been demonstrated in degenerated discs. They may be a source of nociception and of pure low-back pain. Facet joint changes are usually secondary to disca

Postoperative wound infections of the spine.

Postoperative spinal wound infections occur in 1 to 12% of patients. The rate of infection is related to the type and duration of the procedure, comorbidities, nutritional status, and various other risk factors. Antibiotic prophylactic therapy has been clearly shown to decrease the rate of infection dramatically after lumbar surgery. These infections typically manifest with signs and symptoms of wound swelling, erythema, and drainage. Laboratory-detected values such as the erythrocyte sedimentation rate and C-reactive protein can be elevated beyond what is normal for the uncomplicated postoper

Imaging of the spine in sports medicine.

Much controversy exists surrounding the utility of plain film, computed tomography, magnetic resonance imaging (MRI), and bone scintigraphy in the evaluation of sports-related spine injuries. The articles reviewed here offer several different perspectives. Cervical spine radiography, return-to-play criteria following cervical trauma, lumbar pain and pars injuries, utility of MRI in acute sports injuries, and sports-specific injuries are reviewed. The relationship between bone mineral density and sports activity is also reviewed.

Osteochondroma of the thoracic spine and scoliosis.

STUDY DESIGN: The case of a 16-year-old patient with an osteochondroma in T11 and scoliosis is reported. OBJECTIVE: To describe the treatment of an osteochondroma with scoliotic deformity and the imaging methods used for the diagnosis. SUMMARY OF BACKGROUND DATA: Osteochondromas arising in the vertebral column are rare. However, spinal involvement is found with some regularity because osteochondromas are among the most common benign tumors of bone. METHODS: The clinical history, plain radiographs, computed tomography, and magnetic resonance imaging, and pathologic findings of the reported p

[Tuberculous spondylitis of the cervical spine].

WHO estimates that worldwide more than 4 million people are infected with tuberculosis (TB), 95% of them living in third world countries. TB is once again the most frequent infectious disease [1, 2, 7]. Extrapulmonary forms (EPTB) frequently appear with HIV-associated TB [3, 4, 6]. We present the case of a 23-year old patient with abscess forming TB of the right apical lung with infiltration and partial osteolytical destruction of the 7th cervical- and the first thoracal vertebral body. Progressive spinal compression lead to partial sensomotorical deficits. Surgical revision of the abscess, an

Management of metastatic disease of the spine.

Accurate assessment of spinal cord stability, pain, and neurologic function is essential to any rational approach to metastatic disease of the spine. The Harrington classification is useful in terms of selecting treatment options and planning appropriate interventional and adjuvant therapy. For milder cases, with bone involvement in the absence of structural deformity, adjuvant therapy usually is appropriate. In cases of bone collapse, with or without neurologic compromise, surgical consideration should be given to patients with appropriate life expectancy. Surgical principles revolve around a…

Galen: a pioneer of spine research.

Galen of Pergamum AD (2nd century), the most eminent Greek physician after Hippocrates, marked the history of medicine for more than 14 centuries. His doctrines, expressed in his voluminous work, combined the medical heritage of the Hippocratic, the Alexandrian, and some of the most important medical schools of antiquity. The strong influence of the Hippocratic tradition can characteristically be traced in orthopaedics and particularly in Galen's presentation of the spine. Based on his observations, derived from dissection and vivisection of animals, Galen established a pioneer model for the s…

Aneurysmal bone cysts of the spine.

Thirteen patients with aneurysmal bone cyst of the spine (excluding sacral lesions) were retrospectively reviewed. Treatment for aneurysmal bone cysts remains controversial, but surgical resection, irradiation, and embolization are common treatment modalities for those involving the spine. Of 102 patients with aneurysmal bone cysts, 15 had a lesion of the spine, including 2 sacral cases. Of the 13 patients with a lesion of the thoracic or lumbar spine, 9 underwent resection of the lesion, 2 curettage and cementation, and 2 only currettage. Eleven patients underwent segmental arthrodesis with i…

CONCLUSION: Juxtafacet cysts seem to be a degenerative change of the cervical spine rather than a traumatic event. Similar to their counterparts in the lumbar spine, they tend to arise in segments with increased mobility. Surgical treatment is effective.

Treatment of metastases of the cervical spine.

On the experience of 73 patients, the authors state their guidelines on the treatment of bone metastases on the cervical spine. Most of the cases on which no vertebral collapse occur neither neurological deficit, radiation therapy and external support are suggested. Surgery is necessary on case of severe bone destruction, collapse with or without subsequent neurological impairment. Anterior excision is considered the best approach, sometimes complemented by posterior stabilisation.

Internal fixation in the osteoporotic spine.

The presence of osteoporosis in patients who are candidates for spinal surgery can affect preoperative planning. These patients may not need instrumentation for a successful surgical result, but if use of instrumentation is indicated because of instability or deformity, certain principles should be observed. These include using multiple sites of fixation, accepting lesser degrees of deformity correction, and avoiding ending the instrumentation within kyphotic segments. Advances in perioperative medical management, as well as improved instrumentation systems, may also contribute to improving pa

Surgery of the spine in myelodysplasia. An overview.

Significant spinal deformity is particularly common in nonambulatory patients with myelodysplasia. Progressive deformity may be caused by congenital anomalies, paralytic collapse, hip contractures, or spinal cord tethering. Existing or projected functional impairment should be the principle indication for treatment. Surgical treatment is complicated by poor soft tissue coverage, associated contractures, lack of sensation, weak bone, and absence of posterior elements. Successful fusion can be achieved by circumferential (anterior and posterior) fusion and current rigid segmental instrumentation
